Internal evaluation of risk stratification tool using serial procalcitonin and clinical risk factors in pediatric febrile neutropenia: The non-interventional, single institution experience prior to clinical implementation.

Risk stratification of pediatric febrile neutropenia (FN) is an established concept, yet clinical risk tools misclassify nearly 5% of clinical standard-risk episodes with severe outcomes. The internal evaluation of a clinical risk tool before implementation has not been well-described. In this noninterventional cohort study, we evaluated a study decision rules (SDR) tool; a clinical risk tool with serial procalcitonin. The study standard-risk (SSR) group met clinical standard-risk criteria with two serial procalcitonin <0.4 ng/mL. The study high-risk (SHR) group met clinical high-risk criteria

Patient-derived assessment tool using musculoskeletal ultrasound for validation of haemarthrosis.

INTRODUCTION: Haemophilia patients experience painful joint episodes which may or may not be associated with haemarthrosis. We sought to validate a questionnaire developed by the Canadian Haemophilia Society using point-of-care musculoskeletal ultrasound (POC MSKUS) to confirm haemarthrosis. METHODS: The questionnaire comprised of 20 questions (10 each associated with haemarthrosis and arthritis pain) and was administered to adult haemophilia patients reporting to the Haemophilia Treatment Centre (University of California San Diego). We confirmed the presence (or absence) of haemarthrosis usi

Tools and talk: an evolutionary perspective on the functional deficits associated with amyotrophic lateral sclerosis.

We propose that amyotrophic lateral sclerosis (ALS), and frontotemporal dementia may be viewed as a failure of interlinked functional complexes having their origins in key evolutionary adaptations. We discuss how hand-arm function, locomotion, brainstem function (involving vocalization/speech, swallowing and breathing), and cognitive impairment share complex, interdependent evolutionary adaptations that can be traced back several million years. Fine movements of the hand facilitated tool-making and use enhanced by development of bipedalism. Development of the larynx and integration of respirat

Imitation of hierarchical structure versus component details of complex actions by 3- and 5-year-olds.

We investigated developmental changes in the level of information children incorporate into their imitation when a model executes complex, hierarchically organized actions. A total of 57 3-year-olds and 60 5-year-olds participated, watching video demonstrations of an "artificial fruit" box being opened through a complex series of nine different steps. Half of each sample observed the same nine steps performed through either of two different, hierarchically organized procedures, whereas half witnessed differing component action details. Children were found to imitate at both levels but were mor

To Watch is to Work: a Review of NeuroImaging Data on Tool Use Observation Network.

Since the discovery of mirror neurons in the 1990s, many neuroimaging studies have tackled the issue of action observation with the aim of unravelling a putative homolog human system. However, these studies do not distinguish between non-tool-use versus tool-use actions, implying that a common brain network is systematically involved in the observation of any action. Here we provide evidence for a brain network dedicated to tool-use action observation, called the tool-use observation network, mostly situated in the left hemisphere, and distinct from the non-tool-use action observation network.

The development of tool manufacture in humans: what helps young children make innovative tools?

We know that even young children are proficient tool users, but until recently, little was known about how they make tools. Here, we will explore the concepts underlying tool making, and the kinds of information and putative cognitive abilities required for children to manufacture novel tools. We will review the evidence for novel tool manufacture from the comparative literature and present a growing body of data from children suggesting that innovation of the solution to a problem by making a tool is a much more challenging task than previously thought. Children's difficulty with these kinds

What to do and how to do it: sequence learning of action effects and transformation rules.

Many goal-oriented actions, for example in tool use, can be characterized by the involvement of two components: The effect that the acting person wants to achieve and the transformation rule that defines the relationship between a bodily movement and the associated action effect. Both in conjunction specify the concrete action that has to be executed. In our experiments, we utilized a sequence learning paradigm to investigate whether these components are represented separately or are bound together to form a more holistic representation of the action. Participants switched between different tr

Apraxia and Alzheimer's disease: review and perspectives.

Apraxia is one of the cognitive deficits that characterizes Alzheimer's disease. Despite its prevalence and relevance to diagnosing Alzheimer's disease, this topic has received little attention and is without comprehensive review. The review herein is aimed to fill this gap by first presenting an overview of the impairment caused in different clinical situations: pantomime of tool use, single tool use, real tool use, mechanical problem solving, function and manipulation knowledge tasks, and symbolic/meaningless gestures. On the basis of these results, we then propose alternative interpretation

Thalamic Inputs to Posterior Parietal Cortical Areas Involved in Skilled Forelimb Movement and Tool Use in the Capuchin Monkey.

The posterior parietal cortex (PPC) is a central hub for the primate forebrain networks that control skilled manual behavior, including tool use. Here, we quantified and compared the sources of thalamic input to electrophysiologically-identified hand/forearm-related regions of several PPC areas, namely areas 5v, AIP, PFG, and PF, of the capuchin monkey (Sapajus sp). We found that these areas receive most of their thalamic connections from the Anterior Pulvinar (PuA), Lateral Posterior (LP) and Medial Pulvinar (PuM) nuclei. Each PPC area receives a specific combination of projections from these

Distinguishing butchery cut marks from crocodile bite marks through machine learning methods.

