Left subclavian vein puncture for insertion of Swan-Ganz catheters.

A rapid, safe, and reliable method for insertion of Swan-Ganz flow-directed, balloon-tipped catheters using the left subclavian vein is described. Experience with this method in 24 consecutive patients indicates a distinct advantage over the antecubital fossa approach: A venous cutdown is not necessary, the pulmonary artery is readily entered, fluoroscopy is not required, the patient is free to move both arms, and thrombophlebitis and infection occur infrequently. With proper attention to landmarks such as the clavicular tubercle, the procedure may be performed with facility. The major complic

Pulmonary embolism and myocardial hypoxia during extracorporeal membrane oxygenation.

The treatment of a newborn with severe meconium aspiration by venoarterial extracorporeal membrane oxygenation (ECMO) was complicated by myocardial hypoxia with a marked decrease of myocardial contractility. The onset of the cardiac hypoxia was related to a pulmonary artery embolus. The origin of the embolus was a deep femoral vein thrombosis, caused by a central vein catheter, which was inserted 1 day before ECMO by venous cutdown. The possible pathophysiology of myocardial hypoxia in this patient is discussed, especially with regard to myocardial perfusion, supporting the hypothesis of coron

Phrenic nerve damage in the tiny infant during vein cannulation for parenteral nutrition.

An infant weighing 740 gm at birth underwent right internal jugular venous cannulation for the administration of parenteral nutrition at age 17 days. The roentgenogram obtained to assess catheter placement revealed a high right diaphragm for the first time in the course of the infant. The phrenic palsy was incurred during the cutdown in the neck of the tiny, then 650 gm, infant. Two similar patients are mentioned in the literature. The phrenic palsy appeared to delay improvement in the respiratory status of this infant. This patient illustrates the fact that phrenic nerve palsy is a potential

Indications and outcomes for tunneled central venous line placement via the axillary vein in children.

PURPOSE: To assess the indications, safety and outcomes of tunneled central venous catheters (CVCs) placed via a cutdown approach into the axillary vein in children, an approach not well described in this population. METHODS: A retrospective cohort study was performed on pediatric patients who received CVCs via open cannulation of the axillary vein or one of its tributaries between January 2006 and October 2016 at two hospitals. RESULTS: A total of 24 axillary CVCs were placed in 20 patients [10 male (42%); mean weight 7.0 kg (SD 2.9); mean age 10 months (SD 6)]. The most common indications

Emergency vascular access in a patient with limb agenesis.

A 19-year-old white man with congenital agenesis of all four limbs arrived in the emergency room in full cardiopulmonary arrest secondary to Reye's syndrome. The patient was intubated but had no vascular access. After a percutaneous femoral venous line proved unsuccessful, an internal jugular line was placed for venous access. Repeated attempts at percutaneous femoral and axillary arterial line placements were unsuccessful. A femoral cutdown showed 2-mm femoral vessels. The autopsy showed increasingly hypoplastic aortic, iliac, and femoral vessels below the takeoff of the renal vessels. The su

[Parenteral nutrition. Technique of long term central venous catheterization (author's transl)].

The authors compare two techniques of deep venous catheterization for long term total parenteral nutrition. From December 1, 1973 to April 30, 1974, 31 polyethylene catheters were placed by direct transcutaneous puncture of the subclavian or internal jugular vein in 18 patients. From Mayl, 1974 to March 30, 1975, 35 silicone rubber catheters were surgically placed by cutdown of the basilic vein, in 32 patients. The surgical technique has important advantages such as a safe technique, diminished local and generalized complications and the possibility of intermittent perfusion. Therefore the aut

Axillary vein puncture versus cephalic vein cutdown for cardiac implantable electronic device implantation: A meta-analysis.

INTRODUCTION: Cephalic vein cutdown (CVC) and axillary vein puncture (AVP) are both recommended for transvenous implantation of leads for cardiac implantable electronic devices (CIEDs). Nonetheless, it is still debated which of the two techniques has a better safety and efficacy profile. METHODS: We systematically searched Medline, Embase, and Cochrane electronic databases up to September 5, 2022, for studies that evaluated the efficacy and safety of AVP and CVC reporting at least one clinical outcome of interest. The primary endpoints were acute procedural success and overall complications.

Multiple purpose central venous access in infants less than 1,000 grams.

The use of central venous catheters in low birthweight infants has been associated with a high rate of infectious and mechanical-related complications. We reviewed our experience with multipurpose central venous catheters in infants less than 1,000 g to determine the rate of catheter-related sepsis and mechanical catheter malfunction. From October 1981 to August 1984, 20 infants (average weight 778 g) underwent placement of 22 central venous Broviac catheters. In addition to parenteral nutrition, antibiotics, aminophylline, and replacement fluids were infused. Total catheter days were 961, wit

Axillary vein access using ultrasound guidance, Venography or Cephalic Cutdown-What is the optimal access technique for insertion of pacing leads?

We reviewed the different approaches used for central vein access during insertion of cardiac implantable electronic devices. The benefits and hazards of each approach (cephalic vein cutdown, axillary vein cannulation using venography and ultrasound) are discussed. Each approach has its advantages and hazards that need to be considered for the individual patient and balanced against the skills of the operator. The benefits of ultrasound guided venous access in reducing radiation exposure to the patient and implanter, avoiding the need for angiographic contrast and in minimizing the risk of pne

Technical report: Hickman catheter rescue.

