[Endoscopic removal of a fibroepithelial ureteral polyp].

Fibroepithelial polyps are rare benign ureteral neoplasms of mesodermal origin, found mostly in the proximal ureter. Precisely in these cases, ureteroscopy is extremely important in diagnosis and therapy. We report on a case of a distal fibroepithelial polyp that was removed endoscopically. The base of the tumour was coagulated by laser.

Severe hemorrhage secondary to primary, isolated in situ carcinoma of renal pelvis.

Primary transitional cell carcinoma in situ of the renal pelvis is usually found in association with ureteral or vesical neoplasms. However, a case of primary carcinoma in situ of the renal pelvis without previous or concomitant ureteral or vesical carcinoma is presented. Microscopic hematuria is a common finding in cases of carcinoma in situ. Gross hematuria is unusual, but not rare. However, severe hemorrhage (hematocrit 12.1, hemoglobin 3.3 g/dL) as the presenting feature is rare. Diagnosis of such microscopic isolated lesions can be difficult to make pre-operatively. Only after nephrectomy

A Giant Ureteral Inflammatory Myofibroblastic Tumor in a 4-Year-Old Child: A Case Report and Review of the Literature.

Inflammatory myofibroblastic tumor (IMT) is a rare neoplasm of children and young adults. It consists of myofibroblastic cells of borderline malignancy admixed with inflammatory cells. According to the WHO classification, of tumors, it is a soft-tissue tumor with an intermediate malignant potential. It may arise within numerous organs. Ureteral location is exceedingly rare. We report the exceptional case of a 4-year-old boy presenting with a giant right ureteral IMT removed during a nephroureterectomy. Histology and immunohistochemistry confirmed the diagnosis of an IMT with Anaplastic Lymphom

Radiologic features of the interureteric ridge.

Careful attention to the dimensions and symmetry of the interureteric ridge, when it is visible, aids in interpretation of abnormalities in the bladder and ureters. It is important in evaluating renal agenesis, low ureteral calculi, trauma, and neoplasm.

Pelvi-ureteric obstruction due to renal pelvis cyst.

Intramural cysts of the renal pelvis are a separate clinical entity and must not be mistaken for neoplasms. Simple enucleation can result in complete restoration of renal function previously impaired by pelvi-ureteric obstruction.

Comparison of ultrasonography and intravenous urography in the screening and diagnosis of hematuria causes.

INTRODUCTION: Our aim was to compare transabdominal ultrasonography (US) and intravenous urography (IVU) in the evaluation of patients with hematuria. MATERIALS AND METHODS: Two hundred patients with hematuria were assessed by US and IVU, and if needed, by cystoscopy, ureteroscopy, and CT scan, to determine the definite cause of hematuria. The results of US and IVU were compared according to the definite diagnoses. RESULTS: Of 97 patients with microscopic hematuria, 44 (45%) had a documented cause for hematuria, and of 103 patients with gross hematuria, 76 (74%) had a definite disorder (P <

Idiopathic segmental ureteritis, misdiagnosed as ureteral cancer preoperatively: a case report with literature review.

A 53-year-old man presented with right flank pain for 6 days. Computerized tomography revealed a 3 cm long segment of ureteral narrowing with wall thickening and hydronephrosis, suspicious for ureteral cancer. Under the clinical diagnosis of ureteral carcinoma a right nephroureterectomy was performed. The wall of the distal ureter, 2.5 cm from the bladder cuff, had a luminal-narrowing, firm mass-forming lesion with abrupt transition from the adjacent ureter. Histologically, the resected ureteral mass showed transmural fibrosing, chronic inflammation with numerous plasma cells, epithelioid gran

Primary non leukemic myeloid sarcoma of the ureteral wall: a case report of a rare disease.

Myeloid sarcoma (MS) is an extramedullary tumor mass causing proliferation of mature or immature blast cells of one or more myeloid lineages. Involvement of the genitourinary tract is rare. We present a case of MS of the ureteral wall. A 74-year-old man was evaluated for left hydronephrosis and ipsilateral low back pain. A computed tomography scan showed a nodular formation in the pelvic ureter. Urinary cytology revealed cellular atypia, so ureteroscopy was performed showing a distal ureteral mass. The histological examination of the biopsy revealed to be malignant neoplasm. The patient underw

An unusual testicular metastasis from transitional cell carcinoma of the ureter: a case report.

