Relationship between delirium and behavioral symptoms of dementia.

BACKGROUND: Persons with dementia frequently present behavioral and psychological symptoms as well as delirium. However, the association between these has received little attention from researchers and current knowledge in this area is limited. The purpose of this study was to examine the relation between delirium and behavioral symptoms of dementia (BSD). METHODS: Participants were 155 persons with a diagnosis of dementia, 109 (70.3%) of whom were found delirious according to the Confusion Assessment Method. BSD were assessed using the Nursing Home Behavior Problem Scale. RESULTS: Participa

An administrator's perspective on mental health in assisted living.

Administrators of 94 assisted living facilities in Michigan completed a 19-item survey about the extent of mental health problems of residents and how such problems were addressed. In 45 facilities more than half of the residents were reported to be cognitively impaired. The two most common problems were dementia and depression. The most common problematic behaviors were resistance to care and wandering. Two-thirds of the facilities conducted some type of mental health evaluation at admission, and two-thirds conducted evaluations as needed. The most common treatment barriers were patient refus

Exploratory research on outcomes for individuals missing through dementia wandering in Japan.

AIM: To clarify the factors related to outcomes of individuals missing through dementia wandering. METHODS: We carried out binary logistic regression analysis with 160 family members of missing persons (88 survivors, 72 decedents). RESULTS: Living alone at the point of going missing (OR 13.45, 95% CI 1.12-161.56), being found within the local area (OR 6.36, 95% CI 2.26-17.88), and a long period between going missing and being found (OR 6.14, 95% CI 3.14-12.00) were significantly related with death. CONCLUSIONS: Early detection of missing persons is essential in preventing dementia wandering

Interdisciplinary Process Improvement for Behavior Management of Acute Brain Injury.

BACKGROUND: Following a traumatic brain injury (TBI), 70% or more of patients display disruptive behaviors including verbal or physical aggression, wandering, nonadherence or care refusal, social withdrawal, and socially inappropriate behavior. These behaviors often worsen over the first few years after a TBI and place a significant strain on caring relationships. PURPOSE: Our brain injury program aimed to refine our inpatient rehabilitation programming approach, using the evidence base and provider expertise, to address disruptive behaviors among patients with TBI. METHODS: We formed an int

Agitation and other noncognitive abnormalities in Alzheimer's disease.

Agitation and other noncognitive abnormalities in patients with Alzheimer's disease are present in at least 50% of patients and are a serious problem for caregivers. Agitation can be divided into aggressive agitation, physically nonaggressive agitation, and verbal agitation. Persecutory delusions of suspiciousness and stealing are the most common psychotic symptoms. Auditory and visual hallucinations are also associated with delusions. Similar to delusions are misidentifications, which are false beliefs probably secondary to agnosia. They occur in one third of patients with dementia of the Alz

Ruminative minds, wandering minds: Effects of rumination and mind wandering on lexical associations, pitch imitation and eye behaviour.

This study demonstrates that rumination is reflected in two behavioural signals that both play an important role in face-to-face interactions and provides evidence for the negative impact of rumination on social cognition. Sixty-one students were randomly assigned either to a condition in which rumination was induced or to a control condition. Their task was to play a speech-based word association game with an Embodied Conversational Agent during which their word associations, pitch imitation and eye movements were measured. Two questionnaires assessed their ruminative tendencies and mind wand

Closed-Loop Frontal Midlineθ Neurofeedback: A Novel Approach for Training Focused-Attention Meditation.

Cortical oscillations serve as an index of both sensory and cognitive processes and represent one of the most promising candidates for training and targeting the top-down mechanisms underlying executive functions. Research findings suggest that theta (θ) oscillations (3-7 Hz) recorded over frontal-midline electrodes are broadly associated with a number of higher-order cognitive processes and may serve as the mechanistic backbone for cognitive control. Frontal-midline theta (FMθ) oscillations have also been shown to inversely correlate with activity in the default mode network (DMN), a network

[Trazodone for the treatment of behavioral and psychological symptoms of dementia (BPSD) in Alzheimer's disease: a retrospective study focused on the aggression and negativism in caregiving situations].

Behavioral and psychological symptoms of dementia (BPSD) includes anxiety, depression, hallucination, delusion, aggression, irritability, agitation and wandering. BPSD often causes a deterioration of activity of daily living (ADL) and worsens caregiver burden. Trazodone, an atypical antidepressant, is used for the treatment of BPSD, but the effectiveness is controversial. In this study, we retrospectively analyzed the medical records of the 13 AD patients who were rated as having the aggression and negativism in caregiving situation and were treated by trazodone. The BPSD of the per-treatment

When attention wanders: how uncontrolled fluctuations in attention affect performance.

