Predictors for Implant Rupture in Two-Stage Tissue Expander-Based Breast Reconstruction: A Retrospective Cohort Study.

BACKGROUND: Two-stage tissue expander/implant-based method has been used predominantly for breast reconstruction. Implant rupture is one of the bothersome complications, inducing additional morbidity including reoperation. The present study aimed to identify the independent factors associated with development of implant rupture. PATIENTS AND METHODS: Patients who underwent immediate two-stage prosthetic breast reconstruction between 2010 and 2016 were reviewed. Inserted implants were followed up using magnetic resonance imaging every 2 years and/or ultrasound/computed tomography scans every 6

Optimization of In-Continuity Spring-Mediated Intestinal Lengthening.

BACKGROUND: Spring-mediated intestinal lengthening has been studied in numerous animal models to effectively achieve up to a 3-fold increase in length. In this study we are interested in optimizing this method of spring lengthening. METHODS: Juvenile mini-Yucatan pigs underwent laparotomy for spring implantation. Springs were secured by plicating the intestine around the springs. In one set of experiments, varying degrees of plication were compared to determine the necessary narrowing needed to confine the spring. In another set of experiments, dissolvable sutures were used for the plication

Cleft palate repair with the use of osmotic expanders: a preliminary report.

A new method of cleft palate repair by expansion of tissue by means of osmotic expanders implanted in the first stage of treatment is described. Self-expanding expanders manufactured by OSMED (Ilmenau, Germany) were implanted under the mucoperiosteal layer of the hard palate, on purpose to generate more tissue and provide facility for palate repair performed 24-48h later. Nineteen children aged from 2 to 3 years were operated from January 2004 to 15 April 2005. In clefts<10mm, tissue repair was possible without relaxing incisions. In 11 patients with clefts>10mm, cleft palate repair was more d

The true meshing ratio of skin graft meshers.

Skin meshing is frequently used in the coverage of extensive burn injuries as well as other skin and soft tissue wounds. This technique allows coverage of more extensive areas with smaller donor sites and prevents fluid from collecting beneath the skin grafts. The devices used to achieve this expansion differ in their technology and the use of skin carriers. In addition, many of the devices permit meshing at single or multiple ratios depending upon the device chosen. Although commonly used, there have been few definitive studies analyzing the actual expansion ratios achieved by many of these d

The Role of Tissue Expansion Before Baclofen Pump Insertion in the Pediatric Population.

BACKGROUND: Tissue expansion is a well-established technique for soft tissue reconstruction in the pediatric population. We present a case series of this technique to create a safe pocket for baclofen pump insertion to minimize risk of complications including pump migration, extrusion, wound dehiscence and infection. METHODS: A case series of 3 pediatric patients undergoing tissue expansion prior to baclofen pump insertion at a single center in Melbourne. The expansion procedure was performed by senior surgeon N Low in all cases, and patients followed up 6 months after expander-pump exchange.

Management of the complications of tissue expansion during a 5-year period (2005-2010).

We report our experience of using tissue expansion where we deal particularly with complications and their management. Forty patients had tissue expanded during a 5-year period (2005-2010). Indications included reconstruction of a scar (after a burn, after injury, or postoperative), congenital naevi, microtia, and breast reconstruction after mastectomy. Of the 50 expanders inserted, complications occurred in 12 (10 patients). Exposure and perforation of the expander were the most common complications, followed by infection, seroma, and local pain. In all cases, complications were managed succe

Multimodality imaging of breast tissue expanders: types, MRI safety and complications.

Mastectomy remains a common surgical treatment for breast cancer, often followed by breast reconstruction to mitigate the psychological impact. Among reconstruction techniques, the use of tissue expanders is a safe, cost-effective, and widely adopted method, typically involving a two-stage surgical approach. These devices gradually stretch the skin and tissues post-mastectomy to prepare for permanent implant placement. This review outlines the imaging characteristics of breast tissue expanders, emphasizing their types-based on positioning, lumen configuration, and valve design-and associated c

Effect of Operative Plane on Anesthetic Requirements in Tissue Expander Breast Reconstruction Patients.

BACKGROUND: Initially subpectoral, the plane of choice for tissue expander placement in breast reconstruction has evolved over the past decade, with more surgeons moving to prepectoral placement. In making this transition, our institution noted a subjective decrease in the anesthetic requirements of patients undergoing the second stage of their prepectoral reconstruction. Despite this, there is little published literature on the topic. Therefore, our study aims to examine the differences in anesthetic requirements among patients with subpectoral versus prepectoral expanders at the time of exch

Rapid intraoperative tissue expansion with Foley catheter in a challenging cripple Hypospadias.

