Healthcare practitioners' experiences of breaking bad news: A critical interpretative meta synthesis.

OBJECTIVE: To provide an overview of healthcare practitioners' experiences of breaking bad news. METHOD: Interpretative meta synthesis of 14 qualitative papers identified through a systematic search. Data were extracted and constructs elicited via reciprocal translational analysis. RESULTS: Findings comprised four inter-connected themes: (1) Discomfort, particularly difficult emotional and physical responses to breaking bad news; (2) Relational distress, connected to attachment and identification with patients; (3) Inadequate clinician, indicating the fear and rumination about the possibilit

Teaching perspectives on the communication of difficult news of genetic conditions to medical students.

Informing parents that their child has a diagnosis of Down syndrome (DS) is a common example of the delivery of unexpected or difficult news. Expectations and life planning will change, and if detected prenatally, discussions might include the option of pregnancy termination. Medical school curricula currently include training in breaking unexpected news; however, it is difficult to teach and assess. We use the perspectives of clinicians, educators, and a medical student who is the parent of a child with DS to frame a discussion on teaching, practicing, and assessing communication of difficult

Genetic testing and insurance in Australia.

BACKGROUND: Genetic testing offers great benefit for the diagnosis of genetic conditions and to identify and manage risk for conditions such as familial breast cancer. However, potential personal insurance implications exist for some patients who undergo genetic testing in Australia. Currently, insurance companies offering risk-rated products such as life insurance can use genetic test results to discriminate, which may adversely affect applicants' ability to secure a policy. Many comparable countries have banned or restricted life insurers' use of genetic results, while Australia still permit

Searching for Truth: The Forensic Interviewer's Use of an Assessment Approach While Conducting Child Sexual Abuse Interviews.

This study examined the assessment approach interviewers use while conducting interviews to assess truth as narratives are gathered in children's disclosure statements by examining 100 forensic interviews completed at a Children's Advocacy Center. A descriptive review was used to examine the steps engaged by interviewers as they followed a protocol and content analysis was used to identify interviewers' questioning strategies as they assessed children's disclosure narratives during interviews. Findings indicate that interviewers apply a protocol in order to support advancing to a phase of elic

Informing cancer patient based on his type of personality: the suspicious (paranoid) patient.

Imparting bad news had always been an unpleasant task for the physician, as shown from ancient years to our days. In the healthcare sector and as far as the cancer patient is concerned, the imparting of bad news is performed by the patient's doctor within a therapeutic relationship of course. The fundamental question is how a therapist could tailor the information to any patient and if "Is it possible to determine who should be told what, when and how ?". The aim of this paper was to describe the suspicious character or type of personality thoroughly so that any physician can make a diagnosis

[A manic episode as a result of information about breast cancer diagnosis in a patient suffering from bipolar affective disorder].

The paper describes a patient who suffered from bipolar affective disorder for 22 years and the following manic episode appeared after getting information about a diagnosis of breast cancer. During mania the patient presented delusions of being healthy which caused a marked delay in introducing the necessary treatment of the tumour. Besides, we discuss different authors' views about the possibility of evoking manic episodes by psychological factors. Same authors agree, that such an event may happen but others do not. This matter requires further studies, but any doctor who takes care of a pati

Mutilation, deception, and sex changes.

The paper considers and rejects two arguments against the performance of sexual reassignment surgery. First, it is argued that the operation is not mutilating, but functionally enabling. Second, it is argued that the operation is not objectionably deceptive, since, if there is such a thing as our 'real sex', we do not know (ordinarily) what it is. The paper is also intended to shed light on what our sexual identity is and on what matters in sexual relations.

Anonymous reports of child physical abuse: are they as serious as reports from other sources?

Anonymous reports of child physical abuse were compared to reports made by professionals and by nonprofessionals on three characteristics: substantiation rate, seriousness of substantiated incidents, and severity of allegations. Data pertinent to the characteristics were abstracted from 1,207 reports made to the Baltimore City Department of Social Services during 1983. Results suggest that reports made by anonymous sources are more likely to be unfounded than reports made by the other two sources. However, despite the lower substantiation rate, those few anonymous reports (15.5%) that do get s

Answering the relatives of lung cancer patients.

Of 230 bronchial carcinoma patients, 30 underwent resection and 13 (5.6%) survived 5 years. Some of the domestic implications of the diagnosis were sought through study of the 200 non-surgically treated patients and by interviews with 191 of their relatives. Seventy-eight per cent of the relatives said that the illness had not been as bad as they had anticipated. Fifty-five per cent of patients died within 4 months of their first examination and 28.5% of relatives were unprepared for the suddenness of the death. Five per cent of patients died of a massive haemoptysis. For two-thirds of a subgr

Healthcare professionals' involvement in breaking bad news to newly diagnosed patients with motor neurodegenerative conditions: a qualitative study.

PURPOSE: Research on breaking bad news (BBN) in healthcare has mostly focused on the doctor-patient interaction during a single consultation. However, it has been increasingly recognised that BBN is a wider process that also involves other healthcare professionals. This qualitative study explored non-medical1 healthcare professionals' involvement in BBN to newly diagnosed patients with motor neurodegenerative conditions in the UK. MATERIALS AND METHODS: 19 healthcare professionals working with people with motor neurone disease, multiple sclerosis, Parkinson's disease or Huntington's disease t

The Drive For Self-Assertion And The Reality Principle In A Patient With Malignant Pleural Mesothelioma: The History of Giulia.

Life in a contaminated environment is often marked by a cumulative psychological trauma that exhibits a variety of social-environmental aspects. This is why I suggested a psychotherapeutic group intervention for the population of Casale Monferrato, a municipality in Northern Italy that is sadly renowned for asbestos-related events and the high mortality rate of its inhabitants. Groupality appears to show the point of contact between psyche and soma, while also promoting the birth of a more realistic approach to the various levels of suffering and their configuration. The multifamily approach s

Truth Be Told: Evaluation of a Narrative and Skills Intervention in Two Women's Prisons.

This article describes the evaluation of a narrative and skills intervention to enable women to come to terms with their offenses, tell the truth, and then work to deal with the emotions and issues surrounding their offenses, in two southern U.S. women's prisons. The intervention involved skills building in communication, community building, creativity, and caring for the self, using narrative writing, speaking, and movement. There was broad agreement that the intervention had multiple positive impacts, including insight into factors that lead to incarceration, comfort with selves and better a

Patient responses to genetic information: studies of patients with hereditary cancer syndromes identify issues for use of genetic testing in nephrology practice.

Advances in the genetic basis of kidney disease may mean that genetic testing is increasingly important in reducing disease morbidity and mortality among patients. However, there is little research examining patient responses to genetic information for Mendelian and common kidney diseases. Existing research on kidney and other hereditary cancer syndromes can inform three major issues relevant to the nephrology context as follows: (1) how patients understand their risk of disease after genetic counseling and testing, (2) their emotional responses to the information, and (3) their uptake of reco
