Reconstruction of complex hand defects using trapezoidal osteocutaneous free fibular flap.

BACKGROUND: Traumatic defects of multiple metacarpal bones can be addressed using conventional or vascularized bone grafts. When associated with extensive skin and tendon loss, the treatment becomes more challenging. The aim of the study was to describe the results of using free osteocutaneous fibular flap placed in a new fashion for the reconstruction of complex hand defects. PATIENTS AND METHODS: Six patients with complex hand defects underwent reconstruction using free osteocutaneous fibular flap placed in trapezoidal fashion with two-stage tendon reconstruction using fascia lata graft. Isolated Capitulum-Trapezoid Coalition: An Unusual Cause of Wrist Pain.

Chronic wrist pain presents a diagnostic challenge, demanding a comprehensive understanding of its multifactorial etiology. This case report focuses on wrist coalition, an infrequent orthopedic condition characterized by abnormal articulations between carpal bones, often associated with prolonged wrist discomfort. A 45-year-old man with a two-year history of persistent left wrist pain, with no previous history of trauma or systemic illness, had a localized tenderness in the left carpal region with a restricted range of motion, and diminished grip strength on physical examination. Initial inves

Pre-axial mirror polydactyly associated with tibial deficiency: a study of the patterns of skeletal anomalies of the foot and leg.

PURPOSE: The study was undertaken to identify the patterns and spectrum of aberrant development of the skeletal elements of the leg and foot in children with pre-axial mirror polydactyly of the foot. METHODS: Case records and radiographs of eight children (ten feet) with pre-axial mirror polydactyly were studied. Four feet were seen in association with fibular dimelia, three feet with dysplastic trapezoid shaped tibia, two with tibial aplasia and one with tibial hypoplasia. RESULTS: Distinct patterns of anomalies of the hindfoot, midfoot and forefoot were noted, with different patterns of le

Anatomical ligament reconstruction for trapeziometacarpal osteroarthritis.

Ostoearthritis of the trapeziometacarpal (TMC) joint, the key joint in thumb opposition, is one of the most common diseases involving the hand, especially among middle-aged and elderly women, and can seriously impair overall hand function. Previous studies have indicated that joint instability from ligament insufficiency is the pathological mechanism underlying the disease. Therefore, various ligament reconstruction techniques have been developed to improve both the stability and kinematics of the joint. However, none of them involve anatomical ligament reconstruction and are almost always use

Recurrent posttraumatic trapeziometacarpal joint dislocation in a child: A case report.

Traumatic trapeziometacarpal joint dislocation of the thumb accounts for less than 1% of all hand injuries. This injury is even less common in children. Optimal treatment strategies for this injury are still the subject of debate for both children and adults. We report a case of recurrent posttraumatic trapeziometacarpal joint dislocation in an eight-year-old girl. We believe our case is the first report of recurrent acute dislocation leading to chronic dislocation in the English medical literature. Restoring the anatomy and biomechanics of the trapeziometacarpal joint is essential when treati

Bone-ligament-bone Cuenod grafting technique modified by Saffar-Romano for the treatment of chronic scapho-lunate dissociation.

BACKGROUND AND AIM OF THE WORK: The authors show their experience about six patients suffering of chronic scapho-lunate (S-L) dissociation treated with the Cuenod method modified by Saffar-Romano. METHODS: Clinical assessment was performed in all patients and compared before and after surgery at follow-up measuring pain value through the VAS, ROM of the wrist with a goniometer, grip strength by Jamar test, functional outcome with the Italian version of the Disability of Arm, Shoulder and Hand (DASH) score. Radiological assessment consisted in measuring S-L angle and classifying radio-carpic a

Trapezium fracture associated with occult fracture of the proximal pole of the scaphoid.

There has been no case of simultaneous fracture of the trapezium and the proximal pole of the scaphoid. We have treated a case with this combination. The fracture of the proximal pole of the scaphoid was only found on computed tomography imaging. We performed accurate reduction and fixation of both fractures with cannulated headless compression screws in a minimally invasive technique. We obtained rigid initial fixation and good clinical and radiographical results.

Anatomical ligament reconstruction for Neer type IIB distal clavicular fractures using suture buttons without acromioclavicular joint fixation.

BACKGROUND: Coracoclavicular (CC) ligament reconstruction has been reported as an option for distal clavicle fractures with ligament injuries. However, most of the reported techniques use single-bundle reconstruction which is not fully anatomical and sometimes associated with complications such as nonunion, coracoid fractures, or discomfort on the clavicle due to large-diameter drill holes and bulky implants. We therefore developed an anatomical double-bundle reconstruction of the CC ligaments for distal clavicle fractures. The conoid ligament is reconstructed using a suture button, while the

Excision of the Distal Pole of the Scaphoid and the Midcarpal Joint.

