Thoracoscopic division of vascular rings in infants and children.

OBJECTIVE: Traditionally vascular rings in infants and children are treated through an open thoracotomy. Recently, thoracoscopic surgery has been used for these complex procedures. This study reports our early experience with thoracoscopic division of vascular rings and evaluates the efficacy and safety of this approach. MATERIAL AND METHODS: Patients who underwent thoracoscopic division of vascular rings at King Khalid University Hospital, Riyadh, Saudi Arabia, from December 2004 to January 2006 are included. Their data were carefully analyzed looking at demographics, clinical presentation,

Vascular rings: a practical approach to imaging diagnosis.

Vascular ring is a term given to a combination of vascular and often ligamentous structures that encircle the trachea and esophagus. The diagnosis can be difficult because clinical symptoms can be variable and nonspecific, and because vascular arrangements that result in vascular rings in some patients do not form vascular rings in others. The clinical manifestations comprise a spectrum ranging from no symptoms to feeding difficulties, repeated infections, and life-threatening respiratory compromise. The diagnosis of vascular ring can be made by various imaging modalities. Therefore it is impe

[Vascular rings as a cause of the respiratory disturbances in children].

Vascular rings are an infrequent cause of tracheal and/or oesophageal compression in infants and children. The authors present 11 cases of vascular rings in children from 3 months to 15 years of age. There were: double aortic arch in 5 cases, right aortic arch with left arterial ligament in 1 case, aberrant right subclavian artery in 2 subjects and pulmonary artery sling in 3 patients. The diagnosis was established on the basis of chest X-ray examination, echocardiographic examination, bronchoscopy, angiography and magnetic resonance imaging. Surgical correction was performed in all patients w

Iatrogenic, unexpected and other vascular rings in children.

Between May 1955 and July 1987, 33 children with a vascular ring compression syndrome were treated at this institution. There were 4 iatrogenic, 2 unexpectedly found and 27 symptomatic congenital vascular rings. Accurate diagnosis is based on a water soluble iodine contrast dye oesophagogram, bronchoscopy and angiography which are complementary examinations. All symptomatic congenital vascular rings were treated successfully without mortality, but the mortality was 50% if the lesion was iatrogenic in origin or unexpectedly found during palliative or corrective cardiac surgery.

Surgical management in tetralogy of Fallot and vascular ring.

The management of three infants born with a combination of tetralogy of Fallot and a vascular ring causing tracheoesophageal compression is described. There was a double aortic arch in two patients and an aberrant left subclavian artery with left ligamentum arteriosum and right aortic arch in one. Single-stage corrective surgery of both lesions during infancy, performed under profound hypothermia and circulatory arrest, was successful. In our opinion, this is the treatment of choice, when the anatomy of the tetralogy is favorable for primary correction.

Unique Subclavian Vascular Ring Anomaly: Insights from CT Angiography.

Aortic arch anomalies represent a range of congenital vascular malformations resulting from disruptions in the typical embryological development of the aortic arch and its branches. These anomalies, which vary widely in their presentation, can lead to significant clinical symptoms depending on their structure and position. We report the case of a 75-year-old male with intermittent hypertension, palpitations, and episodic warmth in the upper body. Computed tomography (CT) angiography revealed an atypical aortic arch anatomy with a unique right subclavian artery anomaly. The aortic arch displaye

Simultaneous Repair of Right-Sided Coarctation and Vascular Ring.

We present the case of a 4-year old girl with posterior fossa-hemangioma-arterial lesions-cardiac abnormalities/coarctation-eye abnormalities (PHACE) syndrome, an atypical, long-segment, right-sided coarctation of the aorta and vascular ring secondary to an aberrant left subclavian artery and left ligamentum. Simultaneous repair of both lesions was accomplished using a novel technique that included reimplantation of the aberrant left subclavian artery and translocation of the descending aorta to the proximal ascending aorta.

[Surgical repair of vascular ring with aberrant brachiocephalic artery].

The vascular ring causes tracheal and esophageal compression and some cases need the surgical intervention. We report a rare case of vascular ring with aberrant brachiocephalic artery. A nine-month-old boy had suffered from progressing stridor and was admitted to our hospital due to the stridor. The digital subtraction angiography revealed the aberrant brachiocephalic artery which compressed the trachea from anterior aspect. The compression of the trachea was released by tacking forward the aberrant brachiocephalic artery to the sternum with the aid of two stitches. The postoperative course wa

Vascular ring in an extremely low birth weight infant.

Vascular rings due to aortic arch anomalies can be a major cause of tracheoesophageal obstruction in infants. But there is no report of vascular ring in an extremely low birth weight (ELBW) infant. This is a report of an infant with birth weight of 560 g, who has not diagnosed vascular ring until 6 months of age because of asymptomatic process by prolonged tracheal intubation. The patient was treated for double aortic arch with tracheomalacia surgically by simple division of the atretic left arch and the ligamentum arteriosus, aortopexy and tracheostomy before reaching full recovery at 3 years

Vascular ring abnormalities: a retrospective study of 62 cases.

BACKGROUND: The authors analyzed clinical signs of vascular ring anomalies together with appropriate complementary examinations and factors predictive of outcome after surgical treatment. METHODS: The authors reviewed the files of 62 patients with vascular ring abnormalities treated at Necker-Enfants Malades Hospital between January 1990 and January 2000, to analyze age at symptom onset, results of paraclinical examinations, the type of vascular ring abnormality, the surgical indications and type of surgery, and postoperative outcome. Outcomes were divided into 3 categories: cure, partial imp

Transcatheter valve-in-ring and para-ring vascular plug implantation for severe tricuspid regurgitation following annuloplasty ring failure: a case report.

BACKGROUND: Moderate or severe tricuspid regurgitation (TR) recurs in up to one-third of patients within 8 years of surgical annuloplasty repair. Reoperation often carries high risk with poor outcomes. Transcatheter valve-in-ring repair is an emerging alternative treatment. However, residual regurgitation is frequent and may necessitate further procedures. CASE SUMMARY: A 52-year-old female was diagnosed with severe rheumatic valvular heart disease. The patient underwent mechanical aortic and mitral valve replacement. Additionally, tricuspid repair was performed using a semi-rigid annuloplast

Surgical repair of a vascular ring: A frequently misdiagnosed disease.

BACKGROUND: Vascular rings are rare congenital malformations that lead to various degrees of respiratory difficulties or feeding problems by forming a complete or partial ring that compresses the trachea, the esophagus, or both. CASE PRESENTATION: We describe the case of an 18-month-old boy with recurrent respiratory symptoms and dysphagia. Chest radiograph revealed slightly hyperinflated lung fields bilaterally. Barium esophagography suggested vascular ring anomaly. CT angiogram revealed a vascular ring, consisting of a double aortic arch, compressing the trachea and esophagus. The echocardi

Narrowing Down the Symptomatology of Isolated Vascular Rings in Children.

Vascular rings may cause respiratory or gastrointestinal symptoms due to compression of the trachea and/or esophagus. Advances in imaging have enabled early detection in asymptomatic patients posing new management dilemmas. Surgery is expected to relieve symptoms, although this has not been well studied. We sought to evaluate the presence and pattern of symptoms associated with vascular rings before surgical intervention and to detail symptom resolution after surgery. A 10-year retrospective review of patients diagnosed with an isolated vascular ring was performed between January 2010 and Dece
