Movement of finger joints induced by synergistic wrist motion.

BACKGROUND: An experiment has recently been conducted to evaluate and compare the differences in tendon excursions between the flexor digitorum profundus and superficialis using three mobilization techniques. No previous studies deal with the total joint excursions with constant tendon length. The purpose of this study was to investigate the coordinated motion between the finger and wrist joints resulting from passive tension of the muscles while performing synergistic wrist motion. METHODS: The relative joint positions of the hand and wrist were measured using a three-dimensional motion anal

[Changes in the wrist joint in spontaneous course of scaphoid pseudarthrosis].

Scaphoid non-union always leads to carpal arthrosis. What is the natural history and course and sequence of development of arthrosis in scaphoid bone pseudarthrosis and what factors play a role in this? Forty-nine patients with scaphoid non-union were followed up; the period of observation was between three and 39 years. For comparison, 55 patients in whom a Matti-Russe reconstruction had been carried out successfully in the same period were followed up. Both the clinical and the radiological appearance seemed to be better in the operated patients than in the unoperated patients. The time fact

Denervation of the wrist joint--an alternative in conditions of chronic pain.

Nine patients with chronic pain in the wrist secondary to lunatomalacia and non-union of the scaphoid bone were treated by denervation of the wrist joint. This operation was first presented by Wilhelm in 1966. We have been using a modified technique. The purpose of the operation is to achieve relief of pain by selective neurotomy without impairment of hand function and with preservation of wrist mobility. Eight patients were considerably improved; one was not helped at all. No appreciable progression of the pathological changes have occurred radiographically during the period of follow-up. We

Surgical Feasibility Study on Cadaver for Vascularized Wrist Joint Transplantation.

PURPOSE: Despite modern advancements in the treatment of late stages of wrist joint degeneration, few reliable options exist for patients requiring motion-preserving reconstruction of their radiocarpal and midcarpal joints. Vascularized composite allotransplantation (VCA) could be considered an option for wrist reconstruction in the future. The goal of this study was to describe the relevant anatomy and design a potential surgical technique for wrist VCA. METHODS: Anatomic studies were performed on 17 human upper extremities. The arterial system of each cadaver was injected with latex dye or

Wearable Kinesthetic I/O Device for Sharing Wrist Joint Stiffness.

In this paper, we developed a wearable kinesthetic I/O system, which is able to share wrist joint stiffness by measuring and intervening in four muscle activities on the forearm simultaneously through the same electrodes. This achieves interactive peg rehabilitation by sharing muscle activities among patients and therapists. Through the performance study, it was shown that 1) applied EMS and measured wrist joint stiffness, and 2) produced wrist joint stiffness and measured EMG value has a linear correlation, which allows designing a mapping function between the measured EMG value on one person

Wrist joint involvement in systemic lupus erythematosus. An ultrasonographic study.

OBJECTIVE: To define joint alterations in the wrists of patients with systemic lupus erythematosus (SLE) by ultrasonography (US). METHODS: Fifty-two wrists of 26 SLE patients and 30 wrists of 15 healthy controls were evaluated using US by two different experienced operators, blinded to the clinical data. A 14 MHz linear probe was used. Power Doppler (PD) was applied to evaluate the presence of synovial neoangiogenesis as a parameter of active local synovitis. The findings were correlated to the clinical evaluation, serological systemic disease activity parameters (ESR, C3 levels) and the SLE-

Effects of distal radius fracture malunion on wrist joint mechanics.

An experimental model using a static positioning frame, pressure-sensitive film (Fuji), and a microcomputer-based videodigitizing system was used to measure contact areas and pressures in the wrist. Contact areas and pressures were compared in a group of wrists between the normal state and with simulated distal radius fracture malunions of varying degrees. In simulated malunions, radial shortening to any degree slightly increased the total contact area in the lunate fossa, and was significant at 2 mm of shortening. By angulating the distal radius more than 20 degrees either palmar or dorsal, t

Combined wrist arthrodesis and metacarpophalangeal joint arthroplasty in rheumatoid arthritis.

This paper describes a surgical procedure which combines wrist arthrodesis and metacarpophalangeal joint arthroplasty into one operation. Its basic indication is the severely crippled rheumatoid patient who is faced with multiple reconstructive surgical procedures. In addition to the combined wrist fusion and metacarpophalangeal joint arthroplasty, other surgical procedures can sometimes be performed into same hand, thereby completing the entire reconstruction in one operation. Of the 19 patients who have undergone the combined procedure, only one failed to obtain a solid clinical arthrodesis.

Solitary Bone Plasmacytoma of Left Wrist Joint - A Case Report.

