Surgical elimination of inflammatory fibrous hyperplasia associated with combination syndrome: a two-year follow-up case report.

Inflammatory fibrous hyperplasia (IFH), also known as epulis fissuratum, is a reactive mucosal overgrowth caused by chronic denture-related irritation. This report describes a 58-year-old woman with a painful fibrous enlargement in the maxillary anterior vestibule associated with the long-term use of an ill-fitting complete denture. The lesion was excised using a scalpel, with simultaneous vestibuloplasty performed to increase vestibular depth and improve denture stability. Histopathological examination confirmed the diagnosis of IFH. Following soft tissue healing, a new maxillary complete den

Gingival augmentation with an acellular dermal matrix revisited: surgical technique for gingival grafting.

UNLABELLED: Acellular dermal matrix grafts have become popular in gingival augmentation procedures as they eliminate the disadvantages of autogenous graft materials. Since the healing dynamics of acellular dermal matrix grafts are different from those of autogenous grafts, the surgical technique developed for the autograft may not be adequate for the allograft. This article demonstrates a simple and predictable surgical technique involving an acellular dermal matrix for gingival augmentation, using graft stretching to facilitate tissue adaptation and the development of an early blood supply.

The connective tissue graft with labial vestibular extension.

Because of its predictability and simplicity, the connective tissue graft has become the method of choice in most root coverage procedures. However, in cases with prominent frenum involvement and limited vestibular depth, this procedure can be difficult, if not impossible, to accomplish. Building upon a modification of the labial vestibular extension, described by Kazanjian in 1935, a technique is presented, whereby vestibular extension is accomplished in the same one-stage surgical procedure with a connective tissue graft. The learning objective of this article is to introduce this modificati

Surgical and prosthodontic residual ridge reconstruction with hydroxyapatite.

The ultimate goal of a ridge augmentation procedure is to form a bearing surface for the denture that will provide stability, retention, and support. Residual ridge reconstruction with an implanted material must be based on an understanding of the variations of ridge atrophy. A problem-oriented classification describes the different forms of residual ridges. This classification of similar types of resorbed residual ridges can be used to establish a rational plan of treatment. The proper placement of hydroxyapatite is critical to obtain the desired reconstructed residual ridge. In the mandible,

Complete loss of lower lip in a one-year-old treated with delayed free tissue transfer: a case report and comprehensive review of the literature.

Pediatric lower lip reconstruction is rare and technically challenging due to small donor sites, ongoing facial growth, and functional demands, such as speech and oral competence. We present the case of a one-year-old male who sustained complete avulsion of the lower lip from a dog bite. Initial management included a Karapandzic advancement flap for coverage. Definitive reconstruction at 23 months involved a radial forearm free flap with palmaris longus tendon for static suspension, augmented by acellular dermal matrix. Postoperative dehiscence required revision vestibuloplasty with a split-th

Use of amniotic membranes in dentistry - literature review.

The amniotic membrane, which lacks blood vessels, lymphatic vessels and nerves, is a thin membrane with anti-inflammatory and anti-scarring properties. In the field of dentistry, it is primarily employed as a dressing following the removal of malignant or pre-malignant lesions, with the objective of ensuring haemostasis and the absence of allergic reactions. Furthermore, it is employed in the treatment of oro-auricular junctions, cleft palates and in guided bone regeneration, due to the presence of mesenchymal cells. In vestibuloplasty, it has been demonstrated to enhance vestibular deepening,

Low incidence of severe adverse effects after mandibular ridge reconstruction using hydroxylapatite.

The short and long term adverse effects after ridge reconstruction using hydroxylapatite (HA) are presented in this study. The HA was inserted using a modified tunnelling technique, followed by a lowering of the floor of the mouth and a vestibuloplasty using split thickness skin graft, 4-6 weeks later. The study comprised 637 patients followed for a period of 1 to 10 years (mean 6.0+/-2.6 years). Major loss of HA was seen in 17 patients (2.7%). Donor site visibility (skin graft) appeared to improve greatly over the years from 29.2% to 8.8% at the latest follow-up. Neurosurgery deficits also im

Transalveolar fixation of the peri-implant soft tissue in the mandible: surgical method and clinical follow-up.

