A Review on Headaches Due to COVID-19 Infection.

Since December 2019, the time when the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) was spotted, numerous review studies have been published on COVID-19 and its neuro invasion. A growing number of studies have reported headaches as a common neurological manifestation of COVID-19. Although several hypotheses have been proposed regarding the association between headache and the coronavirus, no solid evidence has been presented for the mechanism and features of headache in COVID-19. Headache also is a common complaint with the omicron variant of the virus. COVID-19 vaccination als

Nummular and Side-locked Headaches for the Otolaryngologist.

Side-locked headaches are a common symptom having a wide-ranging differential. Unchanging in laterality, these headaches can represent neuralgias, trigeminal autonomic cephalgias, ophthalmologic disorders, otolaryngologic and craniofacial disorders, vascular disorders, and malignancy. In rarer situations, they have presented secondary to neurosurgical or dermatologic considerations. Loss of cranial nerves and visual changes warrant additional evaluation.

Dedicated neuroimaging analysis in children with primary headaches: prevalence of lesions and a comparison between patients with and without migraines.

BACKGROUND: This study evaluated the prevalence and types of intracranial lesions through dedicated imaging analysis of primary headaches in children and compared them between patients with and without migraine. METHODS: This study included 190 children diagnosed with primary headache who underwent neuroimaging, including brain computed tomography (CT), CT angiography (CTA), and brain magnetic resonance imaging (MRI). All patients with primary headaches was divided into two groups, namely, the migraine and non-migraine groups, on the basis of data from electronic medical records. Clinical cha

[Diagnostic imaging of headache].

The diagnosis of headache is based on the subjective symptom, and it is generally classified into the primary headaches, which have intracranially no abnormality, and the secondary headaches, which have organic abnormality. Neuroimaging examinations, computed tomography (CT) scan, and magnetic resonance imaging (MRI) are useful for differential diagnosis and can perform to rule out vascular malformation, brain tumor, and other pathologies, and to confirm the diagnosis. There are cases of the secondary headaches resembling the primary headaches and patients complaint of pain by complex of them,

[Primary headache and its contemporary management].

The majority of headache complaints are caused by primary headaches (migraine, tension headache, cluster headache and rare headache types). The diagnosis of headache syndromes is based on the anamnesis, therefore it is only made possible by a thorough knowledge of the clinical symptoms. Although the exclusion of any intracranial pathology is a diagnostic criterion, performing a neuroimaging examination is only necessitated by an atypical clinical picture which rarely occurs. A common characteristic of this headache group is that the etiology and pathomechanism are still not clear, nevertheless

Headache. Pharmacological approach to treatment.

The great majority of headaches a physician treats in office practice can be divided into two main categories, muscular contraction headache of tension type and vascular headaches of the migraine type. The most satisfactory symptomatic therapy for tension headache is by the use of a nonnarcotic analgesic agent combined with a tranquilizer or sedative. On the other hand, symptomatic relief of migraine is best obtained by the use of a suppository of ergotamine tartrate and caffeine combined with an antiemetic or antispasmodic. Interval treatment of patients with tension and migraine headache cen

Posttraumatic Headaches and Postcraniotomy Syndromes.

Headaches following head trauma or craniotomy have multiple causes, each of which has characteristic imaging features. Posttraumatic headaches may relate to intracranial hemorrhage, fracture, shear injury, mass effect, or vascular injury. Various complications of craniotomy and craniectomy may manifest with headache. CT and MRI of the brain play important roles in diagnosing these causes of headache.

Parenteral chlorpromazine and a meningitis headache.

Parenteral chlorpromazine is a frequently used agent in the acute management of tension and vascular headaches. However, headaches caused by other more serious diseases may also respond to this drug. This case report describes a patient with aseptic meningitis who experienced complete but temporary relief of her headache with parenteral chlorpromazine, prior to the eventual diagnosis.

The value of brain imaging in children with headaches.

OBJECTIVE: To determine the value of performing computed tomography (CT) on magnetic resonance imaging (MRI) studies in children with chronic headaches. BACKGROUND: Headache is a common complaint in children. With the proliferation of brain imaging centers and the increasing patient demand for CT or MRI studies, brain imaging has become widely used to evaluate headaches. METHODS: A retrospective chart review was conducted of all children referred to the pediatric neurology clinic for evaluation of headaches over a 2-year period. Charts were reviewed for headache characteristics, clinical ind

Early Vascular Ageing in adolescents with migraine with aura: a community-based study.