Nineteen malfunctioning or incorrectly positioned Hickman catheters from a series of 320 catheter placements were referred to the radiology department for salvage. Successful catheter 'rescue' was achieved on 14 occasions (73.6%). Eight catheters were repositioned, five were replaced and thrombolysis was successful in a patient with subclavian vein thrombosis. Catheter repositioning techniques are reviewed and a new technique for catheter replacement using the existing venous access and subcutaneous tunnel is described.

Chronic venous access in patients with cancer. Selective use of the saphenous vein.

BACKGROUND: Dependable chronic venous access is an important aspect of supportive care for patients requiring chemotherapy or other long-term therapy because it enables such patients to be treated on an outpatient basis. METHODS: The authors studied 56 patients with cancer requiring chronic venous access who had an infusion catheter placed into the inferior vena cava (IVC) through open saphenous venotomy, most commonly because superior vena cava (SVC) occlusion, severe chest wall infection, or chest wall malignancy precluded standard subclavian or jugular cannulation. RESULTS: The 56 cathete

Vena cava filters: available devices and current practices.

Deep venous thrombosis and pulmonary embolus are significant clinical problems. Although anticoagulation remains first-line therapy for thromboembolic disease, the placement of inferior vena cava filters plays an important role as alternative or supplemental therapy. Initial filters were high-profile devices intended to be placed via surgical cutdown. Concerns about the size of the delivery systems prompted development of low-profile systems intended specifically for percutaneous placement. The placement of filters has progressively become a percutaneous procedure performed by interventional r

Clinical implications of cephalic vein morphometry in routine cardiac implantable electronic device insertion.

BACKGROUND: Morphometric parameters of the venous vasculature constitute an important aspect in successful cardiac implantable electronic device (CIED) insertion. The purpose of this study was to present morpho-anatomical variations of the cephalic vein (CV) and their effect on the course of CIED implantation procedures, based on the patients from our centre. MATERIALS AND METHODS: We analysed contrast venography results obtained during first-time lead placement. Venography was indicated in the cases of problematic lead introduction with either the CV cutdown or axillary/subclavian vein punct

An alternative technique for totally implantable central venous access devices. A retrospective study of 1311 cases.

AIM: The aim of the present study was to report our experience of totally implantable central venous access devices (TICVAD) implantation using two techniques and attempt to define the better technique. MATERIALS AND METHODS: From January 1998 to September 2003, 1131 patients were reviewed and divided into two groups with implantation by cephalic vein cut-down (group A) done by general surgeons and subclavian vein puncture with the Seldinger technique (group B) done by vascular surgeons. The operative time, early and late complications of these two groups were compared. Data were analysed by

TIVAD implantation for chemotherapy: retrospective comparison between US-guided vein puncture vs. surgical vein cutdown on more than 3000 procedures.

BACKGROUND: Totally implantable venous access devices (TIVADs) are widely used for long-term therapies, including chemotherapy. Given the potential for serious complications, it is crucial to determine the safest implantation technique with the lowest risk profile. This bicentric, retrospective study aims to compare the surgical vein cut-down approach with ultrasound-guided percutaneous vein puncture in terms of procedural success, complication rates, and operative efficiency. METHODS: A retrospective analysis was conducted on 3373 patients who underwent TIVAD implantation at two centers. Pat

[Use of totally implantable venous system in cancer patients. Surgical technique and initial clinical experience].

The experience with a totally implanted venous system (TIVS) in 27 cancer patients (15 males and 12 women) is reviewed, stressing simplicity of the cut down technique, which was successfully performed in 92.7% of patients. The common: est complications are described; port infection (7.4%), subclavian vein thrombosis (3.7%), and cutaneous necrosis (3.7%) at the port level. It is concluded that TIVS is a useful device for prolonged drugs administration and the cut down technique is indicated especially in the outpatients setting.

Closed cannulation of subclavian vein vs open cut-down of cephalic vein for totally implantable venous access port (TIVAP) implantation: protocol for a systematic review and proportional meta-analysis of perioperative and postoperative complications.

BACKGROUND: Totally implantable venous access port (TIVAP) implantation is one of the most often performed operations in general surgery (over 100,000/year in Germany). The two main approaches for TIVAP placement are insertion into the cephalic vein through an open cut-down technique (OCD) or closed cannulation technique of the subclavian vein (CC) with Seldinger technique. Both procedures are performed with high success rates and very low complication frequencies. Because of the low incidence of complications, no single interventional trial is able to report a valid comparison of peri- and po

An unusual case of the cephalic vein with a supraclavicular course.

While the subclavian or axillary vein can be safely and successfully punctured in the majority of cases, some device implanters still prefer cut down to the cephalic vein as the initial approach to venous access for transvenous placement of pacemaker or defibrillator leads out of concern for the risk of pneumothorax, subclavian crush, and other possible complications. However, very occasionally, the cephalic vein crosses superficial to the clavicle to join the external jugular vein, making it rather unappealing for this purpose. Relying on a guide wire introduced through the cephalic vein to g

Percutaneous sheath brachial vein cardiac catheterization in children.

During the past ten years we have performed certain phases of cardiac catheterization on 48 children using a modified percutaneous sheath technique to enter a superficial arm vein. Our patients ranged in age from 15 months to 21 years, with a mean age of 10.0 years. The indications for a superior approach most commonly have included electrophysiologic investigations, femoral vein occlusions, and proximal obstructions of the inferior vena cava. The advantages of a percutaneous sheath technique in the superficial brachial veins include the ease of changing catheters frequently during a case and
