Primary Oral Tuberculosis of Tongue, Tonsil, and Labial Mucosa: A Rare Case Report.

INTRODUCTION: Tuberculosis (TB) is worldwide a leading cause of morbidity, mortality, and a major public health problem particularly in resource constrained countries. Extrapulmonary tuberculosis (EPTB) accounts for about 20% of all TB cases. Primary oral cavity TB is not only rare form of EPTB but also exists with its diagnostic challenges. To our knowledge, a small number of primary oral TB cases have been reported in literature, which is likely due to the limited diagnostic modalities and its rarity. CASE PRESENTATION: We report a 40-year-old female patient presented to Dekemhare hospital

Bait uptake by wild badgers and its implications for oral vaccination against tuberculosis.

The deployment of baits containing vaccines or toxins has been used successfully in the management of wildlife populations, including for disease control. Optimisation of deployment strategies seeks to maximise uptake by the targeted population whilst ensuring cost-effectiveness. Tuberculosis (TB) caused by infection with Mycobacterium bovis affects a broad range of mammalian hosts across the globe, including cattle, wildlife and humans. The control of TB in cattle in the UK and Republic of Ireland is hampered by persistent infection in European badgers (Meles meles). The present study aimed t

Oral vaccination with subunit vaccines protects animals against aerosol infection with Mycobacterium tuberculosis.

Immunity against Mycobacterium tuberculosis depends largely on activation of cell-mediated responses, and gamma interferon has been shown to play a crucial role in this process in both humans and animal models. Since the lung is normally the organ in which infection is initiated and is the major site of pathology, immune responses in the lung play a significant role in restricting initial infection with M. tuberculosis. The aim of the present study was to stimulate efficient immunity in the lung by targeting the gut mucosa. Detoxified monophosphoryl lipid A (MPL) has been shown to be a relativ

Oral Tolerance to Environmental Mycobacteria Interferes with Intradermal, but Not Pulmonary, Immunization against Tuberculosis.

Bacille Calmette-Guérin (BCG) is currently the only approved vaccine against tuberculosis (TB) and is administered in over 150 countries worldwide. Despite its widespread use, the vaccine has a variable protective efficacy of 0-80%, with the lowest efficacy rates in tropical regions where TB is most prevalent. This variability is partially due to ubiquitous environmental mycobacteria (EM) found in soil and water sources, with high EM prevalence coinciding with areas of poor vaccine efficacy. In an effort to elucidate the mechanisms underlying EM interference with BCG vaccine efficacy, we expos

Oral Tuberculosis: A Rare Manifestation of Disseminated Disease in a Patient with Dermatomyositis on Chronic Corticosteroids.

Tuberculosis remains one of the leading causes of death around the world despite advancements in diagnostic testing and medical therapies. It commonly affects the lungs, but isolated extra pulmonary clinical manifestations have been reported. Tuberculosis of the oral cavity is exceedingly rare. We present a case of a patient with dermatomyositis on chronic steroid therapy, who presented with tuberculosis involving the tongue as the only clinical manifestation of disseminated disease. Physicians must be aware of extra pulmonary manifestations of tuberculosis in patients at risk, in order to avo

Cohort evaluation of an all-oral shorter regimen for drug-resistant tuberculosis in Ecuador.

BACKGROUND: Tuberculosis (TB) remains the leading cause of death from infectious agents globally and disproportionately affects people living with HIV. In 2023, 10.6 million TB cases were reported worldwide, including 8,200 in Ecuador. Between 2020 and 2023, TB incidence in Ecuador increased from 31.2 to 48.1 per 100,000 population, alongside rising rifampicin- and fluoroquinolone-resistant TB cases. Treatment success declined from 76.2% in 2019 to 69.2% in 2021, highlighting the need for more effective and patient-centered treatment strategies. This study evaluated the effectiveness, safety,

Oral tuberculosis in a patient with rheumatoid arthritis after long treatment with methotrexate and adalimumab.

The treatment of rheumatoid arthritis (RA) with tumor necrosis factor α (TNF-α) inhibitors has been associated with an increased risk of tuberculosis (TB). Most patients have extrapulmonary disease. We describe a case of tonsil TB in an RA patient treated with methotrexate for 23 years and adalimumab (TNF-α inhibitor) for the last 3 years after an initial negative PPD (purified protein derivative of tuberculin) skin test. Our patient presented with a tonsil ulcer. PPD skin test was now positive; biopsy result of the lesion revealed Mycobacterium tuberculosis on culture, and a granuloma typical

[The course of nonspecific odontogenic allergy in patients with pulmonary tuberculosis in relation to oral hygiene].

The allergic status of 99 patients with different forms of pulmonary tuberculosis was studied by the intracutaneous test with allergens of Streptococcus hemolyticus and Staphylococcus aureus. It was shown that the level of bacterial sensitization in the patients to these allergens mostly depended on the presence of odontogenic foci of chronic inflammation. Combined treatment of the main process with oral cavity hygiene promoted desensitization and lowering of the host allergic predisposition evident from decreasing or elimination of positive reactions to the streptococcal-staphylococcal allerg

Combined Oral Contraceptive Drug-Drug Interaction Study With Ganfeborole, a New Anti-Tuberculosis Agent.

