Congenital pulmonary artery stenoses masquerading as chronic thromboembolic disease.

Stenotic pulmonary vascular lesions, often amenable to balloon angioplasty, can erroneously appear to suggest chronic thromboembolic disease on ventilation/perfusion (V/Q) scan. We report a case of multiple peripheral pulmonary artery stenoses and describe radiologic findings and treatment options.

Lung disease in reformed drug addicts: diagnostic and physiologic correlations.

Pulmonary morphologic alterations secondary alterations secondary to intravenous drug abuse have been previously identified by histopathologic study of autopsy and biopsy material. Recent investigative efforts have been directed toward defining functional abnormalities that would allow for early clinical diagnosis. For this reason, 12 asymptomatic addicts were studied with roentgenography, spirometry, single breath diffusing capacity (DLco), 99Tc perfusion and 133 Xe ventilation scanning. Nine of the twelve were heavy cigarette smokers. None reported historical evidence of chronic bronchitis o

Pulmonary arteriovenous malformation masquerading as massive pulmonary thromboembolus.

We present a case of a 78-year-old male with hereditary haemorrhagic telangiectasia and severe hypoxia incorrectly diagnosed as pulmonary embolism following a false positive ventilation/perfusion scan. Anti-coagulation and thrombolysis was complicated by an upper gastro-intestinal haemorrhage. Pulmonary arteriovenous malformations resulted in a clinically significant right-left shunt and created an apparent perfusion defect evident upon radionuclide imaging, leading to diagnostic uncertainty and a potentially dangerous treatment modality.

Pulmonary artery malformation syndrome.

We present a case to illustrate that pulmonary artery malformation is a potential diagnosis in patients with nonresolving perfusion defects. The diagnosis can usually be made by the history, physical examination, chest roentgenogram, and ventilation-perfusion scanning. If the patient's clinical symptoms are inconsistent with the scan, then pulmonary angiography is warranted. Pleuritic chest pain, hypoxemia, and a perfusion defect are nonspecific and should not be interpreted as indicative of pulmonary embolism but only that it has not been ruled out. Anticoagulation is risky because these pati

Factors affecting the probability of pulmonary embolism in lung ventilation/perfusion scintigraphy.

BACKGROUND: Probabilities for Pulmonary Embolism (PE) are reported on V/Q scintigraphy, though the clinical implications of these probabilities remain unclear. This study examined the factors influencing PE probability groups determined by V/Q scintigraphy. METHODS: Demographic and radiologic data, V/Q scintigraphy results, echocardiographic findings, and pulse oxygen saturation (SpO2) were recorded for 95 patients admitted to the Nuclear Medicine Clinic between January 2022-2023. Patients were categorized into normal, low, intermediate, and high probability groups for PE based on V/Q scintig

Focal hepatic activity during ventilation-perfusion scintigraphy due to systemic-portal shunt due to superior vena cava obstruction from histoplasmosis-induced fibrosing mediastinitis.

A 59-year-old woman with a history of fibrosing mediastinitis secondary to histoplasmosis diagnosed on mediastinoscopy presented with dyspnea. A ventilation-perfusion scan demonstrated decreased perfusion to the entire right lung. In addition, the perfusion images demonstrated focal abnormal activity in part of the liver. On computed tomography of the chest, there was significant soft tissue opacification in the mediastinum occluding the right pulmonary artery, with passage of the injected contrast via collateral vessels to the liver. The main collateral pathway was via the right internal thor

[Obliterating bronchiolitis following measles].

We report the complex clinical course over 15 years in a child who at the age of 19 months developed patchy pneumonic infiltrates and bronchiolitis after measles. Chronic atelectasis led to resection of the right upper lung lobe. He developed bilateral bronchiectases, predominantly in the right lung. Ventilation/perfusion scans showed a major reduction of both ventilation and perfusion mainly on the right side. Early recognition of obliterative bronchiolitis is important in order to provide adequate supportive treatment of possible complications.

The use of leg venous ultrasonography for the diagnosis of pulmonary embolism.

The role of ultrasonography of the lower extremities for the evaluation of patients with suspected pulmonary embolism has become more clearly defined with time. Ultrasonography is a useful first-line test for pulmonary embolism in clinical circumstances in which radiographic imaging is contraindicated or not readily available (eg, pregnancy). Ultrasonography is likely not required for the evaluation of patients with suspected pulmonary embolism who have a negative computed tomography pulmonary angiography, particularly if there are no symptoms of deep vein thrombosis. Ultrasonography is helpfu

Left Upper Lobe Lung Cancer With Swyer-James-MacLeod Syndrome.

Swyer-James-MacLeod syndrome (SJMS) is characterized by unilateral hyperlucency of the lung on chest roentgenogram, unilateral reduction in vascularity on chest computed tomography, and unilateral loss of perfusion on pulmonary ventilation/perfusion scan. The prevalence of SJMS is reported to be 0.01%, with most cases occurring in children and only a few cases in adults. Therefore, SJMS associated with primary lung cancer is extremely rare. We present a case of left upper lobe lung cancer with SJMS by performing robot-assisted thoracoscopic left pneumonectomy.

Pathophysiology of hypoxemia in mechanically-ventilated patients with COVID-19: A computed tomography study.

