Urinary tract infections (UTIs) are common conditions in clinical practice. For uncomplicated UTIs, the causative organisms and their antimicrobial susceptibility profiles are generally predictable, and empiric short course (3 day) antibiotic therapy after an abbreviated laboratory workup is advocated. The retrospective analysis had been performed on 3480 newborns weighted more than 1001 g, over the period of thirty one months. The clinical conditions that cause dysuria in women can usually be differentiated by the history and selected physical and laboratory examinations. From January 1981 to December 1987, 346 children with urinary tract infections, proved by urine culture, were admitted to the Department of Pediatrics, Mackay Memorial Hospital. A five-year review of pediatric patients with urinary tract infections shows that absolute colony counts are not satisfactory as the sole criterion for urinary tract infection. Prior to culture results, patients can be treated empirically if the urine is nitrite-positive or if there is more than an occasional organism per high power field on unspun urine. In this series, no diagnostic or therapeutic benefit resulted from localizing studies. Patients with enuresis and infection should undergo radiologic evaluation for an anatomic abnormality.

INTRODUCTION: Children with recurrent urinary tract infections (rUTI) often show no identifiable cause of their infections. Neutrophil gelatinase-associated lipocalin (NGAL) is known to be upregulated within the uroepithelium and kidney of patients with UTI and exhibits a localized bacteriostatic effect through iron chelation. We hypothesize that some patients with rUTI without an identifiable cause of their recurrent infections have locally deficient NGAL production.
