Surgical reconstruction of major bile duct injuries: Long-term results and risk factors for restenosis.

BACKGROUND: A single-institution retrospective analysis was undertaken to assess long-term results of definitive surgical reconstruction for major bile duct injuries and risk factors for restenosis. METHODS: Patients treated between January 1995 and October 2020 were reviewed retrospectively. The primary outcome measure was patency. RESULTS: Of 417 patients referred to a tertiary center, 290 (69.5%) underwent surgical reconstruction; mostly in the form of a hepaticojejunostomy (n = 281, 96.8%). Major liver resection was undertaken in 18 patients (6.2%). There were 7 postoperative deaths (2.4

The Alpha to Omega of Dialysis Access: Arteriovenous Fistula and Graft (Part 1).

Background: Vascular access is a critical determinant of hemodialysis efficacy in patients with end-stage kidney disease (ESKD). The choice between arteriovenous fistulas (AVFs) and arteriovenous grafts (AVGs) influences long-term dialysis outcomes, with AVFs offering superior patency but higher maturation failure rates and AVGs providing immediate usability at the expense of increased complications. Recent advancements in endovascular techniques and biomaterials have introduced novel approaches to optimizing vascular access. Purpose: This review examines the latest evidence on AVF and AVG cre

Long Term Outcome of Brachial-Basilic Arteriovenous Fistulas.

INTRODUCTION: Autogenous arteriovenous fistulas are the preferred vascular access for hemodialysis due to superior patency and lower infection rates compared with grafts or catheters. The brachiobasilic AV fistula provides a valuable option for patients with unsuitable veins, though it remains underutilized due to technical challenges and wound morbidity. This study aimed to evaluate the clinical outcomes, patency, and complications of brachiobasilic-AV fistula creation over a 10-year period at a tertiary vascular center. METHODS: A retrospective descriptive analysis was conducted of all pati

Heparin-bonded expanded polytetrafluoroethylene vascular graft for femoropopliteal and femorocrural bypass grafting: 1-year results.

OBJECTIVE: Several prosthetic materials have been used for femoropopliteal bypass grafting in patients with peripheral vascular disease in whom a venous bypass is not possible. Expanded polytetrafluoroethylene (ePTFE) is the most commonly used, but patency results have not always equaled those achieved with vein, especially in below-knee reconstructions. This study assessed the performance of a new heparin-bonded ePTFE vascular graft that was designed to provide resistance to thrombosis and thereby decrease early graft failures and possibly prolong patency. METHOD: From June 2002 to June 2003

Promising Results of the Forearm Basilic Fistula Reveal a Worthwhile Option between Radial Cephalic and Brachial Fistula.

BACKGROUND: Use of the forearm basilic vein for the creation of an arteriovenous fistula has been codified as second-choice vascular access for hemodialysis in the last clinical guidelines of the Society for Vascular Surgery in 2008. Poor literature data on this technical option and on its evaluation and outcomes led us to initiate a retrospective single-center study. METHODS: We analyzed the outcomes of every arteriovenous fistula using the forearm basilic vein created in our department. It is a retrospective study in which we collected data prospectively by contacting dialysis centers, neph

Use of Basilic vein in arteriovenous fistulas construction for hemodialysis access. Is it a good option alternative to prosthetic arteriovenous grafts?

PURPOSE: For patients with a failed forearm autogenous fistula (AF) and an exhausted cephalic vein, there is controversy about whether a brachial basilic AF with transposition or an arteriovenous prosthetic bridging graft (BG) must be the second vascular access option. This work measured and compared these two modalities according to patency rates, complications, and revisions. PATIENTS AND METHODS: A retrospective study of 104 cases that had either a brachial basilic AF (72) or an Arteriovenous BG (32). Technical success, operative complications, procedurerelated mortality, maturation time,

Use of Cryopreserved Arterial Allografts as a Reconstruction Strategy in Peripheral Sarcoma Surgery.

BACKGROUND: Complete resection of lower limb soft tissue sarcoma may require vascular resection and reconstruction. While prosthetic grafts and autologous saphenous veins are commonly employed, superior patency has been described for autologous veins. However, their use implies harvesting, mostly on the contralateral limb. Limited data exist on the use of cryopreserved arterial allografts for such vascular replacement. METHODS: We report a retrospective series of 11 patients with locally aggressive extremity sarcomas undergoing 20 vascular resections and reconstructions with cryopreserved art

Comparison of arteriovenous fistula outcomes in patients with and without autosomal dominant polycystic kidney disease: A retrospective study.

