Myeloablative Haploidentical Transplant as an Alternative to Matched Sibling Transplant for Peripheral T-Cell Lymphomas.

The number of HLA-haploidentical allogeneic hematopoietic stem-cell transplantation (Haplo-HSCT) is increasing. Comparative studies about Haplo-HSCT versus allo-HSCT with HLA-matched sibling donors (MSD-HSCT) have been tried in leukemias and B-cell lymphomas. Few studies were reported in Peripheral T-cell lymphomas (PTCLs). We performed a multicenter retrospective study about 52 patients with PTCLs undergoing Haplo-HSCT (n = 20) or MSD-HSCT (n = 32). All Haplo-HSCT recipients received antithymocyte globulin (ATG) based graft versus host disease (GVHD) prophylaxis. The median follow-up for all

Haploidentical stem cell transplant with post-transplantation cyclophosphamide and mini-dose methotrexate in children.

BACKGROUND: Haploidentical stem cell transplantation (haplo-SCT) is an option for patients without human leukocyte antigen-matched related or unrelated donor. Post-transplantation cyclophosphamide (PTCy) is an effective method of graft versus host disease (GVHD) prophylaxis and permits the use of T-cell replete grafts in settings were ex vivo manipulation is not feasible. METHODS: A retrospective study among patients younger than 18 years, with a history of hematologic malignancies who underwent haplo-SCT between 2012 and 2016. All patients received a preparative regimen of fludarabine, busul

Haploidentical stem cell transplantation with post-transplant cyclophosphamide for osteopetrosis and other nonmalignant diseases.

Allogeneic hematopoietic stem cell transplantation (HSCT) is curative for a variety of nonmalignant disorders including osteopetrosis, bone marrow failures, and immune deficiencies. Haploidentical HSCT is a readily available option in the absence of a matched donor, but engraftment failure and other post-transplant complications are a concern. Post-transplant cyclophosphamide (PT-Cy) regimens are gaining popularity and recent reports show promising results. We report our experience with nine pediatric patients with nonmalignant diseases who were transplanted from a haploidentical donor with PT

Low-Dose Post-Transplant Cyclophosphamide in Haploidentical Stem Cell Transplantation: A Systematic Review.

Haploidentical allogeneic stem cell transplants (ASCT) has transformed the treatment landscape for patients with life-threating hematologic disorders who lack matched donors. Post-transplant cyclophosphamide (PTCy) at 50 mg/kg at days +3 and +4 is established as one of the standard regimens for graft-versus-host disease (GvHD) prophylaxis. Studies on whether reducing the dose of PTCy can reduce toxicity while maintaining efficacy suffer from significant heterogeneity and lack a robust synthesis of available data. We conducted a systematic review to investigate the hypothesis that low-dose PTCy

EBV-PTLD in a patient after haploidentical stem-cell transplantation with post-transplant cyclophosphamide.

Here, we describe the case of a male patient with Epstein-Barr virus post-transplantation lymphoproliferative disorder (EBV-PTLD), which developed 18 months after a haploidentical hematopoietic stem cell transplantation (haplo-HSCT) and the administration of post-transplant cyclophosphamide (PTCy). Of note, no anti-thymoglobulin was used in the entire clinical course. Prior to the onset of EBV-PTLD, the patient had pulmonary chronic graft-versus-host disease and was treated with prednisolone and tacrolimus. After stopping immunosuppressive therapy, he was diagnosed with EBV-positive infectious

Haploidentical Bone Marrow Transplantation with Post-Transplant Cyclophosphamide for Children and Adolescents with Fanconi Anemia.

