[Appropriate and inappropriate use of indwelling urinary catheters].

Many hospitalized patients receive a urinary catheter during their stay. In 21-54% of patients, however, there is no appropriate indication for this. The most significant complication caused by the use of urinary catheters is the development of a urinary tract infection (UTI), one of the most common nosocomial infections. In 71-80% of hospital acquired UTIs a urinary catheter is present. The duration of the presence of a catheter is the major risk factor for catheter-associated UTI. Reducing the number of inappropriate catheterisations is an effective way of preventing catheter-related UTIs. I

Characterization of bacterial biofilms formed on urinary catheters.

BACKGROUND: The formation of bacterial biofilms on urinary catheters is a leading cause of urinary tract infections in intensive care units. Cytobacteriological examination of urine from patients is often misleading, due to the formation of these biofilms. Therefore, characterizing these biofilms and identifying the bacterial species residing on the surface of catheters are of major importance. METHODS: We studied the formation of biofilms on the inner surface of urinary catheters using microbiological culture techniques, with the direct contact of catheter pieces with blood agar. The bacteri

Viscoelastic property mapping along encrusted polymeric urinary catheters.

PURPOSE: Mapping of the material viscoelastic property variation along the length axis of polymeric stents. MATERIALS AND METHODS: Five pigtail ureteral stents and five percutaneous stents placed in vivo for different periods were studied. Viscoelastic property changes along the length of polymeric stents were measured using the dynamic mechanical analysis technique. The type of encrustations was identified using FT-IR spectroscopy and their morphology by scanning electron microscopy (SEM). RESULTS: Variations in viscoelastic stiffness were confirmed for both types of stents. In a few cases,

Urinary catheters: selection, management and prevention of infection.

Urinary catheterization is a common procedure for both hospital and community patients. Nurses make many of the decisions in regard to both catheter selection and subsequent catheter care. These decisions, for example, in the type of catheterization (intermittent, indwelling, urethral or suprapubic) should be made on an informed basis. Choosing the optimum catheter material and size can benefit catheter care. Encrustation can be a problem for some catheterized patients and nurses are often involved in trying to prevent or treat it. This and other aspects of catheter management including infect

Resistance of conformable indwelling urinary catheters to encrustation.

Conformable and conventional indwelling urinary catheters, made of latex coated with Teflon and of the same nominal size, were encrusted in vitro. The extent of encrustation was quantified by dissolving the deposits, determining the concentrations of calcium and magnesium in the resulting solutions (by atomic adsorption spectroscopy), and calculating the surface densities of calcium and magnesium. Surface densities of both elements were significantly less on the conformable than on the conventional catheters. This result can be explained by the pulsed flow of urine through the conformable cath

Biocompatibility of urinary catheters. Effect on complement activation.

The ability of 2 brands of urinary catheters to activate the complement system in vitro as a marker of biocompatibility was investigated. Pieces of a silicone/latex catheter and an all-silicone catheter were incubated in human serum. Complement activation was tested in 2 enzyme immunoassays, one evaluating C3 activation and the other the terminal complement complex (TCC) formation. The silicone/latex catheter caused considerably more complement activation in both assays than did the all-silicone one. The findings are in agreement with previous results of tissue toxicity tests. The methods pres

The effect of hydron on latex urinary catheters.

Hydron, a polymeric gel which permits the passage of small molecules through its interstices, was used to coat latex urethral catheters. These catheters, as well as other catheters of varied composition, were exposed to a liquid environment from which struvite crystals could be precipitated. The deposition of struvite on a hydron coated latex catheter was significantly less than upon the other catheters.

Urinary Catheter Management.

Family physicians often treat patients who require urinary management with the use of external urinary devices, clean intermittent catheterization, or indwelling urinary catheterization. External urinary devices are indicated for urinary incontinence (postvoid residual less than 300 mL), urine volume measurement for hospitalized patients, nonsterile urine diagnostic testing, improved comfort for patients in hospice or palliative care, and fall prevention for high-risk patients. Indwelling urinary catheterization is indicated for severe urinary retention or bladder outlet obstruction; wound hea

Urinary catheter management.

