The motives for performing conservation surgery in tumors of the upper urinary tract are progressive renal failure associated with those tumors, frequent bilateral tumors, and tumors on solitary kidney, which does not always comply with the patho-anatomic characteristics of the tumor and possibilities of this type of surgery. We report an unusual pattern of immunostaining for chromogranin A in 24 transitional cell carcinomas (TCC). Positive immunostaining was seen with chromogranin A antisera in 10 of 16 urinary bladder TCC, neither of 2 prostatic TCC, 3 of 3 ureteral TCC and 2 of 3 metastatic TCC. Complete histologic mapping of the urothelium was performed in nine surgical nephroureterectomy specimens. In seven patients, the primary lesion was carcinoma of the renal pelvis; in two, carcinoma of the ureter. In all cases, in areas peripheral to obvious carcinomas, there were extensive epithelial abnormalities akin to those previously described in mapping studies of the urinary bladder.
