Exploring the Experiences of LTCF Staff in Implementing Visitation Policies in England During the COVID-19 Pandemic.

Background: Restrictions on family visitation to long-term care facilities (LTCFs) during the COVID-19 pandemic remain an area of contention for residents, family members and staff members. Current research has explored the experience of family members; however, fewer studies have explored the impact of visitation restrictions from the perspective of LTCF staff members. We examined the experiences of LTCF staff members in implementing visitation restrictions, including maintaining contact with families, in England over the course of the COVID-19 pandemic. Method: A sample of twenty-four LTCF s

Outbreak of tick-borne relapsing fever at the north rim of the Grand Canyon: evidence for effectiveness of preventive measures.

An outbreak of tick-borne relapsing fever (TBRF) originating at the North Rim of Grand Canyon National Park was investigated in 1990. To determine risk factors for the disease, almost 7,000 parties of visitors were surveyed; over half responded, representing > 10,000 people. Fifteen cases of confirmed or probable TBRF were identified in visitors and 2 in employees. All patients except one experienced symptoms after overnight stays in a group of cabins that had not been rodent-proofed after a TBRF outbreak in 1973 (relative risk for visitors [RR] 8.2, 95% confidence interval [CI] 1.1-62). Deployment of a computerized ward visitor registration system in coronavirus disease 2019 epidemic: Experiences of a large academic medical center in Taiwan.

BACKGROUND: Visitors to hospitalized patients during an epidemic might themselves be carriers and are therefore likely to spread the disease in wards. Although measures were taken to restrict hospital visits, traditional paper-based registration is insufficient to screen and monitor the numbers of visitors to a large hospital. METHODS: Throughout March 2020, during the coronavirus disease 2019 crisis, a computer system was deployed in the 2800-bed Taipei Veterans General Hospital (Taipei, Taiwan) to register, screen, and monitor inpatient visitors. This system comprised three parts: online re

A Bayesian Network-Based Browsing Model for Patients Seeking Radiology-Related Information on Hospital Websites: Development and Usability Study.

BACKGROUND: An increasing number of people are visiting hospital websites to seek better services and treatments compared to the past. It is therefore important for hospitals to develop websites to meet the needs of their patients. However, few studies have investigated whether and how the current hospital websites meet the patient's needs. Above all, in radiation departments, it may be difficult for patients to obtain the desired information regarding modality and diagnosis because such information is subdivided when described on a website. OBJECTIVE: The purpose of this study is to suggest

Investigating the Impact of the Family Interaction Model on the Anxiety and Mental Well-Being of Patients During Visit Restrictions in Intensive Care Units: A Mixed-Methods Study.

BACKGROUND: Patients hospitalised in the intensive care unit (ICU) often experience feelings of isolation and a sense of profound loneliness. These individuals frequently report symptoms of anxiety, depression and other psychiatric disorders. AIM: The aim of this study was to examine the impact of the family interaction model on the anxiety and mental well-being of patients during the visit restrictions in the ICUs. Additionally, the experiences of intensive care patients and their relatives regarding this process were investigated. STUDY DESIGN: This research utilised a parallel design mixe

Hospital visitors as controls in case-control studies.

OBJECTIVE: Selecting controls is one of the most difficult tasks in the design of case-control studies. Hospital controls may be inadequate and random controls drawn from the base population may be unavailable. The aim was to assess the use of hospital visitors as controls in a case-control study on the association of organochlorinated compounds and other risk factors for breast cancer conducted in the main hospital of the "Instituto Nacional de Câncer" - INCA (National Cancer Institute) in Rio de Janeiro (Brazil). METHODS: The study included 177 incident cases and 377 controls recruited amon

Study protocol to assess the effectiveness and safety of a flexible family visitation model for delirium prevention in adult intensive care units: a cluster-randomised, crossover trial (The ICU Visits Study).

INTRODUCTION: Flexible intensive care unit (ICU) visiting hours have been proposed as a means to improve patient-centred and family-centred care. However, randomised trials evaluating the effects of flexible family visitation models (FFVMs) are scarce. This study aims to compare the effectiveness and safety of an FFVM versus a restrictive family visitation model (RFVM) on delirium prevention among ICU patients, as well as to analyse its potential effects on family members and ICU professionals. METHODS AND ANALYSIS: A cluster-randomised crossover trial involving adult ICU patients, family mem

Family-centered critical care: an option or obligation?

