Effects of Biomechanical Testing Using a Synthetic Ligament Fabricated from Polyhydroxyalkanoate Biopolyesters for Lateral Ulnar Collateral Ligament Reconstruction in Cadaver.

An injury to the elbow's lateral ulnar collateral ligament (LUCL) is an orthopedic emergency that can impair joint stability and functional biomechanics throughout the upper extremity. The development and application of synthetic ligament substitutes, particularly short-chain-length and medium-chain-length polyhydroxyalkanoate (SCL-PHA and MCL-PHA) co-polymers, represent a promising innovation for lateral elbow stabilization. This experimental cadaveric study aimed to (1) compare biomechanical parameters of torque and angular rotation among control, damage, repair, and reconstruction groups an

Biomechanical Comparison of Ulnar Collateral Ligament Reconstruction With a Modified Docking Technique With and Without Suture Augmentation.

BACKGROUND: Throwing athletes with ulnar collateral ligament (UCL) injury and symptomatic valgus instability can expect to return to the same or higher level of play. Reconstruction with tendon graft is the dominant method of surgical treatment. Recent evidence suggests that spanning the joint with a suture anchored on both sides is biomechanically equivalent to reconstruction, with faster time to return to play. The authors developed a hybrid UCL reconstruction technique augmented with a suture brace to improve joint stability. PURPOSE/HYPOTHESIS: The purpose of this study was to biomechanic

Thumb metacarpophalangeal ulnar collateral ligament injuries: a biomechanical simulation study of four static reconstructions.

PURPOSE: To determine the optimal tunnel placement positions for tendon graft reconstruction of chronic thumb metacarpophalangeal (MCP) ulnar collateral ligament injuries that would stabilize the joint while maintaining motion. METHODS: Four commonly used tunnel placement methods were simulated on a cadaveric model using fresh-frozen thumbs and a suture/screw construct. The methods were as follows: (1) triangular configuration with apex proximal, (2) triangular configuration with apex distal, (3) cruciate configuration, and (4) parallel configuration. Stability was tested by valgus loading at

Biomechanical Stability of Lateral Ulnar Collateral Ligament Reconstruction and Repair of the Elbow: The Role of Ligament Bracing on Gap Formation and Stabilization.

BACKGROUND: Augmented (internal braced) lateral ulnar collateral ligament (LUCL) repair has been biomechanically compared with reconstruction techniques in the elbow. However, LUCL repair alone has not yet been compared with augmented repair and reconstruction techniques. HYPOTHESIS: Internal bracing of LUCL repair would improve time-zero stabilization regarding gap formation, stiffness, and residual torque as compared with repair alone and reconstruction techniques to restore native elbow stability. STUDY DESIGN: Controlled laboratory study. METHODS: Overall, 24 cadaveric elbows were used

Elbow ulnar collateral ligament reconstruction: clinical, radiographic, and ultrasound outcomes at a mean 3-year follow-up.

BACKGROUND: Ulnar collateral ligament (UCL) has the main function to be stress-resistant to elbow valgus overload. Multilayer tears require a reconstruction with a tendon graft. In this study, we report the clinical, radiographic, and ultrasound outcomes after tendon graft reconstruction for symptomatic UCL insufficiency. MATERIALS AND METHODS: Among twenty-six subjects underwent elbow UCL reconstruction from 2006 to 2012, fifteen were available to be evaluated at a mean follow-up of 36 months. Preoperative assessment included clinical examination and MRI. The outcome measures were the Mayo E

The modified docking procedure for elbow ulnar collateral ligament reconstruction: 2-year follow-up in elite throwers.

BACKGROUND: Ulnar collateral ligament injury is most common in the overhead-throwing athlete. Jobe et al published the first report of ulnar collateral ligament reconstruction in throwing athletes with a 62.5% success rate. Recently, Altchek developed a new docking technique for reconstruction of the ulnar collateral ligament. The authors report the first series using a further modification of the docking technique using a 4-strand palmaris longus graft for reconstruction of the ulnar collateral ligament. HYPOTHESIS: The modified docking technique yields a high rate of successful return to pr

Effect of the suture augmentation on the stability and strength of ulnar collateral ligament reconstruction: The anchor and bone tunnel methods.

