Anaesthesia on the Western Front-perspectives a century later.

This year, 2016, marks the centenary of some of the worst battles of World War I. It is timely to reflect on the experiences of those who lived through those events, and the significant effects on the development of anaesthesia that occurred as a result. War has been a catalyst that has accelerated the development of medicine and surgery throughout history. The challenges of the Western Front, with its massive numbers of injured, saw progress in the understanding and management of trauma and shock, and significant improvement in the provision of anaesthesia and the training of anaesthesia prov

British prisoners-of-war: from resilience to psychological vulnerability: reality or perception.

In contemporary culture, soldiers held as prisoners-of-war (POWs) or as hostages are considered at significant risk of mental illness, in particular post-traumatic stress disorder (PTSD). This assumption contrasts with the psychiatric orthodoxy of the First World War when it was concluded in both Britain and Germany that POWs were protected against 'war neurosis'. Although 'barbed wire disease' was identified during time of captivity, post-release effects were not recognized. The repatriation of 'protected' POWs in 1943 prompted a reassessment of the psychological impact of imprisonment when s

Jean-Charles Chatelin (1884-1948), counted among the "Righteous", but forgotten as a neurologist who studied under Pierre Marie.

Charles Chatelin (1884-1948) studied under Pierre Marie (1853-1940) at hôpital La Salpêtrière and went on to a career profoundly affected by World War I. He wrote a remarkable thesis on the clinical aspects and radiography of hereditary craniofacial dysostosis, which had been recently described by Octave Crouzon (1874-1938). A few days after the publication of Georges Guillain (1876-1961) and Alexandre Barré (1880-1967), Chatelin published a comprehensive study of the eponymous syndrome. His study was prepared before that of Guillain and Barré, but only their names are remembered. After examin

In Flanders fields: the Great War, Antoine Depage, and the resurgence of débridement.

OBJECTIVE: The care of traumatic wounds has evolved over hundreds of years, largely as a result of armed conflicts. The lessons learned during World War I in the treatment of extensive soft-tissue injuries proved invaluable in reducing infection and preventing loss of limb and life. Foremost among these was the use of debridement. This report reviews the development of debridement as standard treatment of war wounds and highlights the surgeon largely responsible for its resurgence during one of this century's saddest chapters. SUMMARY BACKGROUND DATA: Before World War I, the care of wounds co

U.S. military enlisted accession mental health screening: history and current practice.

Through the stimulus of war and concerns about neuropsychiatric disability, the U.S. military developed methods to rapidly screen the mental health of World War I and II draftees. Intelligence testing and brief psychiatric screening expanded the accession physical examination and underwent revision to identify only gross mental health disability. Supplemental psychiatric evaluations and written psychological screening tools were abandoned after postwar assessments; they demonstrated poor predictive power in evaluating recruit service capacity for combat environments. Currently, only three ment

Excavation of mass graves with Serbian context: Complexity of the political milieu.

This paper will discuss the complexity of the investigation of mass graves in today's Serbia (part of ex Yugoslavia), belonging primarily to WWII with an exception of one mass grave from WWI, through the prism of historical events behind it. By interpreting historical and political circumstances, we will show why and on what occasions mass graves, as cultural-historical monuments and symbols of the sufferings of Serbs, Jews, Romani, and other nations or ethnic groups, remained outside the focus of expert teams. Also, through examples of explored sites related to WWII (Višnjica, Topola, Kraguje

History of internal fixation with plates (part 2): new developments after World War II; compressing plates and locked plates.

The first techniques of operative fracture with plates were developed in the 19th century. In fact, at the beginning these methods consisted of an open reduction of the fracture usually followed by a very unstable fixation. As a consequence, the fracture had to be opened with a real risk of (sometimes lethal) infection, and due to unstable fixation, protection with a cast was often necessary. During the period between World Wars I and II, plates for fracture fixation developed with great variety. It became increasingly recognised that, because a fracture of a long bone normally heals with mini

[Physicians as Experts of the Integration of war invalids of WWI and WWII].

After the First World War the large number of war invalids posed a medical as well as a socio-political problem. This needed to be addressed, at least to some extent, through healthcare providers (Versorgungsbehörden) and reintegration into the labour market. Due to the demilitarization of Germany, this task was taken on by the civil administration, which was dissolved during the time of National Socialism. In 1950, the Federal Republic of Germany enacted the Federal War Victims Relief Act (Bundesversorgungsgesetz), which created a privileged group of civil and military war invalids, whereas o

Frederic Lewy: how the two World Wars changed his life, work, and name.

In 1912, Friedrich Lewy described the inclusion bodies present in Parkinson disease and in Lewy body dementia. Throughout his life, Lewy fought in two wars - on opposite sides. He was born in Berlin in a Jewish family, and served in the German Army in World War I. In the following years, on many occasions he had to change his line of research due to Nazi persecution. Lewy became a naturalized American, changed his name to Frederic Henry Lewey, and served in the US Army as a lieutenant colonel. Lewy died in 1950 and never used the famous eponym in his papers.

Organization of the German Army Medical Service 1914-1918 and the role of academic surgeons.

