Ciprofloxacin was introduced for treatment of patients with cholera in Bangladesh because of resistance to other agents, but its utility has been compromised by the decreasing ciprofloxacin susceptibility of Vibrio cholerae over time. We correlated levels of susceptibility and temporal patterns with the occurrence of mutation in gyrA, which encodes a subunit of DNA gyrase, followed by mutation in parC, which encodes a subunit of DNA topoisomerase IV. We found that ciprofloxacin activity was more recently further compromised in strains containing qnrVC3, which encodes a pentapeptide repeat prot

BACKGROUND: Vibrio vulnificus is a Gram-negative halophilic pathogen for man that can cause septicemia in patients with chronic liver disease. In healthy subjects, infections are generally local or regional. We report two cases of necrotizing dermo-hypodermitis due to Vibrio vulnificus.
CASE REPORTS: The first patient was a 21-year-old fisherman who developed extensive necrotizing dermo-hypodermitis 24 hours after a penetrating scratch with a fish bone. The second patient was also a fisherman. This 35-year-old man developed a massive area of necrotic tissue on the left leg, also 48 hours afte

Vibrio vulnificus infection with septicemia is a life threatening disease in the immunocompromised hosts. Renal involvement has not been documented. We reported herein 8 patients with V. vulnificus septicemia. All were immunocompromised hosts. Four patients had cirrhosis of the liver, 3 were heavy alcohol drinkers and one had systemic lupus erythematosis. Presenting symptomatology included fever, chills, leg pain and skin rash. Renal failure was observed in 6 patients. Four patients died shortly after admission. Two survived with clinical course of tubular necrosis. Renal failure is therefore

The authors report two cases of Vibrio cholerae isolation in hemocultures even though the copro-cultures where negative. In both cases, post-operated patients are concerned. The first one had supported a renal transplantation and was under immuno-depressor. The second one, after an aorto-aneurysm surgical treatment suffered from a mesenteric infarct. During the disease the vibrio was found in the hemoculture.

OBJECTIVE: Although the incidence in France of V. cholerae non-O1/non-O139 infection in man has increased since 1996, it remains low (7 cases in 1999). After the death in 1994 of an immunodepressed patient presenting a skin lesion showing superinfection by a strain of non-O1/non-O139 V. cholerae following exposure to seawater, we examined 22 samples of sea-water collected from 20 French coastal areas (Mediterranean coast).
METHODS: The sea-water samples were filtered and enriched with alkaline peptone water (APW), and the strains of Vibrio were isolated on TCBS, SS and BCP media and identifie

Vibrio fluvialis is a gram-negative bacillus commonly implicated in outbreaks of gastroenteritis. The bacterium has been identified as an emerging pathogen in recent years, with increasing reports of extraintestinal manifestations. This report discusses a case of bacteremia presumed secondary to bullous cellulitis of the left lower extremity, with blood cultures positive for Vibrio fluvialis and Streptococcus dysgalactiae. The patient presented with progressively worsening quadriplegia and falls, with one week of progressive discoloration of the left lower extremity. He denied any gastrointest

Vibrio cholerae is a major human pathogen causing the diarrheal disease, cholera. Regulation of virulence in V. cholerae is a multifaceted process involving gene expression changes at the transcriptional and post-transcriptional level. Whereas various transcription factors have been reported to modulate virulence in V. cholerae, small regulatory RNAs (sRNAs) have now been established to also participate in virulence control and the regulation of virulence-associated processes, such as biofilm formation, quorum sensing, stress response, and metabolism. In most cases, these sRNAs act by base-pai

Most bacteria are protected from environmental offenses by a cell wall consisting of strong yet elastic peptidoglycan. The cell wall is essential for preserving bacterial morphology and viability, and thus the enzymes involved in the production and turnover of peptidoglycan have become preferred targets for many of our most successful antibiotics. In the past decades, Vibrio cholerae, the gram-negative pathogen causing the diarrheal disease cholera, has become a major model for understanding cell wall genetics, biochemistry, and physiology. More than 100 articles have shed light on novel cell

Vibrio cholerae is the causative agent of the severe diarrheal disease cholera. V. cholerae thrives within the human host, where it replicates to high numbers, but it also persists within the aquatic environments of ocean and brackish water. To survive within these nutritionally diverse environments, V. cholerae must encode the necessary tools to acquire the essential nutrient iron in all forms it may encounter. A prior study of systems involved in iron transport in V. cholerae revealed the existence of vciB, which, while unable to directly transport iron, stimulates the transport of iron thro