BACKGROUND: The initial fixation strength of the ulnar collateral ligament (UCL) reconstruction is increased by combining a suture augmentation (SA). However, no reports have investigated the stability of UCL reconstruction with SA across multiple elbow flexion angles and the influence of SA on the stability and failure strength of the anchor and bone tunnel methods. This study aimed to compare the displacement against a valgus load at multiple elbow flexion angles of the anchor and bone tunnel methods between with and without SA, and to compare the failure strength between these methods with

Reconstruction of the Medial Ulnar Collateral Ligament of the Elbow: Biomechanical Comparison of a Novel Anatomic Technique to the Docking Technique.

BACKGROUND: In recent years, understanding of the anatomy of the ulnar collateral ligament (UCL) has evolved, demonstrating that the insertional footprint of the UCL on the ulna is more elongated and distally tapered than previously described. Current UCL reconstruction configurations do not typically re-create this native anatomy, which may represent a potential area for improvement. PURPOSE/HYPOTHESIS: The purposes of this study were (1) to describe a novel anatomic UCL reconstruction technique designed to better replicate the native UCL anatomy and (2) to biomechanically compare this with

Incidence of Elbow Medial Ulnar Collateral Ligament Repair and Reconstruction: A US Population Database Study 2016 to 2022.

BACKGROUND: Medial ulnar collateral ligament (UCL) injuries of the elbow affect overhead athletes with an increasing incidence over the past few decades. Growing evidence supports UCL repair as a treatment option in patients with low-grade UCL injuries or complete avulsion tears without chronic degenerative changes of the ligament. With this evolution in treatment options, accurate epidemiological data are needed to better understand and treat these injuries. PURPOSE: To investigate the trends of elbow UCL repair and reconstruction as it relates to the incidence of surgically managed UCL inju

Similar Biomechanics Between the Double-Cortical Button and Docking Techniques for Ulnar Collateral Ligament Reconstruction: A Cadaveric Evaluation.

BACKGROUND: The docking technique is widely used to perform ulnar collateral ligament (UCL) reconstructions because of its high failure torque and reliable clinical outcomes. A double-cortical button technique was recently described, with advantages including the ability to tension the graft at the ulnar and humeral attachments and the creation of single bone tunnels. PURPOSE/HYPOTHESIS: To compare the biomechanics between the docking and double-button UCL reconstruction techniques using cadaveric specimens. We hypothesized that there would be no difference in postoperative stiffness or maxim

Similar return to sport between double cortical button and docking techniques for ulnar collateral ligament reconstruction in baseball players.

BACKGROUND AND HYPOTHESIS: A double cortical button technique for ulnar collateral ligament reconstruction (UCLR) has advantages including significant control over graft tensioning, less concern about graft length, and minimized risk of bone tunnel fracture compared with traditional UCLR techniques. This double cortical button technique was recently found to be noninferior in mechanical performance to the traditional docking technique regarding joint strength, joint stiffness, and graft strain. However, clinical outcomes have not been compared between these UCLR techniques. Therefore, the purp

Elbow lateral ulnar collateral ligament reconstruction by transposition of the local extensor fascia septum: surgical technique and preliminary results.

The lateral ulnar collateral ligament (LUCL) is a primary lateral stabilizer of the elbow that originates from the isometric center of the capitulum and inserts into the supinator crest of the ulna. LUCL injury may be due to trauma, chronic strain, or iatrogenic lesion. In patients with symptomatic LUCL insufficiency and recurrent posterolateral rotatory instability, surgical reconstruction can restore elbow stability. In primary acute treatment, the injured LUCL is reattached to the lateral epicondyle with transosseous sutures and anchors placed at the isometric origin of the ligament. If the
