ATM is often dubbed the master regulator of the DNA double stranded break (DSB) response. Since proper induction and repair of DNA DSBs forms the core of immunological diversity, it is surprising that patients with ataxia telangiectasia generally have a mild immunodeficiency in contrast to other DSB repair syndromes. In this review, we address this discrepancy by delving into the functions of ATM in DSB repair and cell cycle control and translate these to adaptive immunity. We conclude that ATM, despite its myriad functions, is not an absolute requirement for acquiring sufficient levels of imm

Recent insights into the neural circuits controlling energy balance and glucose homeostasis have rekindled the hope for development of novel treatments for obesity and diabetes. However, many therapies contribute relatively modest beneficial gains with accompanying side effects, and the mechanisms of action for other interventions remain undefined. This Review summarizes current knowledge linking the neural circuits regulating energy and glucose balance with current and potential pharmacotherapeutic and surgical interventions for the treatment of obesity and diabetes.

Pseudoachondroplasia (PSACH) is a skeletal dysplasia characterized by disproportionate short stature, small hands and feet, abnormal joints and early onset osteoarthritis. PSACH is caused by mutations in thrombospondin-5 (TSP-5, also known as cartilage oligomeric matrix protein or COMP), a pentameric extracellular matrix protein primarily expressed in chondrocytes and musculoskeletal tissues. The thrombospondin gene family is composed of matricellular proteins that associate with the extracellular matrix (ECM) and regulate processes in the matrix. Mutations in COMP interfere with calcium-bindi

Since hospitals are often at the center of health care services, their foundation and evolution provides valuable insights into local political, economic, and cultural contexts. Based on Western and Chinese sources, this essay employs a transnational approach to explain the establishment of the Tung Wah or Oriental Dispensary in San Francisco. This article describes the institution's aims, financing, and combined operation of Western and Chinese medical staffs in an environment of racial discrimination and political power struggles, as well as deep social and cultural divisions during the earl

There are a variety of airway diseases with different clinical settings, which may extend from a surgical approach to total organ replacement. Tissue engineering involves modifying cells or tissues in order to repair, regenerate, or replace tissue in the body and seems to be a promising approach for airway replacement. The successful implantation of stem-cell-based tissue-engineered trachea in a young woman with end-stage post-tuberculosis left main bronchus collapse serves as a prototype for the airway tissue-engineered-based approach. The trachea indeed could represent a perfect model system

Pain is a multistep process that originates in the peripheral nervous system at the site of injury, is transmitted and processed within the central nervous system, and is perceived at the level of the cerebral cortex. Each of these steps in pain transmission is subject to intervention, with the possibility of reducing or blocking the nociceptive information to result in decreased pain. Based on knowledge of pain processes, dentists can use analgesic strategies to prevent or reduce pain.

Lamotrigine (Lamictal, GlaxoSmithKline) is a recently developed anticonvulsant which has been investigated for efficacy in bipolar disorder. Increasing evidence indicates that it may have a different therapeutic profile than more established first-line mood stabilizers such as lithium and valproate. In particular, evidence from well-designed trials suggests efficacy for acute management and prophylaxis of bipolar depression but not mania. However, clinically relevant drug interactions and side effects can occur with lamotrigine, including benign to serious rash. The objectives of this drug pro

Hepatocellular carcinoma is the most common primary liver cancer and a leading cause of cancer-related mortality worldwide. Traditional systemic therapies, such as tyrosine kinase inhibitors, offer limited survival benefits, prompting the emergence of immunotherapy as a transformative approach. This review synthesizes mechanistic insights into the tumor microenvironment of hepatocellular carcinoma with clinical evidence from pivotal trials on immune checkpoint inhibitors. It summarizes outcomes from monotherapy and combination regimens incorporating antiangiogenic agents, tyrosine kinase inhib

UNLABELLED: Genomics-informed cancer care is central to precision oncology; however, integrating genomic competencies into routine practice remains challenging. Despite the development of competency frameworks, limited clarity exists regarding how these competencies are operationalised within oncology settings.
AIM: To synthesise and critically examine how genomic competencies are conceptualised across the literature, identify areas of convergence and gaps and examine implications for clinical implementation and cancer nursing workforce development.
DESIGN: Scoping review.
METHODS: The revi

