A 63-year-old female presented with a tension urinothorax after pyeloscopy, lithotripsy, and percutaneous nephrolithotomy that manifested as dyspnea and abdominal pain. CT Thorax demonstrated a large right pleural effusion with middle and lower lobe collapse with leftward shift concerning for tension hydrothorax. Analysis of the effusion after pigtail catheter demonstrated an exudative effusion with resultant resolution of the effusion after a 5-day hospital course. This case illustrates a uniquely rare complication of percutaneous urologic intervention. We highlight this case to stress the im

PURPOSE: Stent omission after routine ureteroscopy (rtURS) is accepted by current guidelines and may result in decreased patient morbidity and treatment costs. In a value-based healthcare model, the added morbidity and cost of routine stent placement may be scrutinized. Furthermore, data are limited on urologist cost knowledge and it is effect on ureteral stent placement. As such, we seek to describe ureteral stenting practices and urologist cost knowledge amongst US and non-US-based urologists.
METHODS: The ureteroscopic practice patterns and cost awareness of members of the Endourological S

INTRODUCTION: We aimed to evaluate factors predictive of negative ureteroscopy (URS) in ureteral stones.
METHODS: Patients who underwent URS between January 2007 and June 2018 were included in the study. Patients were divided into two groups: group 1- positive URS (841 patients); and group 2 -negative URS (75 patients). These two groups were compared in terms of demographic data, stone characteristics, and postoperative outcomes.
RESULTS: The mean age of the study patients was 44.5±15.1 years. The absence of collecting system dilatation due to the present stone was found to be a significant

BACKGROUND AND PURPOSE: Routine imaging following ureteroscopy for treatment of renal/ureteral calculi continues to be a topic of debate. However, with the increasing focus on healthcare costs and quality, judicious use of diagnostic imaging to optimize outcomes while minimizing resource utilization is a priority. We sought to identify post-ureteroscopy imaging practices among experienced urologists.
MATERIALS AND METHODS: A REDcap questionnaire was sent to urologists in North America. The questionnaire surveyed demographic data, clinical volume, and imaging preferences post-ureteroscopy. Add

OBJECTIVES: An obstructed, infected kidney combined with ureteral stones can be lethal, and requires urgent drainage and complete stone removal. However, the optimal method of stone removal, and its safety and efficacy have yet to be conclusively established. The aim of this study was to determine the safety and efficacy of carrying out ureteroscopy after kidney drainage for septic patients with obstructing stones.
METHODS: From January 2004 to September 2011, 88 patients underwent stone removal by either ureteroscopy (n = 48) or extracorporeal shock wave lithotripsy (n = 40) after drainage o

PURPOSE: Recent studies have demonstrated deleterious effects of ionizing radiation from diagnostic and therapeutic imaging procedures. One of the barriers to minimizing patient exposure is physician awareness. We prospectively studied whether providing surgeons with feedback on their fluoroscopy utilization would affect intraoperative fluoroscopy times.
MATERIALS AND METHODS: In 2007, we prospectively began to track fluoroscopy usage for all urology cases. Nine months later, surgeons started to receive periodic reports with their mean fluoroscopy time compared with their peers. We reviewed a

We report a 28-year-old man who presented with hematuria and intermittent flank pain. Conventional CT urography showed mild left hydronephrosis and an undetermined lesion in the left ureter. By virtual CT ureteroscopy, a fibroepithelial polyp (FEP) was preoperatively diagnosed. The patient had segmental resection of the ureter and pathological proof of FEP was obtained.

We report our experience on performing ureteroscopic procedures using local anaesthesia with or without intravenous analgesia. During a two year period 334 ureteroscopic procedures were performed in our hospital. 159 of them were begun but only 138 completed without the use of general or regional anaesthesia. In 9 patients ureteroscopy was performed with lindocaine jelly in the urethra only, and in 129 with additional intravenous analgesia Fentanyl, a synthetic morphine derivative, was used for intravenous analgesia. Ureteroscopy was performed for stone fragmentation or extraction in 119 patie

PURPOSE: Flexible Ureteroscopy (FURS) in patients with a solitary kidney (SK) has been sparsely studied, especially its safety. Available literature suggests that the renal function remains stable after FURS with low evidence. We aimed to assess and predict the postoperative renal function after FURS in patients with SK, using Machine Learning (ML) models.
METHODS: A dualcenter retrospective study was conducted between 2013 and 2023, including patients undergoing FURS in solitary kidneys. Demographic, peri-operative data, pre- and postoperative serum creatinine and estimated Glomerular Filtra

INTRODUCTION: Ureteroscopy (URS) is commonly performed under general anesthesia (GA) to maximize patient tolerability and minimize surgical complications; however, given the improvements in endoscopic technology and risks associated with GA, alternate forms of anesthesia have been postulated. We aimed to evaluate the outcomes of URS under conscious sedation.
METHODS: We completed a retrospective cohort study from November 2019 to June 2020 at a tertiary-level hospital. All URSs that were performed under urologist-directed conscious sedation were included. Our primary outcome was the ability t

The aim of this article is to illustrate a step-by-step guide to flexible ureteroscopy (URS) for treatment of upper urinary tract calculus. Preoperative evaluation of the patient with focus on comorbidities is paramount for the type of stone procedure to be performed. In this article, the steps of operating room setup, patient positioning, gaining access to the upper urinary tract, flexible URS, laser lithotripsy, basket extraction of stone fragments, and placement of a ureteral stent are described. The importance of clearance of the ureter before flexible URS in the case of pre-existing urete

The use of ureteral access sheaths (UAS) is common practice during routine flexible ureteroscopy procedures. However, debates and concerns continue amongst endourologists on routine UAS placement. UAS placement allows for multiple passages of the ureteroscope, decreases intrarenal pressure, and may improve stone-free rates. However, concerns for the UAS's effectiveness in these claimed benefits and complications related to UAS placement has been documented and investigated by many. In this review, we will discuss the controversies surrounding the placement of UAS during ureteroscopy.
