The influence of ruptured cerebral aneurysm localization on the blood flow velocity evaluated by transcranial Doppler ultrasonography.

The relationship between changes of blood flow velocities in cerebral arteries measured by transcranial Doppler ultrasonography and aneurysm localization was investigated in a group of 165 patients after aneurysmal subarachnoid hemorrhage (SAH). Mean blood flow velocities (MFV) in the middle cerebral artery (MCA) and anterior cerebral artery (ACA) were registered. In patients with aneurysm of internal carotid artery and MCA (group A) statistically significant higher values of MFV from the 1st to the 5th day and on the 12th, 13th, 14th, 15th, and 19th day after SAH were found compared to patien

Transcranial Doppler ultrasonography of the middle cerebral artery in the hemodynamic assessment of internal carotid artery stenosis.

We performed transcranial Doppler ultrasonography of the middle cerebral arteries in 43 patients who underwent cerebral angiography. Twelve patients had normal arteriograms, 18 had internal carotid artery stenosis, and 13 had complete occlusion. Parameters measured included: flow velocity (mean, maximum, and peak systolic), flow acceleration, systolic-to-diastolic ratio, and pulsatility index. Patients with 75% to 100% stenosis had lower average ipsilateral flow acceleration and mean velocity than did normal subjects. Correlation analysis revealed an inverse relationship between degree of sten

Cerebral blood flow alterations in progressive communicating hydrocephalus: transcranial Doppler ultrasonography assessment in an experimental model.

OBJECT: In many cases communicating hydrocephalus is the result of impairments in cerebrospinal fluid absorption in the arachnoid villi at the cranial convexity. Reported methods of creating experimental hydrocephalus have not sought to produce an arachnoidal adhesion in the cranial convexity. In this study the authors investigate alterations in cerebral blood flow (CBF) in experimental communicating hydrocephalus induced by the injection of kaolin into the subarachnoid space at the convexity in neonatal rats. METHODS: In neonatal rats, kaolin was injected into the subarachnoid space at the c

Importance of jugular valve incompetence in contrast transcranial Doppler ultrasonography for the diagnosis of patent foramen ovale.

Transcranial Doppler (TCD) ultrasound with the intravenous injection of agitated saline as contrast (cTCD) is an effective method for detecting right-to-left intracardiac and extracardiac shunt (RLS); however, the sensitivity of cTCD in the diagnosis of RLS remains slightly less than that of transesophageal echocardiography, even in patients with adequate transtemporal ultrasonic bone windows. The authors present a case with cTCD underestimating RLS because of jugular valve incompetence in a 42-year-old man presenting with an episode of transient aphasia. Three weeks after transcatheter closur

Early spontaneous improvement and deterioration of ischemic stroke patients. A serial study with transcranial Doppler ultrasonography.

BACKGROUND AND PURPOSE: The purpose of our study was to investigate whether emergency transcranial Doppler (TCD) findings and their modifications over the first 48 hours are related to early neurological changes in acute ischemic stroke patients. METHODS: Ninety-three patients underwent CT scan within 5 hours of a first-ever ischemic hemispheric stroke, and TCD serial examinations at 6, 24, and 48 hours after stroke onset. We classified TCD findings as follows: normal; middle cerebral artery (MCA) asymmetry (asymmetry index between affected and contralateral MCAs below -21%); and MCA no-flow

Detection of patent foramen ovale by transcranial Doppler and carotid duplex ultrasonography: a comparison with transoesophageal echocardiography.

Patients who have suffered a transient ischaemic attack or minor stroke are examined thoroughly to determine the aetiology of the cerebrovascular incident. An examination of the carotid arteries is compulsory to detect significant stenosis. In some patients, cardiogenic sources of emboli may be suspected. This study examined the possibilities of screening for patent foramen ovale (PFO) using i.v. injection of a microbubble solution with Doppler monitoring of the internal carotid artery (ICA) and the middle cerebral artery (MCA). Transoesophageal echocardiography (TEE) served as the gold standa

Assessment of a Transverse-Sigmoid Sinus Dural Arteriovenous Fistula using Transcranial Doppler Ultrasonography: A Case Report.