No matter how hard subjects concentrate on a task, their minds wander (Raichle et al., 2001; Buckner et al., 2008; Christoff et al., 2009; Killingsworth and Gilbert, 2010). Internal fluctuations cannot be measured behaviorally or from conventional neurophysiological measures, so their effects on performance have been difficult to study. Previously, we measured fluctuations in visual attention using the responses of populations of simultaneously recorded neurons in macaque visual cortex (Cohen and Maunsell, 2010). Here, we use this ability to investigate how attentional fluctuations affect perf

Decreased striatal D2 receptor density associated with severe behavioral abnormality in Alzheimer's disease.

OBJECTIVES: Since patients manifesting behavioral and psychological symptoms of dementia (BPSD) are a burden for their families and caregivers, the underlying neurobiological mechanism of this condition should be clarified. Using positron emission tomography (PET), we previously reported that wandering behavior in dementia was associated with a disturbed dopaminergic neuron system. We herein investigated the relationship between the severity of BPSD and the striatal D2 receptor density in Alzheimer's disease (AD). METHODS: Ten patients with probable AD as per the National Institute of Neurolo

Exploring behavioral outcomes of gonadectomy in pets: a need for personalized approaches.

Gonadectomy, the surgical removal of reproductive organs-is commonly practiced in canine management and is often associated with responsible pet ownership. Although advised for preventing unwanted litter and specific health hazards, its effects on canine behavior remain contentious and little studied. This article analyzes the physiological and behavioral effects of gonadectomy in dogs, emphasizing other sexually dimorphic behaviors including mounting, wandering, and urine marking, alongside aggression, fear, anxiety, separation-related issues, and canine cognitive dysfunction. Gonadectomy imp

Psychiatric symptoms vary with the severity of dementia in probable Alzheimer's disease.

This cross-sectional study examines relationships among the constellation of psychiatric syndromes in Alzheimer's disease (AD) as a function of dementia severity in 1155 patients with probable AD. The frequency of major depression decreased in severe stages, while agitation, aggression, and psychosis were more frequent in late stages. Major depression was associated with anhedonia, sleep disorders, depressed mood, low self-esteem, anxiety, and hopelessness in mild/moderate and severe stages. Agitation was associated with aggression and psychosis in mild/moderate stages, and psychosis was assoc

Anxiety of Alzheimer's disease: prevalence, and comorbidity.

BACKGROUND: Anxiety may be associated with psychiatric morbidity, disability, increased health care utilization, and mortality in Alzheimer's disease (AD) patients as it is in the general adult population. However, the phenomenology of anxiety symptoms in AD and its relationship to dementia progression, comorbid depression, and the presence of other problematic behaviors have not yet been examined. METHOD: Data on anxiety symptoms and their coexistence with other factors were obtained in 523 community-dwelling AD patients through interviews with their caregivers and direct physical examinatio

Fundamentals of pacemakers ECG interpretation - part 2.

BACKGROUND: Modern pacemakers incorporate arrhythmia-response algorithms, ventricular pacing minimization protocols, and safety mechanisms that generate ECG patterns indistinguishable from pathological AV block, sensing malfunction, or device-mediated tachycardia. Failure to recognize these algorithm-driven signatures leads to unnecessary interventions, misdiagnosis, and inappropriate device reprogramming. This manuscript is the second in a two-part series on pacemaker ECG interpretation. METHODS: We conducted a narrative review of peer-reviewed literature and device-specific documentation on

Cognitive rigidity is mirrored by autonomic inflexibility in daily life perseverative cognition.

Mind wandering (MW) can be persistent and therefore has been included in the repetitive thinking conceptualization. In line with a dimensional view of psychopathology, we hypothesized the existence of a MW-Perseverative Cognition (PC) continuum, where the latter is characterized by a rigid and defensive pattern with attentional, behavioral, affective, and autonomic perseverative manifestations. Ambulatory heart rate (HR) and variability (HRV) of 42 participants were recorded for 24h. Approximately every 30 min during waking, subjects reported their ongoing thoughts and moods using electronic d

Feasibility of Internet-Based Mind-Body Training for Adults With Subjective Cognitive Decline: Protocol for a Randomized Controlled Trial.

BACKGROUND: In the United States, the prevalence of Alzheimer disease (AD) is projected to double over the next 30 years, with associated familial and societal costs estimated at US $1 trillion annually if current trends continue. Although pharmacological treatments of AD are showing promise, the adoption of healthy lifestyle behaviors, particularly during the preclinical phase of AD, may reduce dementia rates by up to 45%. Subjective cognitive decline (SCD), defined as persistent self-perceived declines in cognitive functioning compared with previously normal cognitive abilities, has been ide

Gaze behavior of pre-adolescent children afflicted with Asperger syndrome.

Asperger syndrome (AS) is a form of high-functioning autism characterized by qualitative impairment in social interaction. People afflicted with AS typically have abnormal nonverbal behaviors which are often manifested by avoiding eye contact. Gaze constitutes an important interactional resource, and an AS person's tendency to avoid eye contact may affect the fluidity of conversations and cause misunderstandings. For this reason, it is important to know the precise ways in which this avoidance is done, and in what ways it affects the interaction. The objective of this article is to describe th
