Stenting allows weaning and extubation in ventilator- or tracheostomy dependency secondary to benign airway disease.

Central airway obstruction can cause severe respiratory insufficiency leading to mechanical ventilation (MV) or artificial airway (AA) dependency. Interventional bronchoscopic procedures have been reported to be of help in weaning patients with malignant airway stenoses from mechanical ventilation, whereas their use in benign disease is only anecdotal. The objectives of this study are to evaluate early, intermediate and long-term outcome of interventional bronchoscopy and stent placement in the treatment of MV/AA dependency due to benign airway obstruction. In a retrospective cohort analysis f

What interventions facilitate weaning from the ventilator? A review of the evidence from systematic reviews.

INTRODUCTION: Mechanical ventilation is life saving for many very preterm babies but prolonged use can have adverse effects increasing the risk of subglottic injury and chronic lung disease (CLD). Shorter ventilation should reduce these risks and a number of interventions have been tested to facilitate earlier extubation. METHODS: The Cochrane Library was searched for systematic reviews of randomised controlled trials of interventions to facilitate extubation and reduce post-extubation atelectasis. These interventions included nasal continuous positive airway pressure (CPAP), nasal intermitte

Does obesity impact on weaning from invasive ventilation: a secondary analysis of the WEAN SAFE study.

OBJECTIVE: To understand the impact of obesity on outcomes of weaning from invasive mechanical ventilation (MV). METHODS: The study population consisted of patients enrolled in the WEAN SAFE study. We defined 4 groups based on body mass index (BMI), namely: Normal weight (BMI 18.5-24.9 kg/m²), Overweight (BMI 25-29.9 kg/m²), Obesity Class I (BMI 30-34.9 kg/m²), and obesity classes II and III (BMI ≥ 35 kg/m²). The primary outcome was the rate of successful extubation in patients in each BMI group. Secondary outcomes included the ICU and hospital survival, and PEEP levels at time of weaning eli

Effect of Tracheostomy on Weaning Parameters in Difficult-to-Wean Mechanically Ventilated Patients: A Prospective Observational Study.

BACKGROUND AND OBJECTIVE: Weaning parameters are commonly measured through an endotracheal tube in mechanically ventilated patients recovering from acute respiratory failure, however this practice has rarely been evaluated in tracheostomized patients. This study aimed to investigate changes in weaning parameters measured before and after tracheostomy, and to explore whether the data measured after tracheostomy were associated with weaning outcomes in difficult-to-wean patients. METHODS: In a two-year study period, we enrolled orotracheally intubated patients who were prepared for tracheostomy

Hallmarks of Weaning From Mechanical Ventilation: A 3S Framework for Ventilator Liberation-A Narrative Review.

Safe and long-lasting separation from invasive mechanical ventilation is a critical aspect of care provision in the ICU, yet it remains a complex and heterogeneously applied process. Current weaning strategies often rely on fragmented assessments, personal judgment, and poorly integrated decision milestones that individually and collectively may contribute to delayed attempts, weaning, and extubation failure. In this How I Do It article, we propose a comprehensive framework based on 7 hallmarks, organized into 3 sequential and interrelated phases (3S framework): screening, separating, and secu

Synchronized intermittent mandatory ventilation with and without pressure support ventilation in weaning patients with COPD from mechanical ventilation.

This prospective study compared two weaning modalities in COPD patients requiring mechanical ventilation (MV) for acute respiratory failure. Nineteen patients with COPD were studied when their precipitating illness was controlled. Although they satisfied the conventional bedside weaning criteria, they could not tolerate any reduction in the respirator rate below 10 cycles/min. At this time, patients were randomized into two groups receiving either synchronized intermittent mandatory ventilation (SIMV) with pressure support ventilation (PSV) (group 1) or SIMV alone (group 2). The volumetric sup

Modified Venous Excess Ultrasound (mVExUS) in the Prediction of Ventilator Weaning Failure: a Cohort Study.

BACKGROUND: Ventilator weaning failure is a significant challenge in critically ill patients, and venous congestion has emerged as a potentially modifiable factor influencing weaning outcomes. This study aimed to investigate the association between venous congestion identified by the modified venous excess ultrasound (mVExUS) score and ventilator weaning failure in critically ill patients. METHODS: We prospectively enrolled patients aged 18 years or older who had received mechanical ventilation (MV) for at least 48 hours. A mVExUS score, excluding the intrarenal component, and a lung point of

Bronchial squamous cell papilloma incidental finding in bronchoscopy following prolonged ventilator weaning: A case report.

