Vinblastine is a clinically important anticancer alkaloid, and the development of efficient microbial production systems remains a promising strategy for improving its sustainable availability. In the present study, Alternaria alternata Mut-85 was evaluated for vinblastine production using immobilized fungal cultures. BACKGROUND: The standard chemotherapy treatment for large-cell lymphoma in cats is CHOP (cyclophosphamide, doxorubicin, vincristine, and prednisolone) or COP (cyclophosphamide, vincristine, and prednisolone) chemotherapy protocols. Substituting vinblastine for vincristine might have similar efficacy, with lower severity of gastrointestinal adverse events.
HYPOTHESIS/OBJECTIVES: To evaluate whether the addition of vinblastine to a low-dose vincristine COP protocol could reduce the frequency and severity of adverse gastrointestinal effects while maintaining or increasing efficacy.

An HIV-positive man from Somalia presented with severe malaise, weight loss, relapsing fever, lymphadenopathy and splenomegaly. An FDG-PET-scan-guided lymph node biopsy revealed the characteristic histological features of the plasma cell variant of Castleman's disease. A high HHV-8 viral load was detected in the serum (7980 copies/ml). Treatment with HAART, rituximab and vinblastine resulted in a full and rapid recovery and lowered HHV-8 viral load to undetectable levels.

Methotrexate, vinblastine, doxorubicin and cisplatin were used to treat 66 patients with advanced urothelial cancer. Of these 66 patients 58 could be evaluated for response. A total of 84 sites was evaluated in these patients. Response rates were 73% in the bladder, 67% in the renal pelvis, 50% in the ureter, 60% in the lung, 68% in the lymph nodes, 14% in the liver and 25% in the bone. Twenty patients with Stage IV and five patients with Stage III carcinoma of the head and neck were treated with the combination of cisplatin and vinblastine before locoregional therapy. Seven patients with germ cell tumors of the ovary were treated with four cycles of cisplatin, vinblastine, and bleomycin. Six patients were without evidence of disease 17+ to 37+ months after completion of the remission-induction chemotherapy, and one patient relapsed after 9 months. Substantial toxicity of the cisplatin, vinblastine, and bleomycin regimen was noted. Despite the high response rate suggested in this small series, the optimal chemotherapeutic treatment for this disease has yet to be determined.

The successful treatment of a patient with idiopathic thrombocytopenia (ITP) by vinblastine-loaded platelets (VMT) is described. Serum autoantibodies and platelet-bound IgG disappeared after treatment. We infer that this type of therapy does not oeprate on the level of platelet destroying cells but rather affects antibody producing cells. Since there was also a T-cell sensitization versus VMT we favour the hypothesis of a modified self alteration as the cause of the disease and its recovery of VMT.

A phase II trial of vinblastine in patients with refractory epithelial ovarian adenocarcinoma of the ovary was conducted by the Gynecologic Oncology Group (GOG) between March 9, 1988 and July 7, 1988.