Background  Excision of the distal pole of the scaphoid is used to treat arthritis of the scaphotrapezial trapezoid (STT), radioscaphoid joint, and arthritis following scaphoid nonunion. Some patients develop midcarpal instability limiting utilization of this technique. Why some wrists develop postoperative instability while others do not, remains unclear. Purpose  To identify the wrists prone to developing midcarpal joint instability we evaluated the effect of midcarpal joint structure on force transfer through the wrist, we hypothesized that the force transfer will be further altered when a

[Chondrodysplasia with macrocranium and mental retardation in 2 brothers].

Two cases of a very unusual bone dysplasia in two brothers are reported. Features included very severe dwarfism, with macrocrania and mental deficiency. Roentgenograms taken in early infancy were reminiscent of spondyloepiphyseal dysplasia congenital syndrome but the course was not suggestive of this diagnosis. The shafts of the long bones became increasingly narrow, whereas the metaphyses broadened and the epiphyses at the knee became extremely wide. Vertebral bodies were ovoid or trapezoidal. The skull was extremely large. Since two offspring of consanguineous parents were involved, this dis

Biomechanical comparison of a new stand-alone anterior lumbar interbody fusion cage with established fixation techniques - a three-dimensional finite element analysis.

BACKGROUND: Initial promise of a stand-alone interbody fusion cage to treat chronic back pain and restore disc height has not been realized. In some instances, a posterior spinal fixation has been used to enhance stability and increase fusion rate. In this manuscript, a new stand-alone cage is compared with conventional fixation methods based on the finite element analysis, with a focus on investigating cage-bone interface mechanics and stress distribution on the adjacent tissues. METHODS: Three trapezoid 8 degrees interbody fusion cage models (dual paralleled cages, a single large cage, or a

Osteoarticular distal clavicle autograft for the management of instability-related glenoid bone loss: an anatomic and cadaveric study.

BACKGROUND: The reconstructive options for instability-related anterior glenoid bone loss include iliac crest autograft, allograft, or coracoid transfer. The use of distal clavicle autograft (DCG) has also been described. The purpose of this imaging and cadaveric study was to examine the dimensions, morphology, and bone density of the DCG and compare it with the Latarjet procedure. METHODS: We used 49 computed tomography scans from patients with anterior glenoid bone loss to measure the distal clavicle dimensions and bone density. Four glenoid reconstructions were simulated to compare techniq

Carpal coalition with radioscaphoid synostosis and hypoplastic thumb.

Carpal coalition is an anomaly that is usually discovered as an incidental finding on roentgenograms. The most common site is between the lunate and the triquetrum, though fusion of almost every combination of carpal bones has been reported. Carpal coalition can be isolated but has also been associated with numerous congenital malformation syndromes. In this article, we report the case of a 12-year-old boy with left-sided asymptomatic fusion of the trapezoid and trapezium, fusion of the radius and scaphoid, and hypoplasia of the thumb.

What is the role of ultrasonography in the early diagnosis of scaphoid fractures?

Scaphoid fractures are the most prevalent type of carpal bone fractures. High-spatial-resolution sonography detects direct signs of scaphoid fractures such as scaphoid cortical disruption; nevertheless, indirect signs such as radiocarpal effusion and scapho-trapezium-trapezoid effusion can also be visible. The diagnosis is performed when both direct and indirect signs of scaphoid fracture are presented. The presence of indirect signs alone is not enough to complete the diagnosis, for which more advanced imaging modalities are usually required. Here, we review the anatomy of the scaphoid, the c

Isolated dislocation of the trapezoid.

Here we present a rare case of an isolated trapezoid dislocation secondary to a high-speed motor vehicle injury. In this case we highlight that the fact that these injuries are commonly missed on standard radiographs and management invariably requires open reduction and internal fixation.

Outcomes of Surgical Management of Intraosseous Ganglia of the Carpal Bones: A Case Series.

BACKGROUND: Intraosseous ganglia of the carpal bones are uncommon with sparse publications to guide treatment. The purpose of this study was to review a single-institution experience to determine the outcomes of patients with surgically treated intraosseous carpal ganglia. METHODS: Skeletally mature patients with intraosseous carpal ganglia between 1995 and 2016 treated operatively were identified. Demographic information, clinical data, and radiographic studies were evaluated. RESULTS: Thirty-three ganglia in 31 patients were identified. Intraosseous ganglia were located in the lunate (23),

The third metacarpal boss.

A swelling and ache on the dorsum of the wrist in relation to the base of the 3rd metacarpal bone may indicate a bony lesion or an enlarged styloid process. It may also be due to an old fracture of the process or an accessory ossicle (os styloideum). Approximately half the 14 patients (15 wrists) had a history of an injury, usually a hit with the fist. Recurrent occupational strain may also produce symptoms at this site. Excision of the metacarpal base with the adjacent capitate or trapezoid, and repair of the periosteum and extensor tendon, will remove the lump and usually relieve symptoms.
