Utricle and horizontal semicircular canal ampulla removed during labyrinthectomy from 11 patients with advanced Meniere's disease were studied under light and electron microscope. In light microscopy the epithelium of both sensory areas appeared fairly normal. Many sensory cells revealed fluid-filled crescents at the nucleus and many cells were extensively vacuolated. Ultrastructurally, a part of the observations were apparently artifacts due to specimen handling; some were normal, age-related findings. Some changes were considered secondary to dilation of the endolymphatic space. True signs o

In summary: (i) the fact that VNS spares auditory function makes it preferable to labyrinthectomy. A comparison of VNS and the "conservative" operations on the inner ear shown that they have much the same effect on hearing. VNS by controlling vertigo may also provide an additional benefit by eliminating one of the stress factors concerned in the perpetuation of hydrops; (2) in the treatment of vertigo, VNS is now the most effective method of treatment. Because of elimination of vertigo is what chiefly concerns most of our patients, we suggest that when medical treatment fails to control vertig

OBJECTIVE: Diagnostic criteria for Vestibular Migraine (VM) and Ménière's Disease (MD) overlap considerably, complicating accurate diagnoses in patients with headaches, vertigo, hearing loss, aural fullness, and tinnitus. This study assesses the utility of multifrequency vestibular evoked myogenic potentials (VEMPs) in differentiating MD from VM.
STUDY SAMPLE: The research included 49 MD and 32 VM patients from April to June 2024. Each patient underwent a comprehensive interview and symptom documentation, followed by detailed auditory and vestibular function evaluations, including multifreque

Improved radiological examinations with newly developed 3D models may increase understanding of Meniere's disease (MD). The morphology and course of the vestibular aqueduct (VA) in the temporal bone might be related to the severity of MD. The presented study explored, if the VA of MD and non-MD patients can be grouped relative to its angle to the semicircular canals (SCC) and length using a 3D model. Scans of temporal bone specimens (TBS) were performed using micro-CT and micro flat panel volume computed tomography (mfpVCT). Furthermore, scans were carried out in patients and TBS by computed t

OBJECTIVE: Vestibular evoked myogenic potentials (VEMPs) have been suggested as biomarkers in the differential diagnosis of Menière's disease (MD) and vestibular migraine (VM). The aim of this study was to compare the degree of asymmetry for ocular (o) and cervical (c) VEMPs in large cohorts of patients with MD and VM and to follow up the responses.
STUDY DESIGN: Retrospective study in an interdisciplinary tertiary center for vertigo and balance disorders.
METHODS: cVEMPs to air-conducted sound and oVEMPs to bone-conducted vibration were recorded in 100 patients with VM and unilateral MD, re

Harmonic acceleration rotational test and vestibular autorotation test (VAT) were performed on 8 patients with Menière's disease before and after vestibular nerve section. Sinusoidal rotations of 0.01-0.64 Hz were used in the HA rotational test. VAT was performed at frequencies between 0.5 and 6 Hz. Two weeks after operation, almost all the patients consistently showed response asymmetry toward the healthy ear at all stimulus frequencies and a reduction in sensitivity to rotational stimuli at 0.01 Hz in the HA rotational test. Prior to surgery, the time constant of the vestibulo-ocular reflex

Area OP2 in the posterior peri-sylvian cortex has been proposed to be the core human vestibular cortex. We investigated the functional anatomy of OP2 and adjacent areas (OP2+) using spatially constrained independent component analysis (ICA) of functional magnetic resonance imaging (fMRI) data from the Human Connectome Project. Ten ICA-derived subregions were identified. OP2+ responses to vestibular and visual motion were analyzed in 17 controls and 17 right-sided vestibular neuritis patients who had previously undergone caloric and optokinetic stimulation during fMRI. In controls, a posterior

Characterizations of the signs and symptoms of "vestibular migraine" and of Menière's disease seem to overlap, suggesting that both diseases might be due to a common peripheral vestibular dysfunction. Thus, the aim of the present study was to assess vestibular-evoked myogenic potentials (VEMPs) in both disorders to determine whether there might be an electrophysiological link between the two disorders. The amplitude and latency of VEMPs were measured from the sternocleidomastoid muscle in 63 patients with vestibular migraine (median age 47 years, range 24-70 years) and in 16 patients with Meni

OBJECTIVE: The present study was undertaken to assess the sensitivity of vestibular evoked myogenic potentials testing to side-of-disease in Meniere's disease patients and to test the hypothesis that information supplied by vestibular evoked myogenic potentials is complementary to that provided by a conventional vestibular test battery.
STUDY DESIGN: Prospective cohort study.
SETTING: Large specialty hospital, department of otolaryngology.

Vertigo, dizziness, and oculomotor disturbances may occur as manifestations of immune-mediated disorders affecting the inner ear, central vestibular pathways, or multisystem autoimmune disease. Although uncommon, these conditions are clinically important because delayed recognition may lead to irreversible hearing loss, vestibular dysfunction, or neurological disability. This review summarizes the clinical presentation, diagnostic approach, and treatment of immune-mediated vestibular and oculomotor disorders. We suggest a practical classification into isolated immune-mediated inner ear disease

The most common types of vertigo caused by diseases of the peripheral vestibular system are benign paroxysmal positional vertigo (BPPV), Meniere's disease and vestibular neuritis. A thorough examination of the medical history and clinical examination are usually sufficient for the differential diagnostics. Treatment includes differentiated repositioning maneuvers, medicinal treatment and physiotherapy.

OBJECTIVES: The differentiation between Vestibular Migraine (VM) and Meniere's Disease (MD) is difficult because of overlapping symptoms. The study aimed to compare the clinical characteristics and vestibular function test results between VM and MD patients.
METHODS: Seventy-one patients with definite VM and 31 patients with definite unilateral MD were included. All patients received Caloric Test (CT), Video Head Impulse Test (vHIT) and Vestibular Evoked Myogenic Potential (VEMP) test within 7 days after visiting the hospital. Results of these tests were compared between groups.
RESULTS: Mos

OBJECTIVE: The staging system of Meniere's disease utilizes audiograms to probe cochlear dysfunction. We explored the addition of galvanic vestibular-evoked myogenic potentials (VEMP) to further explore vestibular function.
METHODS: Seventy patients with unilateral Meniere's disease were enrolled in this study. Within 2weeks of diagnosis, all subjects underwent pure tone audiometry, cervical and ocular VEMP, and caloric test. The prevalence of abnormal tests and the VEMP characteristic parameters such as latencies and amplitudes were analyzed.
RESULTS: In affected ears, the abnormal rate of

PURPOSE: To investigate whether patients with vestibular migraine (VM) are more susceptible to carsickness than patients with non-vestibular migraine (NVM) or Meniere's disease (MD).
METHODS: Consecutive patients with a diagnosis of definite VM, probable VM, NVM, or MD at our Headache and Dizziness clinics were interviewed using the same three questions to investigate the history of carsickness. The patients who had experienced carsickness in their lifetime and those who had still experienced carsickness in the past ten years were identified. The rates of carsickness were compared between gro