BACKGROUND: The most common symptoms of transverse-sigmoid sinus dural arteriovenous fistula (TS-S DAVF) are pulsatile tinnitus and headache. The clinical characteristics lack specificity and may lead to misdiagnosis or delayed diagnosis. The noninvasive and convenient nature of neurovascular ultrasound has made it an important screening tool for lesions. This paper reports the use of transcranial Doppler (TCD) ultrasonography for the preoperative screening and postoperative follow-up of a patient diagnosed with a transverse-sigmoid sinus dural arteriovenous fistula (TS-S DAVF). Use of transcranial Doppler ultrasonography and jugular oximetry to optimize hemodynamics during pediatric posterior fossa craniotomy.

We describe a 13-year-old child who developed inadvertent intraoperative cerebral oxygen desaturation attributed to empiric hyperventilation during posterior fossa craniotomy, diagnosed by jugular venous oximetry. Although restoration of normocapnia at age-appropriate mean arterial blood pressure improved jugular venous saturation (SjvO), it remained below acceptable values. Transcranial-Doppler (TCD) ultrasonography-guided stepwise increase in mean arterial pressure led to a linear increase in the mean flow velocity of the middle cerebral artery and SjvO₂ to desirable values. Intraoperative S

Mapping real-time hemodynamic changes in venous sinus stenting using intraoperative transcranial Doppler ultrasonography and quantitative angiography.

OBJECTIVE: Venopathic intracranial hypertension (VIH) is a condition of elevated intracranial pressure (ICP) associated with venous sinus stenosis. Venous sinus stenting (VSS) can reconstitute outflow, yet physiological biomarkers to aid patient selection and assess treatment response remain limited. This study assessed intraoperative transcranial Doppler ultrasonography (TCD) and quantitative angiography as tools for detecting cerebral hemodynamic changes during VSS in patients with VIH. METHODS: The authors retrospectively analyzed 11 consecutive patients with VIH who underwent VSS using in

[Cerebral vasospasm after subarachnoid hemorrhage, and inhibitory effect of nicardipine investigated by means of transcranial Doppler ultrasonography].

Cerebral vasospasm is a major complication associated with subarachnoid hemorrhage (SAH). However, real-time evaluation of vasospasm is very difficult via angiography, CT scan, and cerebral blood flow. In this study, for the evaluation of the arterial narrowing, the flow velocity in the middle cerebral artery (MCAFV) was measured with transcranial Doppler ultrasonography (TCD) in 41 patients with ruptured cerebral aneurysms which were repaired surgically within 48 hours after SAH. MCAFV was measured daily until the 25th day after onset. All patients underwent CT scan, angiography, and neurolog

[Surveillance of cerebral blood circulation changes by transcranial Doppler ultrasonography in patients with nasopharyngeal carcinoma after radiotherapy].

BACKGROUND & OBJECTIVE: Radiotherapy is the first choice for patients with nasopharyngeal carcinoma (NPC), but radiation encephalopathy affects the patients' quality of life significantly. Some research results have showed that the brain damage was associated with radiation-induced cerebral vascular injury. The study was designed to determine the cerebral blood circulation changes in NPC patients pre- and post- radiotherapy using transcranial Doppler ultrasonography (TCD), and to evaluate its value in the early diagnosis and treatment of radiation encephalopathy. METHODS: The blood flow veloc

Correlation of transcranial Doppler ultrasonography with MRI and MRA in the evaluation of sickle cell disease patients with prior stroke.

We prospectively evaluated a group of patients with sickle cell disease and a clinical history of prior stroke, comparing transcranial Doppler sonography (TCD) to both magnetic resonance imaging (MRI) and magnetic resonance angiography (MRA) to determine its efficacy for the detection of flow abnormalities associated with prior cerebral infarction. Using MRI as the standard examination, there was 94% sensitivity and 30% specificity, and using MRA as the standard examination, there was 91% sensitivity and 22% specificity. We concur with other reports that the transcranial Doppler examination is

[A study of cerebral hemodynamics using transcranial Doppler ultrasonography with special reference to clinical evaluation of pulsatility index].

Transcranial Doppler Ultrasonography (TCD) is now commonly used to evaluate the cerebral hemodynamics. However, the blood velocity measured with TCD is not always accurate, because as a rule the direction of the blood flow does not completely coincide with that of the ultrasonic beam. The error in the measurement could be as large as 15%. This could be eliminated by calculating pulsatility index (PI) from the Doppler ultrasonography. This study evaluates the reliability of the PI as an index of cerebral hemodynamics. In total, 164 measurements were made in patients with various cerebrovascular