We describe haploidentical bone marrow transplantation with post-transplant cyclophosphamide (PT-CY) for 30 patients with Fanconi anemia (FA). Twenty-six patients were transplanted upfront, and the preparatory regimens included fludarabine 150 mg/m2 + total body irradiation 200 to 300 cGy ± CY 10 mg/kg without (n = 12) or with rabbit antithymocyte globulin (r-ATG) 4 to 5 mg/kg (n = 14). Four patients were rescued after primary or secondary graft failure after related or unrelated donor transplantation with the above regimen with (n = 2) or without r-ATG (n = 2). Comparison of upfront haploidentical hematopoietic stem cell transplantation and salvage haploidentical hematopoietic stem cell transplantation after immunosuppressive therapy in children with acquired severe aplastic anemia - a multicenter study.

BACKGROUND: For children with severe aplastic anemia, if the first immunosuppressive therapy (IST) fails, it is not recommended to choose a second IST. Therefore, for patients without matched sibling donor (MSD) and matched unrelated donor (MUD), haploidentical hematopoietic stem cell transplantation (Haplo-HSCT) can be chosen as a salvage treatment. This article aims to explore the comparison between upfront Haplo-HSCT and salvage Haplo-HSCT after IST. METHODS: 29 patients received salvage Haplo-HSCT, and 50 patients received upfront Haplo-HSCT. The two groups received Bu (Busulfan, 3.2mg/kg

Lymphocyte subset reconstitution and clinical outcomes following haploidentical hematopoietic stem cell transplantation.

BACKGROUND: Immune reconstitution is critical for outcomes after allogeneic hematopoietic stem cell transplantation (allo-HSCT), but its prognostic role in haploidentical peripheral blood stem cell transplantation (haplo-PBSCT) remains unclear. METHODS: We retrospectively analysed 577 patients who underwent T-cell replete haplo-PBSCT between 2016 and 2024. Longitudinal reconstitution of CD4⁺, CD8⁺ T cells and their subsets, CD3⁺CD69⁺, CD3⁺HLA-DR⁺ T cells, NK cells and B cells was assessed from 1 to 12 months post-transplant. Cox regression and causal mediation analyses were performed to ident

Haploidentical Hematopoietic Stem Cell Transplantation for AML Patients with Persistent Molecular MRD.

OBJECTIVE: The combined use of quantitative real-time polymerase chain reaction (qPCR) and next-generation sequencing (NGS) to detect molecular measurable residual disease (mMRD) has been shown to have prognostic value for patients undergoing matched-hematopoietic stem cell transplantation (HSCT). However, there have been no related studies in the context of haploidentical HSCT (haplo-HSCT). METHODS: We included 148 acute myeloid leukemia (AML) patients who were in first complete remission (CR1) and underwent HSCT at Union Hospital (Wuhan, China) between 2019 and 2023. Haploidentical Stem Cell Transplantation for Hematological Disorders: Real-World Experience from India.

Haploidentical transplant (haploSCT) has its own unique complications; hence, we studied the outcome of haploSCT from a cancer hospital in India. We retrospectively analyzed the haploSCTs performed at our center between March 2015 and mid-August 2022 using posttransplant cyclophosphamide (PTCy). Ninety-nine patients (95 malignant and 4 nonmalignant) underwent 101 haploSCTs. Myeloablative (MA), nonmyeloablative (NMA), and reduced intensity conditioning (RIC) were used in 35 (34.6%), 43 (42.5%), and 23 (22.7%) transplants, respectively. The median CD34 +  was 5.9 (1.8-10) ×10 6 /kg. The median t

Immunosuppressive therapy versus haploidentical transplantation in adults with acquired severe aplastic anemia.

Our study aimed to compare treatment outcomes between hematopoietic stem cell transplantation (HSCT) from haploidentical donors (HID) and immunosuppressive therapy (IST) in adults with acquired severe aplastic anemia (SAA). The medical records of 113 SAA adults who received IST, including rabbit ATG and cyclosporin (N = 37), or HID HSCT (N = 76) within 6 months of diagnosis at two institutions were retrospectively reviewed. Estimated 8-year overall survival (OS) was comparable between the IST and HID HSCT groups (75.6 vs. 83.7%, respectively, P = 0.328), but failure free survival (FFS) was sig

Clinical applications of haploidentical hematopoietic stem cell transplantation in severe aplastic anemia.

