A 64-year-old female presented to the ED with severe abdominal pain. Initially it was suspected to be spontaneous aortic rupture or dissection. We report on a male newborn with urinary ascites and perirenal urinoma due to calyceal rupture caused by bladder outlet obstruction and posterior urethral valves. Despite the presence of left-sided high-grade reflux, the "pop-off" mechanism occurred in the non-refluxing right kidney. Impressive normalization of the upper urinary tract anomalies and renal function occurred after surgical treatment of the valves and removal of obstruction.

The appearance of fetal urinoma is rare in prenatal care, especially when associated with polyhydramnions. Many previous reports have concluded that the visualization of a prenatal urinoma is probably a sign of underlying renal dysplasia or poor function. Thus, the management of the reported cases, conservative treatment or uniroma drainage, has not been unified. In this paper, we present two cases of prenatally detected urinoma with prenatal drainage to improve the prognosis.

BACKGROUND: Rupture of the urinary collecting system with peripelvic extravasation of urine is an uncommon pathologic condition usually associated with ureteral obstruction from calculi.
CASE: We report a patient with calyceal rupture and peripelvic extravasation of urine secondary to distal ureteral obstruction by recurrent ovarian carcinoma. Diagnosis was established with computed tomography and renal scans. Placement of an indwelling ureteral stent via a nephrostomy resolved the urinoma.
CONCLUSION: Gynecologic oncologists should be aware that calyceal rupture is a potential complication

One case of ureterohydronephrosis fistulized into an urinoma in a patient with an aortobifemoral prosthesis is reported. The analysis of the literature shows that this kind of expression is exceptional, both because of the location of the urinoma and of its tuberculous origin.

Congenital anterior urethral diverticulum (AUD) is a rare condition. We report a case of AUD associated with urinary ascites, bilateral urinomas and dysplastic kidneys. The diagnosis of urethral obstruction was established in utero by means of ultrasound. The right perirenal urinoma appeared "septated", and we believe that this is related to its hemorrhagic content. Improvement of the clinical condition and renal function was noted after decompression by bilateral cutaneous loop ureterostomy.

Leakage from the kidney, ureter, bladder, and urethra, which mainly occur due to trauma, forms urinoma. The best tool for diagnosis of urinoma is enhanced-computed tomography with delayed imaging. Apart from injury, ureteral stone can induce urinoma, however, it is rare. Herein, we report a case with urinoma, which probably formed as a result of overdistention of renal pelvis after a large amount of fluid intake (about 3 L) in a short period of time (about an hour) in the presence of a 3mm stone in ipsilateral ureter.

Fetal pelvicaliceal dilatation due to ureteropelvic junction obstruction is the most common cause of antenatal hydronephrosis; it rarely leads to a spontaneous rupture resulting in urinoma formation. A large contained urinoma measuring 12 x 8 x 8 cm3 was diagnosed in the left kidney of a female fetus at 28 weeks gestation. Analysis of fluid from serial punctures of the cyst indicated that renal function on that side was poor, but because ultrasound findings of the thorax suggested lung growth impairment, a shunt was inserted. A 60-year-old woman with an adenocarcinoma of the lung was referred to our department for a bone scan to assess the extension of the metastatic bone disease. The images showed several foci of radiotracer uptake in the bone consistent with malignant involvement. Besides, a considerable accumulation of tracer was shown in the right perinephric region as well as in the renal pelvis. SPECT/CT images demonstrated a superior perirenal urine leak due to rupture of the collecting system. The leakage was treated by urinary catheter decompression with complete recovery after 4 weeks.

OBJECTIVE: To analyze the management of prenatal urinoma and ureteropelvic junction obstruction (UPJO) postnatally by a case report and literature review.
METHODS: We report a case of an UPJO and urinoma in a newborn. After birth, renal function was absent, and nephrectomy was performed. The literature from 1985 to date has been reviewed.
