Risk of dementia following herpes zoster infection among patients undertreatment versus those not: A systematic review and meta-analysis.

BACKGROUND AND AIMS: According to the previous studies, herpes zoster (HZ) has been associated with cognitive function and dementia. There is a hypothesis claiming that dementia risk may be reduced by receiving the antiviral treatment for HZ. The purpose of this systematic review and meta-analysis was to shed light on the association between dementia and HZ in individuals receiving and not receiving antiviral medications. METHODS: Studies investigating the association between HZ and dementia were identified through a systematic search in PubMed/MEDLINE, Scopus, Embase, Google Scholar, and Coc

Clinical and racial predictors of adverse pathology at radical prostatectomy: implications for undertreatment among patients receiving radiation and hormonal therapy.

OBJECTIVES: Under-grading exists in up to 7% of patients undergoing radical prostatectomy (RP) for prostate cancer (PC). We assessed whether underrepresented race and ethnicity disproportionately increased the odds of adverse pathology at RP in patients with biopsy Gleason score 6 or 7 PC at high-risk for upgrading and/or upstaging at RP based on age and PC indices at presentation. METHODS: This retrospective cohort study analyzed 76,474 patients in the National Cancer Database (2015-2021) with biopsy Gleason score 6 or 7 N0M0 PC. Odds ratio (OR) at RP of adverse pathology defined as prostate

Factors associated with potential over- and undertreatment of hyperglycaemia and annual measurement of HbA1c in type 2 diabetes in norwegian general practice.

AIMS: To identify individual and general practitioner (GP) characteristics associated with potential over- and undertreatment of hyperglycaemia in type 2 diabetes and with HbA1c not being measured. METHODS: A cross-sectional study that included 10233 individuals with type 2 diabetes attending 282 GPs. Individuals with an HbA1c measurement during the last 15 months were categorized as potentially overtreated if they were prescribed a sulphonylurea and/or insulin when the HbA1c was less than 53 mmol/mol (7%) when aged over 75 years or less than 48 mmol/mol (6.5%) when aged between 65 and 75 yea

Clinical Phenotype Transition in Pemphigus May Suggest Undertreatment with Rituximab: Findings from a Retrospective Single-Centre Analysis of Relapsed Patients.

UNLABELLED: <p>Introduction: Pemphigus is an autoimmune bullous disease caused by autoantibodies against desmoglein (DSG) 1 and/or 3 and comprises two main subtypes: pemphigus vulgaris (PV) and pemphigus foliaceus (PF). PV affects the skin and/or mucosa and includes two forms: mucocutaneous PV (mcPV; anti-DSG1 and anti-DSG3) and mucosal PV (mPV; anti-DSG3). PF, characterized by anti-DSG1 autoantibodies, is limited to the skin. Pemphigus is effectively treated with rituximab, a B-cell-depleting therapy. However, about half of patients relapse, with a subset exhibiting a shift in clinical subtyp

Estimated undertreatment of carbapenem-resistant Gram-negative bacterial infections in eight low-income and middle-income countries: a modelling study.

BACKGROUND: Carbapenem-resistant Gram-negative (CRGN) bacterial infections are an urgent health threat, especially in low-income and middle-income countries (LMICs), where they are rarely detected and might not be treated appropriately given inadequate health system capacity. To understand this treatment gap, we estimated the total number of CRGN bacterial infections requiring an active agent and the number of individuals potentially initiated on appropriate treatment in eight large LMICs. METHODS: For eight selected countries (Bangladesh, Brazil, Egypt, India, Kenya, Mexico, Pakistan, and So

What did COVID-19 pandemics teach us about single-fraction radiotherapy for painful bone metastases-State of the art or undertreatment?

BACKGROUND: Choosing the optimal treatment approach for patients with painful bone metastases during the COVID-19 pandemic became challenging. A simple technique, single fraction radiotherapy was recommended for these patients usually referring to bone metastases as a single entity, although it is a very heterogeneous group of patients. AIM: This study aimed to analyze the response to palliative single fraction radiotherapy in relation to age, performance status, primary tumor, histopathology, and bone localization in the group of patients with painful bone metastases. METHODS: A clinical, p

Clinical management of chlamydia and gonorrhea infection in a county teaching emergency department--concerns in overtreatment, undertreatment, and follow-up treatment success.

To date, several studies have examined overtreatment or undertreatment of Neisseria gonorrheae, Chlamydia trachomatis, or both in women. However, no study has looked at both subpopulations together, along with eventual treatment of disease-positive patients who were not empirically treated. This study is unique, for it looks at all of these subpopulations to assess overall efficacy of management of these diseases in women. A 1-year prospective, descriptive study was performed in a teaching county hospital Emergency Department (ED). There were 1260 women receiving a pelvic examination and routi

Review of the effect of opioid-related side effects on the undertreatment of moderate to severe chronic non-cancer pain: tapentadol, a step toward a solution?

OBJECTIVE: Opioids are among the most effective and potent analgesics currently available. Their utility in the management of pain associated with cancer, acute injury, or surgery is well recognized. However, extending the application of opioids to the management of chronic non-cancer pain has met with considerable resistance. This resistance is due in part to concerns related to gastrointestinal and central nervous system-related adverse events as well as issues pertaining to regulatory affairs, the development of tolerance, incorrect drug usage, and addiction. This review focuses on the inci

Chronic kidney disease in Norwegian primary care: underdiagnosis and undertreatment - findings from the VISTAKID (Vestfold Investigation of Stage 3-5 Kidney Disease) study.

INTRODUCTION: Chronic kidney disease (CKD) is common yet frequently unrecognized in primary care. We aimed to quantify the burden of CKD stages 3–5 identified via routine general practice (GP) testing, compare characteristics of patients known vs unknown to nephrology, and estimate population prevalence to inform shared-care pathways. METHODS: We assembled a laboratory cohort of adults whose serum creatinine tests were ordered by GPs in Vestfold between April 2010 and January 2021. Individuals with at least one eGFR < 60 mL/min/1.73 m2 were identified, and CKD stages 3–5 were confirmed accordi

Primary Congenital Hypothyroidism in Children Below 3 Years Old - Etiology and Treatment With Overtreatment and Undertreatment Risks, a 5-Year Single Centre Experience.

Worldwide neonatal screening for congenital hypothyroidism (CH) is a gold standard of active surveillance in newborns. Prompt diagnosis, subsequent timely treatment implementation, and proper dosage of levothyroxine (L-T4) are crucial for normal growth and development, especially of the central nervous system. However, overtreatment may have a potential negative impact on further neurodevelopment. We retrospectively analysed data of 99 newborns with CH diagnosis, referred to the Endocrinology Outpatient Clinic of the Institute of Mother and Child in Warsaw, Poland from the CH screening program

Molecular sources of residual cardiovascular risk, clinical signals, and innovative solutions: relationship with subclinical disease, undertreatment, and poor adherence: implications of new evidence upon optimizing cardiovascular patient outcomes.

Residual risk, the ongoing appreciable risk of major cardiovascular events (MCVE) in statin-treated patients who have achieved evidence-based lipid goals, remains a concern among cardiologists. Factors that contribute to this continuing risk are atherogenic non-low-density lipoprotein (LDL) particles and atherogenic processes unrelated to LDL cholesterol, including other risk factors, the inherent properties of statin drugs, and patient characteristics, ie, genetics and behaviors. In addition, providers, health care systems, the community, public policies, and the environment play a role. Majo
