Four patients presented with ruptured intracranial aneurysms during pregnancy. Problems encountered during the management of these cases included delayed diagnosis, obstructive hydrocephalus, cerebral ischemia due to vasospasm and recurrent subarachnoid hemorrhage. Recent advances in the management of ruptured intracranial aneurysms, including early computerized tomographic scanning, intravascular volume expansion and induced hypertension for the management of cerebral vasospasm, and the timely obliteration of the aneurysm are applicable to the subarachnoid hemorrhage patient even if her condi

The treatment strategy for iatrogenic intracranial vasospasm has not been established. We reported a rare case of stent-retriever use for the treatment of iatrogenic vasospasm after mechanical thrombectomy. The patient presented acute ischemic stroke due to the occlusion of the left middle cerebral artery (MCA). The patient underwent a successfully mechanical thrombectomy, however, severe stenosis was visualized in MCA, which was considered to be an iatrogenic mechanical vasospasm. This vasospasm was successfully treated with the Solitaire stent device, and the patient recovered from ischemic

The effects of subarachnoid hemorrhage (SAH) on intracranial prostaglandins (PGs) were studied in canines. Subarachnoid hemorrhage was produced by the "two hemorrhage" method. Basilar artery caliber and regional cerebral blood flow (rCBF) in the occipital cortex were reduced by 42% and 43% during delayed vasospasm, respectively. Once delayed vasospasm had developed, intravenous infusion of OKY-046, a selective inhibitor of thromboxane (TX) A2 synthetase, induced no significant change in angiographic vasospasm but caused a significant increase in rCBF. In delayed vasospasm, cortical levels of P

The effect of an intra-arterial administration of diltiazem on experimental cerebral vasospasm was previously reported by us. In the present study, the effect of diltiazem on cerebral vasospasm, intracranial pressure and systemic blood pressure were investigated using dogs. Cerebral vasospasm was induced by an injection of 5 ml of autologous blood into the cisterna magna. Diltiazem (10(-6) M) was administrated into the cisterna magna and the dilating effect of the drug on the basilar artery was monitored angiographically. The intrathecal administration of diltiazem relieved the vasospasm for m

Streptococcus pneumoniae are Gram-positive bacteria that are responsible for many types of illnesses including pneumonia, sinus infections, and community-acquired meningitis. One important complication of bacterial meningitis is intracranial vasculopathy. Possible etiologies include vasculitis, vasospasm, endocarditis, or intra-arterial thrombosis. We present a case report of S. pneumoniae meningitis treated with antibiotics in which clinical improvement correlated with serial transcranial Doppler ultrasonography (TCD) improvement, suggesting vasospasm or vasculitis as a possible mechanism for

We describe two patients with subarachnoid haemorrhage due to a ruptured intracranial aneurysm and severe symptomatic vasospasm. The aneurysm was occluded with detachable coils followed by intra-arterial infusion of papaverine to treat vasospasm as an one-stage procedure. There was significant resolution of the vasospasm. The long-term clinical outcome in one patient was excellent, the other still has minor deficits. Combined endovascular aneurysm therapy followed by intra-arterial spasmolysis with papaverine is a technically feasible therapeutic alternative in patients with symptomatic vasosp

BACKGROUND: Dantrolene has a safe side-effect profile and a mechanism of action that makes it attractive as an option for treatment of cerebral vasospasm. The authors report 2 cases of refractory cerebral vasospasm secondary to aneurysmal subarachnoid hemorrhage that were successfully treated with intra-arterial (IA) dantrolene.
CASE DESCRIPTION: Two patients, a 63-year-old woman and 36-year-old woman, developed severe vasospasm refractory to IA vasodilators after rupture of anterior communicating artery aneurysms. IA dantrolene was injected in doses of 15-30 mg in the affected distributions

Cerebral vasospasm (specifically, intracranial arterial spasm) is variously defined as: (1) an arteriographically evident narrowing of the lumen of one or more of the major intracranial arteries at the base of the brain due to contraction of the smooth muscle within the arterial wall, or due to the morphological changes in the arterial wall and along its endothelial surface that occur in response to vessel injury; (2) the delayed onset of a neurological deficit following subarachnoid hemorrhage, thought to be due to ischemia or infarction of a portion of the brain; or (3) the combination of th