Failed hypospadiass cases may result in hypovascular, scarred penis with residual penile chordee and leave the patient with minimal residual skin for penile resurfacing and urethroplasty. Local tissue expansion has become a good alternative to provide skin for penis by using expanders however they require long periods of time for expansion. Besides, rapid tissue expansion was also described in different tissues. We used rapid intraoperative expansion technique by using a Foley catheter in a failed hypospadias case who had minimal residual skin secondary to infection and we concluded that rapid

Use of self-expanding, hydrophilic osmotic expanders (hydrogel) in the reconstruction of congenital clinical anophthalmos.

PURPOSE OF REVIEW: Rehabilitation of the congenitally anophthalmic orbit is frustrating to both the parents and physician. Traditional methods involve using progressively enlarging static acrylic conformers to expand the conjunctival socket, followed by placement of conventional static spherical orbital implants, dermis-fat grafts, or inflatable balloon expanders for orbital enlargement. Limitations of these methods typically result in less-than-optimal cosmetic outcomes, with retardation of bony orbital and overlying soft tissue growth adversely affecting midfacial growth and symmetry. Expanded occipito-cervico-pectoral flap for reconstruction of burned cervical contracture.

Postburn neck contracture and hypertrophic scarring can cause functional limitation and aesthetic disfigurement. Reconstruction of severe deformities and scar of neck following healing from burns confronts the surgeon with some of the most challenging problems in reconstructive surgery. Through knowledge of available reconstructive technique accurate diagnosis of tissue deficiency and secondary distortion, imaginative planning and definitive, careful execution of ones surgical plan are the bare minimum items for achieving improvement in a burned deformed neck. Radiation testing of the AeroForm CO2-based breast tissue expander implant.

BACKGROUND: Tissue expanders are used in breast reconstruction after mastectomy to stretch the remaining tissue to create space for placement of permanent breast implants. The AeroForm™ Tissue Expander, developed by AirXpanders™ Inc., contains electronic components designed to activate the release of carbon dioxide from an internal reservoir to inflate the expander. Breast cancer patients who undergo mastectomy and tissue expander/implant-based breast reconstruction may require radiation therapy at doses up to 50-60 Gy while the expander is in place. The ionizing radiation used in postmastecto

Basic fibroblast growth factor eluting microspheres enhance distraction enterogenesis.

PURPOSE: The purpose of this study was to determine if distraction enterogenesis using self-expanding polycaprolactone (PCL) springs is a potential therapy for short bowel syndrome. Sustained release basic fibroblast growth factor (bFGF) microspheres have been shown to induce angiogenesis and intestinal regeneration in tissue engineered scaffolds. We hypothesized that the provision of bFGF-loaded microspheres would increase angiogenesis and thereby enhance the process of enterogenesis. METHODS: A 10-mm segment of rodent jejunum was isolated and an encapsulated PCL spring inserted. Blank or bF

Outcomes of Air Versus Saline-filled Breast Expanders: A Systematic Review and Meta-analysis.

BACKGROUND: Among the possible implant-based reconstructive strategies, the two-stage tissue expander-to-implant procedure is one of the most common options in patients not ideal candidate to direct-to-implant reconstruction. Recently, other filling options such as air or carbon dioxide (CO2) have been reported as alternative fill media than saline for tissue expansion. The aim of this systematic review was to qualitatively and quantitatively synthetize the available evidence on the topic. METHODS: A systematic review and meta-analysis were conducted, and they were reported according to PRISM

Post-mastectomy Breast Reconstruction With Gas vs Saline Tissue Expanders: Does the Fill Type Matter?

The most common reconstruction technique following mastectomy is a 2-stage technique that involves tissue expansion followed by definitive implant-based reconstruction (IBR). Tissue expanders (TEs) have classically used saline for initial fill; however, TEs with an initial gas fill (GTE)-including the CO2-based AeroForm (AirXpanders, San Francisco, CA) TE and TEs initially filled with atmospheric air-have been increasingly used in the past decade. We aimed to compare the outcomes in breast reconstruction for tissue expanders initially filled with saline vs gas. PubMed was queried for studies c