Type 1 diabetes (T1D) is characterised by the loss of functional pancreatic β-cells, for which lifelong insulin therapy remains the standard of care. Given that the gut microbiome can influence host health and that shifts in gut microbial profiles have been observed in T1D, growing interest has emerged in the role of the gut microbiome in T1D, particularly for its therapeutic potential. The current review aims to provide an overview of existing knowledge on the gut microbial metabolic pathways and microbiota-derived metabolites that are dysregulated or altered in T1D. Subsequently, we address

Sepsis is defined as life-threatening organ dysfunction caused by a dysregulated host response to infection. Treatments that influence this dysregulated host response are sparse. The immunopathophysiology of sepsis entails overzealous inflammation causing acute organ dysfunction, as well as a profound and/or persistent anti-inflammatory response that increases susceptibility to secondary infection. The immune response in sepsis is under the influence of various endogenous and exogenous factors, including genetic makeup, age, sex, comorbidities, metabolism, prior microbial exposure and medicati

Crohn's disease (CD) is a chronic inflammatory bowel disease (IBD) characterised by endoscopic inflammation, progressive bowel damage and gastrointestinal lesions. Although treatment strategies for CD have traditionally focused on a stepwise pharmacological approach to achieve clinical remission or symptom resolution, these treatment goals correlate poorly with disease activity. Thus, achieving full clinical remission and full endoscopic healing alone may be insufficient, as patients may remain at risk of inflammatory complications. Individualised 'treat-to-target' (T2T) pharmacological and tr

While current analytical methodologies can readily identify cannabis use, definitively establishing recent use within the impairment window has proven to be far more complex, requiring a new approach. Recent studies have shown no direct relationship between impairment and Δ9-tetra-hydrocannabinol (Δ9-THC) concentrations in blood or saliva, making legal "per se" Δ9-THC limits scientifically unjustified. Current methods that focus on Δ9-THC and/or metabolite concentrations in blood, saliva, urine, or exhaled breath can lead to false-positive results for recent use due to the persistence of Δ9-TH

Systemic lupus erythematosus (SLE) is a complex multi-organ autoimmune disease marked by the production of autoantibodies against nuclear structures, formation of immune complexes, and chronic inflammation triggered by their tissular deposition. SLE is characterized by alternating periods of relapse and remission and each flare has the potential to cause new organ damage related to either the disease process or the medication toxicity. Despite remarkable progress across its multiple domains, SLE is still an area with many unmet needs, calling for innovative and practical solutions. The efforts

Although interest in within-person variability in grandiose narcissism is growing, measurement tools are lacking that allow studying fluctuations in this personality characteristic in a differentiated manner (i.e., distinguishing narcissistic admiration and rivalry). This study explores whether a measurement approach using the six-item version of the Narcissistic Grandiosity Scale (NGS Rosenthal et al. (2007)) and six additional newly formulated adjectives allows assessing state admiration and rivalry. Structural characteristics and convergent validity of this approach were examined in an expe

The concept of health equity-the attainment of the highest possible level of health for all members of society-requires equitable access to all aspects of healthcare, including pediatric drug development. However, many communities are under-represented in pediatric drug development programs. Barriers to participation include geographic, economic, racial/ethnic bias, legal, cultural, linguistic, and other factors. While there is no "one size fits all" approach to addressing these barriers, community engagement and collaboration is recognized by the Centers for Disease Control, the World Health

A Lactobacillus-dominated vaginal microbiota (VMB) has been associated with health and considered an important host defense mechanism against urogenital infections. Conversely, depletion of lactobacilli and increased microbial diversity, amplifies the risk of adverse gynecologic and obstetric outcomes. A common clinical condition that exemplifies dysbiosis is bacterial vaginosis (BV). BV is currently treated with antibiotics, but frequently recurs, due in part to persistent dysbiosis and failure of lactobacilli to repopulate the vagina. New treatment options are needed to address BV. The VMB i