All models of evolution of human behaviour depend on the correct identification and interpretation of bone surface modifications (BSM) on archaeofaunal assemblages. Crucial evolutionary features, such as the origin of stone tool use, meat-eating, food-sharing, cooperation and sociality can only be addressed through confident identification and interpretation of BSM, and more specifically, cut marks. Recently, it has been argued that linear marks with the same properties as cut marks can be created by crocodiles, thereby questioning whether secure cut mark identifications can be made in the Ear

Tool use and related errors in ideational apraxia: the quantitative simulation of patient error profiles.

The behaviour of ideational apraxic patients on simple tasks involving multiple objects is typically marked by a variety of errors. While some of these errors concern the sequential organisation of action through time, many relate to the misuse of, or failure to use, necessary or appropriate tools. In this paper we apply the computational model of Cooper and Shallice (2000) to five standard multiple object tasks used in clinical assessment and demonstrate how, when lesioned, the model can account for the error profiles of two ideational apraxic patients discussed by Rumiati et al. (2001). Appl

Predicting opioid use disorder before and after the opioid prescribing peak in the United States: A machine learning tool using electronic healthcare records.

Existing predictive models of opioid use disorder (OUD) may change as the rate of opioid prescribing decreases. Using Veterans Administration's EHR data, we developed machine-learning predictive models of new OUD diagnoses and ranked the importance of patient features based on their ability to predict a new OUD diagnosis in 2000-2012 and 2013-2021. Using patient characteristics, the three separate machine learning techniques were comparable in predicting OUD, achieving an accuracy of >80%. Using the random forest classifier, opioid prescription features such as early refills and length of pres

What neuropsychology tells us about human tool use? The four constraints theory (4CT): mechanics, space, time, and effort.

Our understanding of human tool use comes mainly from neuropsychology, particularly from patients with apraxia or action disorganization syndrome. However, there is no integrative, theoretical framework explaining what these neuropsychological syndromes tell us about the cognitive/neural bases of human tool use. The goal of the present article is to fill this gap, by providing a theoretical framework for the study of human tool use: The Four Constraints Theory (4CT). This theory rests on two basic assumptions. First, everyday tool use activities can be formalized as multiple problem situations

Cortical representation of novel tool use: Understanding the neural basis of mechanical problem solving.

INTRODUCTION: Using tools effectively is a fundamental human ability. Besides the proper recall of semantic knowledge, the application of mechanical problem solving strategies allows one to execute tool-related tasks properly. Past fMRI studies have shown a mainly left-lateralized network, including ventral, ventro-dorsal, and dorso-dorsal streams while using familiar tools with access to semantic information. However, to what degree the network is recruited when applying mechanical problem solving strategies to handle novel tools remains unclear. METHODS: An event-related fMRI study includin

Tool use kinematics across different modes of execution. Implications for action representation and apraxia.

Studying the characteristics of movements performed under different action conditions may foster the understanding of disturbed tool use in apraxia and may enhance the knowledge about the links between states of action representations. We registered hand and arm movements during a hammering action executed under three task conditions: pantomime, demonstration with the hammer only, and actual execution with hammer and nail. Various movement parameters were calculated to characterize the kinematic aspects of the hammering movements. An apraxia score that reflects conceptual errors was derived fr

Ontogeny of manipulative behavior and nut-cracking in young tufted capuchin monkeys (Cebus apella): a perception-action perspective.

How do capuchin monkeys learn to use stones to crack open nuts? Perception-action theory posits that individuals explore producing varying spatial and force relations among objects and surfaces, thereby learning about affordances of such relations and how to produce them. Such learning supports the discovery of tool use. We present longitudinal developmental data from semifree-ranging tufted capuchin monkeys (Cebus apella) to evaluate predictions arising from Perception-action theory linking manipulative development and the onset of tool-using. Percussive actions bringing an object into contac

Dissociation between physical reasoning and tool use in individuals with left hemisphere brain damage.

Tool use and physical reasoning are often assumed to rely on shared cognitive and neural mechanisms. At some level, this correspondence is expected: using an object typically requires understanding its physical properties. However, both capacities are complex and multicomponential, so the relationship between them may vary across levels of representation and task demands. Here, we asked whether third-person physical reasoning about object dynamics can dissociate from tool use (i.e., performing a tool's typical action) in individuals with left-hemisphere stroke. We tested 11 patients, five of w

Heritability of gray matter volume and asymmetry in chimpanzees (Pan troglodytes) and their association to cognitive abilities and tool use.

Genetic studies have increasingly identified key mechanisms that underlie individual and phylogenetic variation in behavioral and brain phenotypes. Here, we used quantitative genetics to estimate heritability in whole brain and region-specific variation in gray matter in a sample of captive chimpanzees. We included the contributions of sex and age to individual variation in gray matter as well as their association with cognition and motor functions and found small to moderate heritability in average gray matter volume in the majority of brain regions. By contrast, weaker estimates of heritabil