PURPOSE: Firm, immobile mucosa around endosteal implants is one prerequisite for a reliable long-term result. In the atrophic mandible, this may be achieved by a vestibuloplasty on the facial side. However, on the lingual side, the floor of the mouth is often high and the mucosa mobile, especially after unintentional detachment of the lingual flap during implant surgery. To overcome this problem, a surgical technique applying a transalveolar suture circumscribing the inserted implants was developed. PATIENTS AND METHODS: The operation was performed in six patients, and they were evaluated ove

Surgical management of the atrophic edentulous ridge--a prosthetic view.

The vast majority of edentulous patients manage to wear complete dentures with relative ease. There are however a small group of patients described as "Denture Cripples" by Hopkins, Stafford and Gregory (1980), for whom the provision of complete dentures using conventional means has proved impossible. Clinical examination usually reveals marked atrophy of the alveolar ridge, the prominence of sharp bony ridges and an atrophic denture bearing mucosa. The patient complains of loose, painful dentures and an inability to masticate a normal diet. The patients can often be helped by correcting error

A Bear of a Case: Multidisciplinary Reconstruction Following a Grizzly Bear Mauling.

Grizzly bear attacks are rare but often result in devastating craniofacial injuries requiring staged, multidisciplinary reconstruction. A 61-year-old male sustained extensive facial trauma, including complete lower lip avulsion and anterior mandibular loss, after a grizzly bear attack. He underwent early stabilization followed by virtual surgical planning-guided fibula free flap reconstruction with immediate dental implants, and a radial forearm free flap incorporating palmaris tendon to restore oral competence. The patient's postoperative course was uncomplicated and he was decannulated and d

Amniotic membrane for oral surgical defect coverage: A clinical study.

Reconstruction of oral soft-tissue defects remains limited by donor-site morbidity and inconsistent healing with currently available grafts. In this prospective interventional study, we assessed human amniotic membrane (HAM) as a biologically active scaffold in 15 patients (6 cleft alveolus, 2 oral submucous fibrosis, 7 vestibuloplasty; ages 8-65 years). HAM integrated without complications in all cases, with no necrosis, infection, or immunologic rejection; median pain (VAS) fell from 2 on day 1 to 0 by day 7 (p < 0.00001) and swelling resolved by day 7 (p < 0.00001). Epithelialization reache

Periodontal plastic surgery.

Mucogingival surgery has evolved in the past decade into periodontal plastic surgery. Periodontal plastic surgery deals with those regenerative and reconstructive procedures designed not only to restore form and function but also to enhance esthetics. This paper reviews those techniques that currently comprise periodontal plastic surgery as well as the current literature pertinent to this topic. The role of the dental laser as well as its limitations will also be discussed.

Destructive membranous periodontal disease (ligneous periodontitis): a case report and 3 years follow-up.

Destructive membranous periodontal disease is a rare, destructive and poorly defined entity, which is the part of a systemic disease due to plasminogen deficiency and fibrin deposition. The disease is characterised by gingival enlargement and periodontal tissue destruction that leads to rapid tooth loss despite treatment attempts. Biopsy is essential to rule out other periodontal disease in the differential diagnosis.

Implant retained prosthesis on a free fibula flap: A challenging procedure for prosthetic and soft tissue management.

A 39-year-old woman who had undergone fibular flap reconstruction and simultaneous implant placement after the surgical removal of a cemento-ossifying fibroma was experiencing mild discomfort 3 years later and requested an implant-supported prosthesis to restore her edentulous left maxillary quadrant. Initially, an interim removable partial denture was provided and later duplicated with clear resin. The interim prosthesis identified the excessive thickness of the grafted cutaneous soft tissue and the inadequate emergence angle of the nonfunctioning implants. By using the interim prosthesis as

Gillies fan flap for the reconstruction of an upper lip defect caused by noma: case presentation.

The case of a 65-year-old noma patient with a defect involving her upper lip is presented. The defect also included missing teeth numbers eleven, twelve, 21, and 22 and the adjoining alveolus. One-stage lip reconstruction was carried out with Gillies fan flap followed by vestibuloplasty and commissuroplasty. An acrylic partial denture was subsequently fabricated to replace the missing teeth and thereby restore function. Even though the incidence of cancrum oris has reduced worldwide, cases are still being reported in Africa and can leave behind a significant facial defect. Whereas most cases o

Esthetic periodontics.

Periodontal therapy has developed beyond the scope of the treatment of periodontal pathoses. Periodontal plastic surgery has evolved beyond the techniques of mucogingival surgery and now consists of the regenerative and reconstructive procedures designed to enhance esthetics. This article briefly reviews the historical development of root coverage, ridge augmentation, and papilla preservation and provides an update of the current pertinent literature.