INTRODUCTION: Solitary Plasmacytoma is an infrequent form of plasma cell (PC) dyscrasia which presents as a single mass of monoclonal PCs, located either intraosseous or extramedullary. Solitary bone plasmacytoma (SBP) is characterized by a mass of neoplastic monoclonal PCs primarily occurring in the axial skeleton without systemic involvement. This case delineated an unusual appendicular-osteoarticular (wrist joint) involvement, which was diagnosed through an exhaustive set of investigations and managed by radiotherapy (RT), which is the standard of care. The rarity of the clinico-radiologica

Modified wrist pivot method enabling straightforward temporomandibular joint dislocation reduction.

Temporomandibular joint (TMJ) dislocation reduction maneuvers can be difficult, even for experienced professionals, as muscle spasms and trismus can impede the process if not addressed promptly and effectively. This technical note presents a modification to the wrist pivot method, making the reduction of mandibular luxation more comfortable and efficient. Gravity assists the reduction, and if it fails, a fulcrum using the thumb wrapped in gauze is introduced, with the second lower molar as support. This technique was successfully tested in eleven cases, all reduced without sedation. The wrist

Failure of Universal 2 Wrist Joint Replacements: A Retrieval Study.

BACKGROUND: The Universal 2 total wrist arthroplasty was one of the most common wrist replacements, but long-term results were disappointing, due to substantial damage to the polyethylene component and, in some cases, metallosis. The purpose of this research was to investigate the underlying reasons for this polyethylene damage. MATERIALS AND METHODS: From a single clinical center, retrieval analysis was undertaken on six Universal 2 wrists and two additional polyethylene components. All components were analyzed at the macroscale and dimensional measurements of polyethylene components were un

Wrist Joint Skeletal Changes in Children With Transfusion-dependent Thalassemia.

BACKGROUND: Arthropathies and bone deformities are well known to occur in patients with thalassemia major and have been attributed to the disease or to its therapy. Before the advent of chelation therapy, these children developed widened diploic space and "hair-on-end" pattern in skull, "cobweb" pattern in the pelvis, and the lack of the normal concave outline in the long bones because of extensive marrow proliferation. After the introduction of iron-chelation therapy, these patients were noted to develop metaphyseal abnormalities and vertebral changes resembling spondylo-metaphyseal dysplasia

4D rotational x-ray imaging of wrist joint dynamic motion.

Current methods for imaging joint motion are limited to either two-dimensional (2D) video fluoroscopy, or to animated motions from a series of static three-dimensional (3D) images. 3D movement patterns can be detected from biplane fluoroscopy images matched with computed tomography images. This involves several x-ray modalities and sophisticated 2D to 3D matching for the complex wrist joint. We present a method for the acquisition of dynamic 3D images of a moving joint. In our method a 3D-rotational x-ray (3D-RX) system is used to image a cyclically moving joint. The cyclic motion is synchroni

Signs of muscle thixotropy during human ballistic wrist joint movements.

A study was conducted on healthy subjects to determine whether voluntary ballistic wrist flexion movements are influenced by immediately preceding conditioning of the forearm muscles. Single rapid wrist flexion movements were made in response to an auditory "Go" signal. Rectified surface EMG was recorded from wrist flexors and extensors, and joint position was measured by a goniometer. The movements were preceded (2-3 s) by four different conditioning routines: 40-s rest (Rest), 10-s voluntary alternating wrist joint flexion and extension movements (Osc), and 10 s of 25 degrees weak isometric

[Ten years' experience with our method of wrist joint arthrodesis].

PURPOSE OF THE STUDY: Total arthrodesis of the carpal joint is a complex operative procedure that results in the restoration of joint stability in frontal and sagittal planes and improvement in function of the tendons of digits' extensors and flexors. We developed our own method based on the use of a special implant; in this study we report our long-term results. MATERIAL: Since 1992 we have carried out 54 total carpal arthrodeses, using our method, on 51 patients; three patients were treated bilaterally. Both wrists were treated in. This group comprised 34 women and 19 men, with the average

Wrist injuries part 1: pinpointing pathology in a complex joint.

The wrist is a complex joint and a common site for injury in virtually every type of recreational activity. This article describes a workup that will lay the groundwork for a diagnosis more specific than wrist sprain or strain. Instructions are given for taking a detailed history, palpating the five zones of the wrist, performing provocative maneuvers, giving diagnostic injections, and obtaining imaging studies. Plain x-rays of both wrists may be sufficient to diagnose a fracture or subluxation, but if x-rays are negative and a fracture is still suspected, other imaging studies may be needed.

[Fresh fractures! Classification, management, results and problems: the wrist joint].

Carpal bone fractures are rare, with an incidence of 13% compared to that of fractures and luxations of the hand bones. Generally they are intraarticular fractures. Fracture of the scaphoid bone occurs most frequently, with an incidence of 75%. The greater number of fractures is found in the middle-third of the wrist bordering on the os lunatum. They are usually treated conservatively by splinting with a cast. Surgery is indicated only when the fracture is open, instable, dislocated or combined with carpal instabilities.