BACKGROUND: Migraine with aura is associated with an increased risk of cardiovascular disease, yet the pathophysiology is unknown. Suggested underlying mechanisms of aura formation point into the direction of an abnormal vasoreactivity that also extends to the extracranial vasculature. METHODS: In the Early Vascular Ageing Tyrol study, a community-based non-randomized controlled trial conducted in 45 schools and companies in Tyrol (Austria) and South-Tyrol (Italy) between May 2015 and September 2018 aiming to increase cardiovascular health in adolescents, headache syndromes were classified ac

Magnetic resonance angiography in facial and other pain: neurovascular mechanisms of trigeminal sensation.

For much of the twentieth century migraine and cluster headache have been considered as vascular headaches whose pathophysiology was determined by changes in cranial vascular diameter. To examine nociceptive neural influences on the cranial circulation, the authors studied healthy volunteers' responses to injection of the pain-producing compound capsaicin in terms of the caliber of the internal carotid artery. The study was conducted using magnetic resonance angiographic techniques. Injection of capsaicin into the skin innervated by the ophthalmic (first) division of the trigeminal nerve elici

Reversible cerebral vasoconstriction syndrome in children: an update.

Headaches are one of the most common neurologic complaints leading to emergency room visits in pediatric patients. Of the different type of headache presentations, thunderclap headaches require a particularly urgent work-up. In children, recurrent thunderclap headaches are more often associated with reversible cerebral vasoconstriction syndrome (RCVS) than other etiologies such as subarachnoid hemorrhage. RCVS is a vascular disorder of incompletely understood etiology, characterized by diffuse vasoconstriction of the cerebral arterial vasculature, and commonly associated with recurrent severe

Colorflow duplex ultrasonography determines the presence and nature of vasculogenic headaches.

Eighteen consecutive patients with severe headache syndromes underwent noninvasive interrogation to explore the intracranial vascular involvement in their headaches. The authors used a colorflow duplex scanner with a 2.25 MHz, tightly curved, phased-array transducer to insonate and visualize the terminal segments of the vertebral arteries; the anterior, middle, and posterior cerebral circulations; and the basilar artery. Blood flow velocities and vessel caliber were obtained during a headache-free period and during a grade 8-10 headache in all patients. Comparisons of these data indicated that

Imaging of Headache in Pregnancy.

Pregnant women are most likely to have primary headaches, such as migraine and tension-type headaches, which can be diagnosed and treated without brain imaging. Primary headaches may even start de novo during pregnancy, especially in the first few months. However, when the headache occurs late in pregnancy or in the peripartum period, secondary causes of headaches need to be considered and evaluated by brain and/or vascular imaging, generally using magnetic resonance techniques. There is considerable overlap between the cerebrovascular complications of pregnancy, including preeclampsia/eclamps

Tension headache.

BACKGROUND: Tension headache and migraine are opposite ends of a benign recurrent headache spectrum. OBJECTIVE: To provide an approach to the diagnosis of benign recurrent headache. DISCUSSION: Tension headache is characterised by symmetry, non-disabling severity and the absence of vascular features (throbbing quality, nausea and photophobia). The differentiation between tension headache and attacks with more vascular features has important therapeutic implications as attacks with significant vascular features tend to respond better to antimigrainous approaches rather than those employed in

[Clinical characteristics of cluster headache].

The authors describe in detail the clinical characteristics of 48 patients suffering from cluster headache. They investigate the question of nomenclature and survey the clinical forms of diagnosis. They deal with differential diagnosis comparing migraine, trigeminal neuralgia and headaches origin from systemic illness. They point out that the clinical characteristics of their patients co-responding to the data in the literature. They emphasize that the precise anamnesis can result the diagnosis of this type of headache which is the basis of the treatment.

Cerebral blood flow changes in cluster headache.

Serial cerebral blood flow studies performed by the intra-carotid 133Xenon method were fortuitously determined during the course of a cluster headache in a 32 year old man. The initial study was performed about 10 min after the headache began and showed values at the upper limit of normal. Twenty min after the headache started a second procedure showed that the autoregulatory response on hyperventilation was normal. Ergotamine tartrate was given intra-muscularly 23 min after the headache began and there was partial relief. A third cerebral blood flow estimation showed abnormally high values. T