Testicular tumor metastasis from transitional cell carcinoma (TCC) of the ureter is very rare. Herein, we report a case who presented with a palpable mass of left testis, after he underwent radical nephroureterectomy and bladder cuff excision and adjuvant radiotherapy and chemotherapy for the TCC of left ureter tumor for 5 months. We performed left radical orchiectomy and histology revealed a TCC identical to the original ureteral tumor. In a review of the literature, we found this case was the second case of metastatic testicular tumor from the ureteral TCC. In patients presenting the symptom

Laparoscopic Anderson-Hynes procedure as a treatment of ureteropelvic junction obstruction caused by fibroepithelial polyp.

Fibroepithelial polyps (FEPs) are rare benign ureteral neoplasms of mesodermal origin. As an intrinsic cause of ureteropelvic junction obstruction (UPJO) they are very uncommon. Excretory urography and diuretic renography used by many urologists to diagnose a UPJO may not detect the ureteral polyp. We present a case of ureteropelvic junction obstruction caused by a fibroepithelial polyp, which was not diagnosed preoperatively. The patient underwent successful laparoscopic excision of the polyp with Anderson-Hynes pyeloplasty. We believe that laparoscopy is a useful, minimally invasive operativ…

[Primary adenocarcinoma of the ureter. Case report].

Primary ureteral adenocarcinoma is an infrequent histological type of urinary neoplasm. Many authors consider intestinal metaplasia the pivotal event of the pathogenetic process, whether it occurs on a pre-existing urothelial carcinoma or on a normal urothelium. Diagnosis is essentially based on case history and clinical findings (hematuria and pain) and on diagnostic imaging. Treatment is surgical and the ideal procedure is nephroureterectomy with excision of a bladder margin adjacent to the ureteral opening and ispilateral para-aortoiliac lymphadenectomy. A 76-year-old man with primary adeno

Percutaneous nephrostomy: experience in 107 kidneys.

We were successful using percutaneous nephrostomy to relieve upper urinary obstruction in 98 of 107 kidneys. In two thirds of our patients obstruction was due to neoplasm, with prostatic cancer the leading cause. Percutaneous nephrostomy proved to be ideally suited for drainage of pyonephrosis and in azotemic patients. Its use can obviate surgical nephrostomy and prolonged ureteral catheter drainage. We also are encouraged by our early experience with extensions of percutaneous nephrostomy, including percutaneous ureteral stenting, renal stone dissolution, and percutaneous pyelolithotomy.

Retroperitoneal fibrosis.

Retroperitoneal fibrosis is the formation of a fibrotic plaque in the prelumbar or presacral area which becomes clinically significant when it produces ureteral obstruction. It can occur as a result of taking certain drugs, retroperitoneal fluid collections, inflammatory disease of various intraabdominal organs, previous surgery or radiation therapy, and various metastatic neoplasms. The majority of the causes of retroperitoneal fibrosis are considered idiopathic.

New instrument for placement of circle tube nephrostomy.

The circle tube nephrostomy recently has become a popular method for supravesical urinary drainage. A new instrument and technique to facilitate placement of the circle tube have been developed and used successfully in 16 patients requiring nephrostomy drainage for ureteral obstruction secondary to advanced pelvic neoplasms. This instrument and technique offer a simple, safe and effective method of establishing circle tube drainage.

[Urothelial neoplasia as a late complication of transitional-cell carcinoma of the bladder].

We evaluated the long term incidence of lower urinary tract neoplasms in patients with primary transitional bladder carcinoma. 8 patients had lower urinary tract neoplasm after a median elapse of time of 44 months (range 28 to 70). In 5 patients wa present vesicoureteral reflux. All the patients were undergone trial of BCG after the first recurrence of bladder carcinoma. We suppose that the vesicoureteral reflux may be the possible cause of ureteral neoplasm onset. Because of patients with primary bladder tumors are at the risk for lower urinary tract neoplasm, we suggest the need regular and

[The ureters: findings at multidetector computed tomography].

Multidetector computed tomography (MDCT) and advances in CT urography techniques have enabled vast improvements in the depiction of the ureter. Studies of the ureter can find a wide variety of conditions including congenital defects and anatomic variants (anomalies in the origin, distribution, and distal insertion of the ureter) as well as all benign and malignant causes of focal and diffuse wall thickening (inflammatory and infectious processes, and neoplasms, as well as iatrogenic thickening and postsurgical changes). Other benign processes like ureteral kinking and stenosis due to extrinsic

Excretory urographic demonstration of ureteral displacement by sigmoid fecal impaction simulating retroperitoneal tumor.

Two cases of curvilinear lateral ureteral displacement in children with abdominal masses are reported. The masses proved to be sigmoid fecal impactions. This possibility must be considered before embarking on radiographic or surgical evaluation of possible retroperitoneal neoplasm.