Vibrio cholerae, the causative agent of cholera, swims in aqueous environments with a single polar flagellum. In a spatial gradient of a chemical, the bacterium can migrate in "favorable" directions, a property that is termed chemotaxis. The chemotaxis of V. cholerae is not only critical for survival in various environments and but also is implicated in pathogenicity. In this chapter, we describe how to characterize the chemotactic behaviors of V. cholerae: these methods include swarm assay, temporal stimulation assay, capillary assay, and receptor methylation assay.

In this monographic issue, we have the pleasure to present contributions from six of the leading laboratories at the forefront of Vibrio cholerae genetics, ecology and evolution, together with a brief tribute by Diego Romero to Doctor Jaime Ferrán y Clua, a pioneering Spanish bacteriologist who developed the first vaccine against this pathogen. V. cholerae is a free-living aquatic bacterium that interacts with and infects a variety of organisms. In humans it causes cholera, the deadly diarrhoea that was responsible for millions of deaths during seven pandemics since 1817, and still thousands e

Vibrio cholerae can switch between motile and biofilm lifestyles. The last decades have been marked by a remarkable increase in our knowledge of the structure, regulation, and function of biofilms formed under laboratory conditions. Evidence has grown suggesting that V. cholerae can form biofilm-like aggregates during infection that could play a critical role in pathogenesis and disease transmission. However, the structure and regulation of biofilms formed during infection, as well as their role in intestinal colonization and virulence, remains poorly understood. Here, we review (i) the eviden

A report of a fatal case of necrotizing fasciitis of the leg due to Vibrio parahaemolyticus infection. This organism frequently causes minor infections in seawater-exposed wounds. In this case the patient died due to fulminant sepsis caused by necrotizing fasciitis, despite surgical debridement. This case serves as a reminder for targeted antibiotic coverage for Vibrio in any serious saltwater-related infection, and as an illustration of the rapid and severe systemic toxicity associated with Vibrio parahaemolyticus necrotizing fasciitis, even in the setting of minor initial skin findings.

Vibrio vulnificus infection is the leading cause of death related to seafood consumption in the United States. This virulent, gram-negative bacterium causes two distinct syndromes. The first is an overwhelming primary septicemia caused by consuming raw or undercooked seafood, particularly raw oysters. The second is a necrotizing wound infection acquired when an open wound is exposed to warm seawater with high concentrations of V. vulnificus. Most patients, including those with primary infection, develop sepsis and severe cellulitis with rapid development to ecchymoses and bullae. In severe cas

PURPOSE: To describe the epidemiology, pathophysiology, diagnosis, treatment, and prevention of Vibrio vulnificus, a rare organism that surfaced following Huricane Katrina and caused serious problems for many victims.
DATA SOURCES: Selected scientific literature supplemented with case presentations of patients seen following Katrina.
CONCLUSIONS: After Hurricane Katrina and as a result of extensive flooding of below-sea-level New Orleans, toxic waters contaminated many areas of the city. As a result of this extensive flooding of toxic waters, people were exposed to V. vulnificus.

Experimental data confirming our earlier suggestion, that cholerae toxin (CT) possesses superantigen (SA) properties are presented. When used in very small doses, CT has been found to induce polyclonal activation of T lymphocytes, essentially exceeding that observed in classical T mitogens characteristic of SA. CT, in contrast to mitogens and similarly to other SA, is shown to display this activity only in the presence of antigen-presenting cells. Experiments with the use of monoclonal antibodies to the variable region of the beta-chain of the T-cell receptor (V beta TCR) have demonstrated tha

Snakebite may be complicated with bacteria infections, most of which are Gram-negative bacillus. Snakebite complicated with Vibrio vulnificus infection has not been reported previously. Vibrio vulnificus infection is characterized by a necrotizing wound infection similar to the local lesion of severe snakebite. In Vibrio infections, aggressive debridement with the administration of strong antibiotics as early as possible is the only effective treatment. We report a 79-y-o man who suffered from snakebite on the right palm with rapid progression of hemorrhagic bulla, necrotizing fasciitis, compa