Bronchial squamous cell papilloma (BSCP) is an uncommon tumor seen in the bronchi, while squamous cell papillomas are frequently observed in different organs. The fortuitous detection of BSCP underscores the significance of comprehensive diagnostic assessments for respiratory conditions. This case details the situation of a 37-year-old, non-smoking female who was referred to our center due to seizures. After a three-week hospital stay, attempts were made to wean her off ventilator support. However, the patient experienced difficulties during spontaneous breathing trials. A bronchoscopy was per

Transcutaneous Monitoring of Carbon Dioxide to Optimize Ventilator Weaning in At-Risk Adults After Cardiopulmonary Bypass.

BACKGROUND: Cardiopulmonary bypass use during surgery disrupts microcirculation, which can contribute to lung injury, particularly in patients with pulmonary comorbidities. Continuous transcutaneous carbon dioxide (CO2) monitoring assists clinicians to trend values related to metabolic and respiratory status between intermittent arterial blood gas measurements. We sought to review the literature to support adoption of this technology to optimize weaning in patients with pulmonary comorbidities following open heart surgery. METHODS: Multiple databases were reviewed. Inclusion criteria were lim

Can donepezil facilitate weaning from mechanical ventilation in difficult to wean patients? An interventional pilot study.

BACKGROUND: Management of difficult to wean patients is a dilemma for health care system. Recently published studies demonstrated efficacy of donepezil to counteract respiratory depression in sleep apnea. However, to the best of our knowledge, pharmaceutical interventions with donepezil to facilitate weaning have not been tested so far. Therefore in the present study, we evaluated the efficacy of using donepezil on weaning course in difficult to wean patients. METHODS: In this non-randomized interventional clinical study, difficult to wean patients with prior inappropriately depressed respira

COVID-19 ARDS: Points to Be Considered in Mechanical Ventilation and Weaning.

The COVID-19 disease can cause hypoxemic respiratory failure due to ARDS, requiring invasive mechanical ventilation. Although early studies reported that COVID-19-associated ARDS has distinctive features from ARDS of other causes, recent observational studies have demonstrated that ARDS related to COVID-19 shares common clinical characteristics and respiratory system mechanics with ARDS of other origins. Therefore, mechanical ventilation in these patients should be based on strategies aiming to mitigate ventilator-induced lung injury. Assisted mechanical ventilation should be applied early in

Insights in pediatric ventilation: timing of intubation, ventilatory strategies, and weaning.

PURPOSE OF REVIEW: Mechanical ventilation is a common intervention provided by pediatric intensivists. This fact notwithstanding, the management of mechanical ventilation in pediatrics is largely guided by a few pediatric trials along with careful interpretation and application of adult data. RECENT FINDINGS: A low tidal volume, pressure limited approach to mechanical ventilation as established by the Acute Respiratory Distress Syndrome Network investigators, has become the prevailing practice in pediatric intensive care. Studies by these investigators suggest that high positive end expirator

Successful weaning from mechanical ventilation after coronary angioplasty.

Weaning failure can be caused by myocardial ischaemia during the switch from mechanical to spontaneous ventilation. We report ischaemic left ventricular failure and ischaemic mitral insufficiency during weaning. Angiography showed that the coronary vessels were stenosed. Transluminal angioplasty made weaning possible. We conclude that acute ischaemic mitral insufficiency may contribute to cardiac failure during weaning and that angioplasty, by reversing it, can allow successful weaning.

Capnometric recirculation gas tonometry and weaning from mechanical ventilation.

The aim of this study was to describe changes in regional intramucosal PCO(2) (Pr(CO(2)) measured with capnometric recirculation gas tonometry [CRGT]) in patients with acute respiratory failure, who proceed from mechanical ventilation to weaning. In addition, we compared the predictive power for the weaning outcome of CRGT measurements obtained during mechanical ventilation to the frequency/ tidal volume (f/VT) ratio. A total of 24 patients (31 weaning trials) were included in the study, but four of the 24 patients (17%) were excluded because of extubation failure. Of the remaining 27 weaning

[Problems in weaning from artificial ventilation: 'motor neuron disease'].

Three patients, two men aged 71 and one aged 73 years, were given artificial respiration because of acute respiratory failure. Subsequently they could not be weaned from artificial respiration, due to causes that were not immediately clear. It was ultimately found that the patients suffered from 'motor neuron disease', in two of them due to progressive spinal muscular atrophy, while the third, apart from loss of anterior horn motor cells, also had thoracic hydromelia. The patients died after termination of the artificial respiration.
