Implementation of Metacognitive Reflection and Insight Therapy for Occupational Therapy (MERIT-OT) to Support Veterans Receiving Occupational Therapy Mental Health Services.

ImportanceMetacognitive Reflection and Insight Therapy for Occupational Therapy (MERIT-OT) is a novel adaptation of an evidence-based psychotherapy model designed to enhance metacognitive capacity and support recovery. Its philosophical alignment with occupational therapy-particularly the emphasis on meaning-making and life participation-makes it a promising intervention for Veterans with serious mental illness. However, to date no implementation studies of MERIT-OT have been conducted.ObjectiveTo evaluate the implementation of MERIT-OT within a Veterans Affairs (VA) occupational therapy setti

Hepatitis C Testing in the US Veterans Health Administration: Uptake of United States Preventive Services Taskforce Universal Hepatitis C Screening Guidance.

BACKGROUND: Hepatitis C virus (HCV) is a risk factor for advanced liver disease and mortality; however, efficacious direct-acting antiviral therapy is available for those who are screened for and ultimately diagnosed with HCV, highlighting the importance of population-based screening. For nearly 25 years, the Veterans Health Administration's (VHA) National Viral Hepatitis Program has leveraged resources to improve screening, diagnosis, and access to HCV care. We aim to assess the status of HCV screening in veterans in care 2021-2023 and identify emerging high-prevalence groups to direct future

A two-state comparative implementation of peer-support intervention to link veterans to health-related services after incarceration: a study protocol.

BACKGROUND: Approximately 600,000 persons are released from prison annually in the United States. Relatively few receive sufficient re-entry services and are at risk for unemployment, homelessness, poverty, substance abuse relapse and recidivism. Persons leaving prison who have a mental illness and/or a substance use disorder are particularly challenged. This project aims to create a peer mentor program to extend the reach and effectiveness of reentry services provided by the Department of Veterans' Affairs (VA). We will implement a peer support for reentry veterans sequentially in two states.

Patient, Program, and System Barriers and Facilitators to Detoxification Services in the U.S. Veterans Health Administration: A Qualitative Study of Provider Perspectives.

BACKGROUND: Because substance use disorder (SUD) treatment is expanding, and detoxification (detox) is often the entry point to SUD treatment, it is critical to provide ready access to detox services. OBJECTIVES: The purpose of the current study was to examine patient, program, and system barriers or facilitators to detox access within an integrated health care system with variable rates of detox utilization across facilities. METHODS: Inpatient and outpatient providers from 31 different U.S. Veterans Health Administration detox programs were interviewed. RESULTS: Qualitative analyses ident

Sexual orientation moderates the association between health care utilization-related factors and mental health service nonutilization among United States military veterans.

This study examined mental health treatment nonutilization among sexual minority versus heterosexual Veterans with demonstrated psychiatric need. Data were analyzed from 820 Veterans with psychiatric need who participated in the National Health and Resilience in Veterans Study, which surveyed a nationally representative sample of U.S. military Veterans. Results revealed that current mental health service nonutilization did not differ between sexual minority versus heterosexual Veterans (70.5% vs. 74.0%). Significant interactions between sexual orientation and sex assigned at birth, primary sou

Racial, ethnic, and sex differences in psychiatric diagnosis, mental health sequelae, and VHA service utilization among justice-involved veterans.

OBJECTIVE: Intervening in the cycle of symptom exacerbation and recidivism among justice-involved veterans is critical given elevated rates of psychiatric diagnoses and mental health sequelae. To responsively and effectively address justice-involved veterans' needs, it is essential to examine distinct groups who are at heightened risk (e.g., marginalized communities). Although racial and ethnic disparities within the justice system are well established, veteran-focused research remains limited. HYPOTHESES: We hypothesized that significant differences exist in service utilization, psychiatric

Individual and Military Factors That Modify the Association Between Recent Sexual Trauma and Health Outcomes Among U.S. Service Members and Veterans.

Sexual trauma (ST), which includes both sexual harassment and sexual assault, is associated with a variety of adverse mental and physical health outcomes in military and civilian populations. However, little is known about whether certain individual or military attributes or prior experiences may modify the relationship between recent ST and mental or physical health outcomes. Data from a longitudinal cohort study of current and former military members were used to examine whether individual and military factors modify the association between recent ST and health outcomes (posttraumatic stress

Does Street Outreach Engage Its Intended Target Population? Clinical Experience in the Veteran's Health Administration Homeless Service Programs.

OBJECTIVE: Clinical street outreach programs serve people experiencing unsheltered homelessness, who have been shown to have greater medical and psychiatric comorbidities, and increased social and financial challenges. However, outreach programs may struggle in practice to engage the most vulnerable of these individuals. METHODS: Data from the Veterans Health Administration's (VHA's) Homeless Operations Management System (HOMES) from 2018 to 2019 (N = 101,998) were used to compare sociodemographic, clinical, and financial characteristics of literally homeless veterans contacted through street

Evaluating the impacts of critical time intervention implementation on health service utilization among homeless-experienced veterans: a retrospective observational study.

BACKGROUND: Critical Time Intervention (CTI) is an evidence-based, time-limited case management practice that improves housing outcomes among homeless-experienced adults. While CTI's effect on housing outcomes has been well-studied, its impacts on health services utilization are less clear. In the context of a U.S Department of Veterans Affairs (VA) CTI implementation effort, we assessed relationships between CTI implementation, including fidelity, and Veterans' VA health services utilization. METHODS: We conducted a retrospective observational study of 9,051 Veterans who received services, 1

Maintaining relationship safety while promoting relationship health: Family service referrals among veterans screened for intimate partner violence.

Intimate partner violence (IPV) poses a tremendous public health burden across large health care systems. While the predominant response to IPV focuses on individual screening and referral, relationship and family services provide an opportunity to prevent low-risk verbal aggression from escalating and to address some forms of bidirectional physical IPV. This study examines mental health and screening variables associated with IPV experience and subsequent referral to family services using a large national data set of N = 256,894 patients screened for IPV in the first year of the Veterans Heal

ACOG Committee Statement No. 12: Health Care for Women and Gender-Diverse Active-Duty and Reserve Uniformed Service Members and Veterans.

An increasing percentage of the active-duty and reserve uniformed services force are women, and they are the fastest growing population in the Veterans Health Administration (VHA). Additionally, the VHA is one of the largest providers of gender-affirming care in the United States. Asking about a patient's military service and being aware of the unique health and reproductive health care needs of this population are critically important. Members of these populations may be at increased risk for a variety of health-related issues, including intimate partner violence, a history of military sexual