The patient was dying after undergoing aggressive treatment for a malignancy. Patient and family wanted "everything" to be done and the patient was transferred to the ICU for treatment of acute respiratory failure. The next day the patient's condition deteriorated further. The family decided against chest compressions or defibrillation; however, other aggressive treatment was continued. A "chemical code" was initiated and the patient was ventilated. The family was informed. As they stood in the hall outside the unit, the patient's wife asked if she could be with her husband. A nurse explained

[Novel approach for the early detection of inflammatory rheumatic diseases in the population using a mobile screening unit].

Current evidence-based guidelines for patients with rheumatoid arthritis (RA) emphasize the need for early diagnosis and treatment with disease-modifying antirheumatic drugs. However, these recommendations have not been implemented universally in Germany. This prompted the German Cooperative Rheumatology Group Rhine-Ruhr to devise a screening programme for inflammatory rheumatic diseases (IRD) using a specially equipped bus, which was sent on a 5-week tour of a series on local towns. Visitors were asked to fill out a questionnaire (RheumaCheck) and to undergo a blood test for rheumatoid factor

The role of community screening for developmental dysplasia of the hip at the 8-month baby check.

BACKGROUND: Screening for developmental dysplasia of the hip (DDH) after the neonatal period is controversial. Due to recent changes in the screening policy in England & Wales, routine clinical screening for DDH at the 8-month baby check is no longer recommended. RESULTS: This paper looks at the effectiveness of screening for DDH by health visitors in the Flintshire area. A total of 525 8-month baby checks were performed in our area in 2007. Thirty babies were referred to their general practitioner of which six were referred on to a specialist children's orthopaedic surgeon. None of these req

Epidemiological features of a newly described serotype of Shigella boydii.

We report the clinical, microbiological, and epidemiological features of an emerging serotype, Shigella boydii 20. We interviewed patients about symptoms, and history of travel and visitors during the week before illness onset. Seventy-five per cent of the 56 patients were Hispanic. During the week before illness onset, 18 (32%) travelled abroad; 17 (94%) had visited Mexico. Eight (21%) out of 38 who had not travelled had foreign visitors. There were eight closely related patterns by PFGE with XbaI. S. boydii 20 may be related to travel to Mexico and Hispanic ethnicity. Prompt epidemiological

Emergency Nursing and Staff Experiences With Visitation Restrictions During the Coronavirus Disease 2019 Pandemic: A Qualitative Descriptive Study.

INTRODUCTION: At the onset of the coronavirus disease 2019 pandemic, visitors were prohibited in the emergency department, leaving patients bereft of their companionship and advocacy. However, little was known about the conflicts and distress of emergency department staff related to this policy. This study aimed to qualitatively explore emergency nursing and staff experiences regarding the no-visitor policy. METHODS: A qualitative descriptive approach explored short answers to 1 open-ended question. Emergency nurses and assistive personnel (patient care technicians and emergency medical techn

Caregivers as essential members of healthcare teams: lessons learned from enforcement of the "no visitors" policy during the COVID-19 pandemic in Ontario, Canada.

BACKGROUND: The COVID-19 pandemic brought to the forefront caregivers’ unmeasured, unpaid work. Despite increasing recognition of caregivers as essential care-partners, during the pandemic, “no visitor” hospital restrictions prevented caregivers from engaging in their pivotal role within the healthcare system. This qualitative research aimed to better understand the wider effects of the pandemic on cancer care in Ontario, Canada with a focus on the experiences of cancer patients, caregivers, healthcare providers, and healthcare decision-makers (leaders in oncology who direct care, manage units

Do we have enough evidence how seasonal influenza is transmitted and can be prevented in hospitals to implement a comprehensive policy?

PURPOSE: To identify if there is enough evidence at low risk-of-bias to prevent influenza transmission by vaccinating health-care workers (HCWs), patients and visitors; screening for laboratory-proven influenza all entering hospitals; screening asymptomatic individuals; identifying influenza supershedders; hand-washing and mask-wearing by HCWs, patients and visitors; and cleaning hospital rooms and equipment. PRINCIPAL RESULTS: Vaccination reduces influenza episodes of vaccinated (4.81/100 HCW) compared to unvaccinated (7.54/100) HCWs/influenza season. A Cochrane review found for inactivated

Family Presence in Pediatric Cardiac Procedural Settings: A Qualitative Study of Clinicians, A Key Stakeholder Group.

BACKGROUND: With increased emphasis on healthcare transparency, parents are increasingly asking to be present for procedures performed on their children, especially in high-acuity contexts like care of children with congenital heart disease (CHD), where procedures may inform critical care decisions. In addition, observations of complex care may better communicate clinical knowledge and benefit grieving after adverse events. We examined clinicians' views on current family presence (FP) efforts and on the expansion of FP to include the observation of operative procedures. METHODS: This is a qua
