BACKGROUND: Liddle's syndrome (or pseudoprimary aldosteronism) is a rare hereditary disease; only 18 cases have been reported since 1963. Its cause remains unclear, but one of its features is increased cell membrane permeability to ions.
PATIENTS AND METHODS: A diagnosis of Liddle's syndrome was made in 4 new cases, all female, two of them sisters (cases n0 3 and 4), at the ages of 2, 12, 5 and 4 years. Potassium sparing diuretics in combination with thiazides represent a therapeutical enrichment if applied correctly that is with respect to contraindications, interactions (especially with ACE-inhibitors and AT1-antagonists) and to pharmacokinetic properties. Triamterene has to be administered twice a day while amiloride and spironolactone are effective for 24 hours. Of course, pharmacokinetic parameters of potassium sparing diuretics have to be considered also in fixed combinations.

A 22-year-old man was hospitalized because of hypertension, hypokalemic alkalosis and suppressed plasma renin activity. Although these findings were similar to hyperaldosteronism, plasma aldosterone concentration and urinary aldosterone excretion were lower than the normal range. Adrenocortical function also was normal except for aldosterone. Treatment with spironolactone, salt restriction and potassium supplementation improved the hypokalemia but not the hypertension. Blood pressure decreased markedly after administration of triamterene.

A 35-year-old man with congenital nephrogenic diabetes insipidus (NDI) is reported. Renal biopsy revealed miniaturized and rounded mitochondria of the proximal tubular cells and decreased brush-borders. Trichlormethiazide combined with triamterene resulted in an apparent reduction of daily urine volume and concomitant increment in urine osmolarity. The present case seems rare in that some morphological changes in the renal tissues could be demonstrated in an adult case with congenital NDI.

The cases of a 14-year-old farm labourer is presented as a further example of nonacetazolamide responsive hypokalaemic periodic paralysis. The family history was negative, the clinical picture classical, with prompt reversal of symptoms after the use of potassium salts. However, the administration of acetazolamide led to precipitation of an attack within a few days. The use of triamterene 100mg daily resulted in complete cessation of the attacks for the last 12 months with the maintenance of normal serum potassium levels without the use of supplementary potassium.

A. amoreuxi venom caused a high foetal resorption rate in rats, particularly when injected on days 9-11 of gestation. Vertebral and ossification defects and foetal weight loss were observed in many of the viable foetuses obtained from mothers treated with scorpion venom. Agranulocytosis associated with spironolactone administration is described in a 57-year-old man. Four days after initiation of spironolactone, leukocyte counts decreased from 8.2 to 2.3 X 10(9)/L with 6% neutrophils. Spironolactone, domperidone, and prochlorperazine were discontinued. A new method for the determination of quinidine in human serum by fluorometry is described. The interference of triamterene encountered with other methods is greatly reduced, whereas a number of other diuretics do not interfere at all.