The pathogenesis of hypoxemia during acute respiratory distress syndrome caused by SARS-CoV-2 infection (C-ARDS) is debated. Some observations led to hypothesize ventilation to perfusion mismatch, rather than anatomical shunt, as the main determinant of hypoxemia. In this observational study 24 C-ARDS patients were studied 1 (0-1) days after intubation. Patients underwent a CT scan analysis to estimate anatomical shunt and a clinical test to measure venous admixture at two fractions of inspired oxygen (FiO2), to eliminate oxygen-responsive mechanisms of hypoxemia (ventilation to perfusion mism

Lymphangitic carcinomatosis: lung scan abnormalities.

Lymphangitic carcinomatosis of the lung is a late and fatal manifestation of cancer. The diagnosis is seldom made ante mortem. Four patients with breast cancer had autopsy-proven lymphangitic metastases of the lung. Of these patients, two had normal chest x-rays and three had normal ventilation lung scans. The perfusion lung scans of all four showed irregular peripheral perfusion defects. Autopsies showed permeation of the lymphatics by tumor and tumor emboli in small arteries and arterioles.

Superior vena cava and right atrial thrombus detected on lung perfusion scintigraphy.

Ventilation and perfusion scanning is still used as the first modality for evaluating pulmonary embolism in pregnant and renal failure patients and those who are allergic to radiographic contrast. Hot spots in the right atrial area on perfusion scan are the result of the presence of thrombi. These thrombi are of 2 varieties. One type is a free-floating thrombus, which needs emergency thrombectomy, and another type is thrombus formation in the atria, predisposed by the presence of catheters. We report a study showing essentially normal perfusion but intense tracer uptake in the superior vena ca

Interest of ventilation/perfusion SPECT/CT in patients with post-COVID condition: a multicenter observational study.

PURPOSE: The COVID-19 pandemic has led to a significant number of survivors experiencing persistent health issues, now recognized as long COVID. Respiratory symptoms, such as dyspnea, chronic cough, and reduced exercise capacity, are prevalent and can severely impact quality of life. Ventilation/Perfusion (V/Q) SPECT/CT is a hybrid imaging modality that offers both functional and anatomical insights and has proven valuable in evaluating pulmonary embolism (PE). However, its role in long COVID remains underexplored. METHODS: A retrospective, multicenter study was conducted using a national regi

Quantifying local lung perfusion and ventilation using correlated SPECT and CT data.

UNLABELLED: A clinically applicable method for quantifying lung perfusion and ventilation on a subregional (local) scale from SPECT scans in order to estimate local lung function in patients with pre-existing pulmonary disease and to monitor local treatment effects was developed and evaluated. METHODS: SPECT 99mTc perfusion and 81mKr ventilation images were corrected for photon attenuation and scatter effect with a postreconstruction correction method incorporating a variable-effective linear-attenuation coefficient calculated from spatially-correlated CT data. A new algorithm was developed t

Pulmonary thromboemboli after neonatal asphyxia.

Two surviving neonates with pulmonary thromboembolism, diagnosed by ventilation-perfusion lung scan, are described. Both infants had respiratory distress, and one had features consistent with persistent pulmonary hypertension of the neonate. These reports demonstrate that substantive pulmonary emboli can occur in neonates and may not be recognized without an appropriate level of clinical suspicion.

Perfusion scan in pulmonary tumor microembolism: report of a case.

A 76-year-old male patient was admitted to our hospital with the chief complaint of dyspnea. A chest CT scan showed pericardial effusion, mediastinal lymphadenopathy and a tumor in the right ventricle with invasion to the main trunk of the pulmonary artery. A 99mTc MAA perfusion lung scan revealed multiple small subsegmental peripheral defects and a "fissure sign", while the 133Xe ventilation scan was normal. After the chest CT scan and scans of both lungs, tumor microembolism was highly suspected. Open chest surgery was performed. A huge tumor in the right ventricle involving the main trunk o

Pulmonary arterial hypertension: value of perfusion scintigraphy.

Records of 34 patients with established causes of pulmonary arterial hypertension were retrospectively reviewed. The ventilation-perfusion scans were blindly classified according to standardized criteria as normal or high, low, or intermediate probability of pulmonary embolism as the cause of pulmonary arterial hypertension. Twelve of 13 patients with primary pulmonary hypertension had normal or low-probability scans, but the perfusion pattern was not helpful in distinguishing between histologic subtypes. All eight patients with large-vessel thromboembolic hypertension had high-probability sca

A diagnostic challenge: unveiling chronic thromboembolic pulmonary hypertension in the absence of visible thrombi: a case report.

BACKGROUND: Chronic thromboembolic pulmonary hypertension (CTEPH) is a severe condition characterized by persistent dyspnoea and hypoxia. Diagnosis is challenging, especially when computed tomography pulmonary angiography (CTPA) shows no visible thrombi and symptoms overlap with common conditions like heart failure. CASE SUMMARY: We present a 77-year-old female with heart failure with preserved ejection fraction who experienced progressive, refractory dyspnoea and hypoxia. Despite a CTPA showing no large, organized thrombi, a strong clinical suspicion prompted a ventilation/perfusion (V/Q) sc

Diagnosis of pulmonary embolus by spiral CT: a case study.

A thirty-eight-year old immunosuppressed woman presented with respiratory distress and was diagnosed with Pneumocystis Carinii Pneumonia. Pulmonary embolism was also suspected on clinical grounds. A Ventilation-Perfusion Scan was indeterminate. Contrast-enhanced spiral computed tomography of the chest confirmed the presence of a central pulmonary embolus and helped to avoid pulmonary angiography in this critically ill patient.