The First World War, mankind's first high-technology conflict, resulted in unprecedented mass mortality. Medical services were confronted with overwhelming challenges in treating casualties from mobile warfare, trench warfare, battles on different fronts and infectious diseases. In this article, we give an overview of the organization of the German army medical service using contemporary reports in order to describe surgical and medical developments that originated from the experience. Consulting physicians (beratende arzte), many of whom were internationally known specialists, had a great imp

The music of war: Seven World War 1 composers and their experience of combat.

The effect of World War 1 military service on composers has been neglected in comparison with poets and artists. This article describes the wartime service of Arthur Bliss, Ralph Vaughan Williams, Ivor Gurney, EJ Moeran, Gordon Jacob, Patrick Hadley, and Maurice Ravel. The relationship between experiences of combat and the psychological health of these men is examined, with consideration being given to predisposition and possible causative influences of military service on their later careers, examined from individual and societal perspectives.

A heroic blood transfusion during the First World War (January 1915).

This article documents a rare surviving material testimony of a battlefield blood transfusion performed during the First World War. The study reconstructs a "heroic" father-to-son arteriovenous transfusion carried out in Cherbourg Naval Hospital on January 24th, 1915. Analysis of the preserved glass cannula highlights an original French adaptation of the Alexis Carrel transfusion method. The paper explores the evolution of early transfusion technologies and vascular techniques before the standardization of modern blood banking. This memorial object illustrates the intersection of wartime surge

Atatürk's kidney disease: pyelonephritis or trench nephritis? A medical and historical analysis.

BACKGROUND: The aim of this study is to examine Atatürk's recurrent renal disease throughout his life in light of historical and medical evidence and to evaluate, from a medical history perspective, whether this condition is more consistent with pyelonephritis or with "trench nephritis," as described among soldiers during World War I. METHODS: This study was designed as a qualitative narrative review using document and content analysis. Primary and secondary sources obtained from the Archives of the Presidency of the Republic of Türkiye, the National Library, newspaper archives, physicians' r

[Health problems of combatants during the First World War].

The First World War because of the use of new weapons, injured more than 3 500 000 people (500 000 in the face), more than diseases (tuberculosis, typhoid fever, etc.) or even weather circumstances. The healing of the war wounds through surgery undertook a significant evolution thanks to the use of asepsis and antiseptics. Mortality go down, opening the way to the physical and psychological rehabilitation of those injured by the war.

The effect of the First World War (1914-1918) on the development of British anaesthesia.

One of the greatest but also most unfortunate ironies in life is how modern medicine owes some of its existence to the deadly cancer of war. For those whose image of armed conflict is one of disease, death and destruction, this will no doubt be a surprise. However, these very conditions have allowed military surgeons and physicians unparalleled opportunities to experiment and develop using large and dependent populations of potential patients. The catalyzing effect of war has seen the ambulance, the hospital, plastic surgery, preventative medicine and penicillin as just a few products whose hi

Symonds on fear and Post Traumatic Stress Disorder (PTSD).

Prominent English neurologist Sir Charles Symonds, during World War II service with the Royal Air Force, published a series of articles emphasizing the role of fear initiating psychological breakdown in combat airmen (termed Lack of Moral Fibre). Having served in a medical capacity in the previous war, Symonds re-presented the phylogenetic conceptualizations formed by his colleagues addressing 'shell shock'. In 2013, the Diagnostic and Statistical Manual of Mental Disorders, fifth edition (DSM-5) re-classified Post Traumatic Stress Disorder (PTSD), removing the diagnosis from the category of A

How the US Army Forged Hand Surgery.

As the United States plunged into World War II, the surgeon general, Norman T. Kirk, scrambled to care for the complex hand injuries sustained in combat. To remedy this problem, Major General Kirk appointed Sterling Bunnell, a general surgeon and a World War I veteran with a keen interest in hand injuries, to serve as the consultant to the Secretary of War. Kirk and Bunnell formed 9 US military hand centers that treated 22,000 hand injuries in World War II. Bunnell and his pupils would later form the nucleus of the American Society for Surgery of the Hand. Through Dr. Bunnell's expertise, skil

Shell shock in Ireland: The Richmond War Hospital, Dublin (1916-19).

The history of mental disorders occasioned by World War I is a complex and important history, indelibly linked with social, political and cultural circumstances, and the history of the war itself. The Richmond War Hospital was a 32-bed establishment on the grounds of the large Richmond District Asylum in Dublin which, from 16 June 1916 until 23 December 1919, treated 362 soldiers with shell shock and other mental disorders, of whom more than half were considered to have recovered. Despite the limitations of the Richmond War Hospital, it was a generally forward-looking institution that pointed

Traumatic shock and electroshock: the difficult relationship between anatomic pathology and psychiatry in the early 20th century.

In the conviction that a look at the past can contribute to a better understanding of the present in the field of science too, we discuss here two aspects of the relationship between early 20th century anatomic pathology and psychiatry that have received very little attention, in Italy at least. There was much debate between these two disciplines throughout the 19th century, which began to lose momentum in the early years of the 20th, with the arrival on the scene of schizophrenia (a disease histologically sine materia) in all its epidemiological relevance. The First World War also contributed