BACKGROUND: Transcranial color-coded duplex Doppler (TCCD) is commonly used to detect and monitor vasospasm in subarachnoid aneurysmal hemorrhage (aSAH). However, contrast enhanced TCCD (CE-TCCD) may be more effective. The objective of this study was to compare the accuracy of TCCD and CE-TCCD in the detection of vasospasm.
METHODS: This study was a prospective comparison of TCCD and CE-TCCD for the detection of vasospasm, using computed tomography angiography (CT Angio) as a reference examination. The setting was the Department of Anesthesiology and Intensive Care at the Bicêtre University H

Mechanical properties of human intracranial arteries were studied with regard to their aging changes as well as cerebral vasospasm. There exists a considerable difference in the development of sclerosis between the intracranial and the extracranial arteries. Intracranial arteries are already stiff at birth and become stiffer with age. Their increase in stiffness is attributable to the marked thickening of their walls, which compensates for the decrease of their elastic modulus with age. The pressure-diameter curve of the intracranial vertebral arteries subjected to subarachnoid hemorrhage is b

Digital subtraction angiography (DSA) has been used as the standard method for detecting cerebral vasospasm after subarachnoid hemorrhage (SAH). Multislice computed tomographic angiography (CTA) is a relatively recent method used for evaluating the vasculature of the intracranial arteries. The purpose of this study was to compare multislice CTA and DSA for the detection and quantification of cerebral vasospasm after SAH, and to analyze the usefulness of multislice CTA. Eight patients with SAH underwent initial CTA with DSA within 72 hours after the onset of symptoms and follow-up multislice CT

Increase in transcranial Doppler ultrasound flow velocities in the major basal arteries correlates with symptomatic vasospasm. Transcranial Doppler examinations are performed using a pulsed Doppler Probe via the trans temporal approach. Transcranial colour-coded real time sonography can be useful and help to identify the cerebral arteries. Maximum flow velocities of > 200 cm/sec are associated with cerebral ischemia and infarction. A maximum rise in Doppler velocity of more than 50 cm/sec/24 h is correlated with poor outcome. Using a diagnostic cutoff of 130 cm/sec a 87% positive predictive va

In a series of 404 patients with intracranial saccular aneurysm, 61 were over 65 years of age. Ten of these aged patients had no surgery. The operative mortality in the aged patients was 11.8%. While the mortality rate in patients below 65 years was 6.1%. The main complication following intracranial surgery for aneurysm consisted of cerebral infarction due to vasospasm, hydrocephalus, intracranial hematoma and general complications. The incidence of angiographical vasospasm was 22.6% in the aged patients and 43.6% in the young patients. There is no significant difference in vasospasm following

The purpose of this study was to compare transcranial Doppler sonography (TCD) and 99Tcm-HMPAO cerebral scintigraphy in detecting the effects of acute intracranial hypertension by pressure transmission using a previously validated model. In 20 New Zealand rabbits, cerebral blood flow velocities of basilar artery and carotid siphon were simultaneously monitored in baseline conditions and during acute intracranial hypertension by pressure transmission. This hypertension was induced by progressive steps of 5 mmHg, for a 5 min duration by elevating a saline infusion bottle connected to the subdura

Reversible cerebral vasoconstriction syndrome (RCVS) is a rare but serious cerebrovascular disorder characterised by transient narrowing of the cerebral arteries, triggered by internal or external stressors. We present a case of RCVS in a 28-year-old female patient 10 days postpartum, after a haemorrhagic shock. The initial acute management of the abdominal haemorrhagic shock syndrome included massive transfusion of blood products along with vasopressors and crystalloids. Although her condition stabilised initially, she later exhibited severe neurological deficits of unknown cause, prompting f

BACKGROUND: Vasospasm is a common complication of aneurismal subarachnoid hemorrhage (SAH) that may lead to cerebral ischemia and death. The standard method for detection of vasospasm is conventional cerebral angiography, which is invasive and does not allow continuous monitoring of arterial radius. Monitoring of vasospasm is typically performed by measuring Cerebral Blood Flow Velocity (CBFV) in the major cerebral arteries and calculating the Lindegaard ratio. We describe an alternative approach to estimate intracranial arterial radius, which is based on modeling and state-estimation techniqu

A case of vasospasm after traumatic subarachnoid haemorrhage (SAH) is reported here. Transcranial Doppler Sonography (TCD) was used to evaluate mean flow velocity (MFV) changes of the basal cerebral arteries related to vasospasm. Acceleration of MFV of the right middle cerebral artery (MCA) indicating vasospasm was first noted on TCD evaluation, and then proved by carotid angiography (CAG). Evaluation of all TCD results revealed that the process of relaxation or normalization of the spastic artery started from the proximal side of the basal intracranial artery and gradually moving to the dista