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is associated with distinct vascular remodeling; however, its impact on arteriovenous fistula (AVF) outcomes in hemodialysis patients remains unclear. This study aimed to compare AVF patency and survival between patients with and without ADPKD. METHODS: We conducted a retrospective observational cohort study including 38 patients with ADPKD and 38 age-matched non-ADPKD patients who underwent first-time AVF creation between 2013 and 2021. Patients were followed for 24 months. Primary failure, primary patency, and assisted primary

Comparison of ultrasound features and lesion sites in dysfunctional arteriovenous fistula.

This study aimed to investigate ultrasound features of arteriovenous fistula stenosis and their relationship with primary patency after percutaneous transluminal angioplasty (post-intervention primary patency) and compare this classification with that using lesion location. Hemodialysis patients who underwent ultrasound-guided percutaneous transluminal angioplasty for arteriovenous fistula stenosis from July 2020 to December 2021 were retrospectively evaluated. Lesions (excluding inflow arteries) were categorized into five groups based on ultrasound features, and the clinical characteristics a

Longitudinal micro-incision creation prior to balloon angioplasty for treatment of arteriovenous access dysfunction in a real-world patient population: 6-month cohort analysis.

INTRODUCTION: Routine hemodialysis depends on well-functioning vascular access. In the event of vascular access dysfunction, percutaneous transluminal balloon angioplasty (PTA) is conducted to restore patency. Although an angioplasty procedure can provide an excellent immediate result by opening the access to allow dialysis to continue, the long-term patency rates are less than satisfactory. The goal of this study was to assess the outcomes of patients who underwent a novel vessel preparation via longitudinal, controlled-depth micro-incisions prior to PTA. METHODS: This multicenter, prospecti

The Role of High-Frequency Wall Vibrations in Adverse Vascular Remodeling of Arteriovenous Fistula for Hemodialysis.

KEY POINTS: Flow-induced high-frequency vascular vibrations, computed through fluid-structure interaction simulations, are associated with fistula complications. The association between vibrations and arteriovenous fistula complications suggests a potential new mechanism responsible for vascular remodeling. The frequency and amplitude of vibrations are potential biomarkers for arteriovenous fistula surveillance. BACKGROUND: Despite progress in research, the mechanobiological mechanisms behind adverse vascular remodeling and failure in arteriovenous fistula (AVF) for hemodialysis remain unclea

Relationship between laparoscopic and microscopic findings of peritoneum in peritoneal dialysis patients.

Long-term exposure to the peritoneal dialysis solution (PDS) causes functional and morphological alterations that diminish the efficacy of peritoneal dialysis (PD). Macroscopic and microscopic findings, submesothelial compact zone (SMC) thickness and vascular patency, were associated with PD duration. The relationship between microscopic and laparoscopic morphological findings in PD patients was determined. A total of 78 laparoscopic intraperitoneal findings were recorded during PD catheter removal and 45 peritoneal tissues were obtained from the anterior parietal peritoneum. We examined macro

Surgical outcomes of vascular reconstruction in soft tissue sarcomas of the lower extremities.

BACKGROUND: Limb-sparing procedures are currently considered the standard treatment for lower limb soft tissue sarcoma (STS). Surgical excision combined with vascular resection may be necessary to provide an adequate safety margin and to improve the oncologic outcomes. In this scenario, vascular reconstruction is required to preserve limb function. We evaluated the long-term patency and survival outcomes of arterial and venous reconstruction after resecting lower limb STS in the largest single-center case series to date. METHODS: Between November 1995 and July 2014, 25 patients with lower lim

Small-caliber mesothelial cell-layered polytetraflouroethylene vascular grafts in New Zealand white rabbits.

Reduction in the thrombogenicity of small-caliber synthetic vascular grafts by lining them with mesothelial cell has been suggested as a method to reduce thrombosis. The purpose of this research is to determine whether creation of a mesothelial lining on the inner surface of a synthetic vascular graft would improve the patency rate of a small-caliber vascular grafts. Carotid interposition grafting was performed using mesothelial-lined grafts (MLG) in 30 New Zealand rabbits and compared with similar carotid interposition grafts using non-mesothelial-lined grafts (NLG) on the contralateral side.

Regional vs. Local Anesthesia for Arteriovenous Fistula Creation in Adults With End-Stage Kidney Disease: A Grade Assessed Systematic Review and Meta-Analysis.

Arteriovenous fistula (AVF) is the preferred vascular access for hemodialysis in patients with end-stage renal disease (ESRD), yet high rates of primary failure and suboptimal patency remain significant challenges. The choice of anesthetic technique may influence vascular dynamics and AVF outcomes. This study aims to compare the effectiveness of regional anesthesia (RA) versus local anesthesia (LA) on clinical outcomes following AVF creation in adults with ESRD. A systematic review and meta-analysis was conducted in accordance with PRISMA 2020 guidelines. PubMed, Cochrane Library, Scopus, and