BACKGROUND: A zoonotic infection caused by Brucella spp., brucellosis, is endemic in some areas of the world, like in our country. One of the most devastating conditions related to this infection is endocarditis, although it is rare. Unfortunately, adequate studies on the characteristics of Brucella endocarditis have not been performed. In addition, there was no consensus on optimal type and duration of medical and interventional therapies. AIM: To answer the following questions: what are the clinical characteristics of Brucella endocarditis, which type of therapy should be performed, and can

Kidney Transplantation With Corticosteroids Alone After Haploidentical HSCT From The Same Donor.

BACKGROUND: Allogeneic hematopoietic stem cell transplantation (HSCT) can lead to donor-specific tolerance. Patients reported in the literature that underwent kidney transplantation (KT) after a previous HSCT from the same haploidentical donor typically received short-term immunosuppression, mainly for safety reasons and concerns of triggering graft-versus-host disease. METHODS: We describe the case of a 22-year-old patient who developed chronic kidney failure after receiving haploidentical HSCT from his father for the treatment of metastatic rhabdomyosarcoma. Five years after HSCT, he receiv

[Alemtuzumab-based HLA-haploidentical transplantation for relapsed/refractory aggressive NK-cell leukemia].

A 30-year-old man presented with fever of unknown origin, pancytopenia, and hepatosplenomegaly. Peripheral blood analysis revealed elevated Epstein-Barr virus (EBV) DNA levels, with EBV detected in the NK cell fraction, leading to a diagnosis of chronic active EBV disease (CAEBV). Despite immunosuppressive and etoposide therapy, fever and organ dysfunction worsened. He subsequently underwent peripheral blood stem cell transplantation (PBSCT) from an HLA-haploidentical donor with post-transplant cyclophosphamide. On day86 post-transplant, the patient developed fever, recurrent pancytopenia, ele

Transplantation after CD45-ADC corrects Rag1 immunodeficiency in congenic and haploidentical settings.

BACKGROUND: Mutations in the recombinase-activating genes 1 and 2 (RAG1, RAG2) cause a spectrum of phenotypes, ranging from severe combined immune deficiency to combined immune deficiency with immune dysregulation (CID-ID). Hematopoietic cell transplantation is a curative option. Use of conditioning facilitates robust and durable stem cell engraftment and immune reconstitution but may cause toxicity. Transplantation from haploidentical donors is associated with poor outcome in patients with CID-ID. OBJECTIVES: We sought to evaluate multilineage engraftment and immune reconstitution after cond

Successfully treated acute adult T-cell leukemia with haploidentical stem cell transplantation.

Adult T-cell leukemia-lymphoma caused by human T-lymphotropic virus type 1 is a rare and aggressive tumor with high morbidity and mortality. Historically it has been treated with chemotherapy, but more recent studies suggest improvement in survival with the combination of chemotherapy and stem cell transplantation. Our case study reports a middle-aged African American woman with no other risk factors diagnosed with the acute form of adult T-cell leukemia-lymphoma and in remission after completing chemotherapy and a haploidentical stem cell transplantation.

Haploidentical Hematopoietic Stem Cell Transplantation for XIAP Deficiency: a Single-Center Report.

PURPOSE: X-linked inhibitor of apoptosis (XIAP) deficiency caused by mutations in the XIAP/BIRC4 gene is a rare inherited primary immunodeficiency also known as X-linked lymphoproliferative syndrome type 2 (XLP2). Hematopoietic stem cell transplantation (HSCT) is currently the only curative strategy available. However, few studies of haploidentical HSCT have been published regarding the outcomes in patients with this syndrome. METHODS: We evaluated the XIAP gene analysis and clinical characteristics of four Chinese patients with XIAP who underwent haploidentical HSCT. RESULTS: The mutations