After colorectal resection surgery, early urinary catheter removal has been promoted as a part of the national Surgical Care Improvement Project. However, the decrease in urinary tract infection expected with this strategy must be balanced against an increased risk for urinary retention. A systematic review of the literature was undertaken to summarize the evidence for and against early postoperative urinary catheter removal. For nonpelvic colorectal resection, the evidence supports removal of the catheter on postoperative day 1 for patients who are not at high risk for urinary retention, incl

Inhibiting host-protein deposition on urinary catheters reduces associated urinary tract infections.

Microbial adhesion to medical devices is common for hospital-acquired infections, particularly for urinary catheters. If not properly treated these infections cause complications and exacerbate antimicrobial resistance. Catheter use elicits bladder inflammation, releasing host serum proteins, including fibrinogen (Fg), into the bladder, which deposit on the urinary catheter. Enterococcus faecalis uses Fg as a scaffold to bind and persist in the bladder despite antibiotic treatments. Inhibition of Fg-pathogen interaction significantly reduces infection. Here, we show deposited Fg is advantageou

Reduction of unnecessary use of indwelling urinary catheters.

BACKGROUND: The most effective way to reduce catheter-associated urinary tract infections (CA-UTIs) is to avoid unnecessary urinary catheterisation and to minimise the duration of catheterisation. AIM: To implement and assess the effect of an intervention to reduce the duration of urinary tract catheterisation. METHODS: This quality improvement project was set up as a before-after comparison consisting of a 2-month pre-intervention period, a period in which the intervention was implemented and a 2-month post-intervention period. The intervention included educational sessions to increase phys

Indwelling urinary catheters: accurate monitoring of urine output.

Caring for patients who have an indwelling urinary catheter is a common feature of clinical nursing practice in the acute care environment, which for some patients is essential to their medical management. However, evidence suggests that in many cases catheterization may be unjustified, exposing the patient to a significant risk of acquiring a urinary tract infection, which extends length of stay and imposes an extra burden of care and cost. Improving the care for these patients depends on good education, implementation of best practice, adequate resources, and effective devices and treatment.

Optimization of silicone urinary catheters for hypospadias repair.

OBJECTIVE: We have experienced difficulty with the removal of all-silicone Foley catheters after hypospadias repair, relating to the formation of a 'cuff' of residual balloon material that fails to deflate, after aspiration of the instilled volume of water. This could potentially lead to both short- and long-term complications (stenosis, fistula). In all-silicone paediatric catheters, we investigated the production of such 'cuffs', and any other significant deformity which may be associated with deformation of the catheter balloon mechanism, in vitro. MATERIALS AND METHODS: Catheters were inf

Bacterial biofilms in patients with indwelling urinary catheters.

Bacteria have a basic survival strategy: to colonize surfaces and grow as biofilm communities embedded in a gel-like polysaccharide matrix. The catheterized urinary tract provides ideal conditions for the development of enormous biofilm populations. Many bacterial species colonize indwelling catheters as biofilms, inducing complications in patients' care. The most troublesome complications are the crystalline biofilms that can occlude the catheter lumen and trigger episodes of pyelonephritis and septicemia. The crystalline biofilms result from infection by urease-producing bacteria, particular

Patient-perceived discomfort with two coated urinary catheters.

Intermittent catheterization is now the preferred method for urethral catheterization of the urinary bladder when an indwelling catheter is not required. Nevertheless, the procedure may cause discomfort or pain. This randomized, prospective study was done to compare patient-perceived discomfort between two commonly used hydrophilic, single-use catheters in women. A total of 196 women referred to the urogynaecology section in the outpatient department were randomized to be catheterized with either a LoFric (n=102) or a SpeediCath (n=94) catheter. Both catheters are hydrophilic, low friction, si