New drugs are urgently needed to treat drug-resistant tuberculosis in combination regimens. Ganfeborole demonstrated bactericidal activity and good tolerability in clinical trials. In preclinical studies, ganfeborole showed embryofetal developmental effects, currently mandating highly effective non-user dependent contraception in women of childbearing potential. We conducted a Phase 1, open-label, single-center, fixed sequence, 1-way drug-drug interaction (DDI) study in 20 healthy women of non-childbearing potential aged 18-65 years. The primary objective was to assess ganfeborole's effect at

Three cases of oral mucosal tuberculosis in patients on tumour-necrosis-factor-α blockers.

We present three cases of oral mucosal lesions caused by Mycobacterium tuberculosis in patients treated with anti-tumour necrosis factor-α for psoriasis or rheumatoid arthritis. Diagnosis of oral mucosal tuberculosis was not easily established in any of the cases. A comparison between these cases and other previously described forms of oral mucosal tuberculosis is presented.

Sustained protection against tuberculosis conferred to a wildlife host by single dose oral vaccination.

BACKGROUND: Vaccination of wildlife against bovine tuberculosis (TB) is being considered by several countries to reduce the transmission of Mycobacterium bovis infection to livestock. In New Zealand, where introduced brushtail possums (Trichosurus vulpecula) are the major wildlife hosts, we have previously shown that repeat applications of a lipid-encapsulated oral bacille Calmette-Guerin (BCG) vaccine reduce the incidence of naturally acquired TB in wild possums. Here we extend this conceptual demonstration to an operational level, assessing long-term protection against TB conferred to free-l

Treatment Adherence With an Oral 9-Month Regimen for Rifampicin-Resistant Tuberculosis in South Africa.

BACKGROUND: Adherence to anti-tuberculosis (TB) therapy is an important determinant of treatment outcome in rifampicin-resistant tuberculosis (RR-TB). Understanding adherence to contemporary treatment regimens in routine care is needed to support implementation in TB programs. We aimed to characterize temporal adherence patterns among people receiving oral treatment for RR-TB. METHODS: We conducted a prospective observational cohort study at a referral TB hospital in South Africa. People aged ≥15 years with pulmonary RR-TB starting an oral 9- to 12-month regimen were included. Treatment adher

Diagnostic accuracy of oral swab for detection of pulmonary tuberculosis: a systematic review and meta-analysis.

OBJECTIVES: Tuberculosis (TB) remains a significant concern in terms of public health, necessitating the timely and accurate diagnosis to impede its advancement. The utilization of oral swab analysis (OSA) presents a promising approach for diagnosing pulmonary TB by identifying Mycobacterium tuberculosis (MTB) within oral epithelial cells. Due to disparities in the diagnostic performance of OSA reported in the original studies, we conducted a meticulous meta-analysis to comprehensively assess the diagnostic efficacy of OSA in pulmonary TB. METHODS: We conducted a comprehensive investigation a

Combined inhalation and oral supplementation of Vitamin A and Vitamin D: a possible prevention and therapy for tuberculosis.

Tuberculosis is continuing as a problem of mankind. With evolution, MDR and XDR forms of tuberculosis have emerged from drug sensitive strain. MDR and XDR strains are resistant to most of the antibiotics, making the management more difficult. BCG vaccine is not providing complete protection against tuberculosis. Therefore new infections are spreading at a tremendous rate. At the present moment there is experimental evidence to believe that Vitamin A and Vitamin D has anti-mycobacterial property. It is in this context, we have hypothesized a host based approach using the above vitamins that can

Flexible and dispersible paediatric oral formulations produced via extrusion spheronisation for the treatment of tuberculosis.

Until now, there has been a lack of suitable formulations for combined tuberculosis (TB) therapy to treat paediatric populations. The objective of this research was the development and manufacture (at small scale) of an age-appropriate flexible dose combination of dispersible spherical pellets of isoniazid (INH), pyrazinamide (PZD) and rifampicin (RIF) for TB treatment. These pellets were prepared using either crospovidone (XPVP) alone or XPVP in combination with microcrystalline cellulose (MCC) as an extrusion-spheronisation (ES) aid (ES-aid). Each drug was incorporated into ES-aid separately

Virulence of Candida Isolates in Patients with Tuberculosis and Oral/Oesophageal Candidiasis: Co-Infection Evaluation.

Tuberculosis (TB) is an infection caused by Mycobacterium tuberculosis complex (MTBC), which can be exacerbated by fungal infections. This study evaluated the clinical characteristics and virulence of Candida spp. in patients with tuberculosis. Antifungal sensitivity, phospholipase and proteinase production, biofilm formation, phagocytic index, and reactive oxygen (ROS) and nitrogen (RNS) species were assessed. Candida spp. were isolated from 14 patients, 28.5% women and 71.4% men, mainly from sputum and tracheal secretions. Five (35.7%) patients were co-infected with Mycobacterium, Candida, a

The impact of an oral glucose load on IFN-γ-release in persons infected with Mycobacterium tuberculosis.

BACKGROUND AND OBJECTIVE: To diagnose tuberculosis infection (TBI), whole blood is incubated with M.tuberculosis (Mtb)-specific peptides and the release of interferon-γ (IFN-γ) is measured in IFN-γ-release assays (IGRAs). Hyperglycaemia and fluctuations in blood glucose may modulate IFN-γ-release. Here, we investigated if glucose intake affects IFN-γ-release or IGRA results in IGRAs taken during an oral glucose tolerance test (OGTT). METHODS: Persons with TB disease (TB) or TBI underwent a standard 75-g OGTT at the start and end of treatment for TB or TBI. Blood for the IGRA QuantiFERON-TB Go
