Endoscopic management of a bleeding Zenker's diverticulum: A case report.

INTRODUCTION: Bleeding from a Zenker's diverticulum is a rare occurrence, and currently no formal guidelines exist for its management. It is a potentially life-threatening condition and an important consideration in the differential diagnosis of haemoptysis or haematemesis. PRESENTATION OF CASE: A 63-year-old man presented with haemoptysis and haematemesis while on dual antiplatelet therapy for a recent NSTEMI. After resuscitation he underwent a CT angiogram which demonstrated an active contrast blush and pooling of contrast in a pharyngeal diverticulum. At endoscopy a large Zenker's divertic

Zenker's diverticulum: A case study of dysphagia in an elderly gentleman.

Zenker's diverticulum (ZD) is an acquired, false, and pulsion diverticulum formed by the protrusion of mucosa and submucosa through Killian's dehiscence. The index case is a 91-year-old Nepali patient with a known history of diabetes mellitus, hypertension, rheumatic heart disease, and stage 4 chronic kidney disease, who has been experiencing dysphagia, halitosis, and chronic cough for 25 years. A computed tomography (CT) scan of the chest revealed an outpouching filled with air, fluid, and some solid contents, arising at the level of the cricoid cartilage and extending posteriorly to the esop

Zenker's diverticulum misinterpreted as a thyroid mass: Case report.

We discuss an unusual presentation of Zenker's Diverticulum (ZD). A 76-year-old man presented with a left sided neck mass which was misdiagnosed as a thyroid mass due to the anatomical location and size. The ultrasound and fine needle-aspiration cytology findings were inconclusive, and a CT scan was then considered which reported a large pharyngeal pouch. Our recommendation is to consider an early CT scan in patient's where there is a clinical suspicion or risk factors for the development of pharyngeal pouch specially when the fine-needle aspiration cytology findings are inconclusive. This wou

Efficacy and Safety of Flexible Endoscopic Management of Zenker's Diverticulum.

Zenker's diverticulum (ZD), a pulsion diverticulum of hypopharynx is a rare but treatable cause of morbidity in geriatric population. Traditionally a surgical disease but due to its associated high morbidity, flexible endoscopy has become a lucrative option. We reviewed 997 patients from 23 original studies who underwent flexible endoscopic diverticulotomy (FED) of ZD. Composite technical and clinical success rate for the study cohort was 99.4% and 87.9%, respectively. Composite failure rate was 10.0% but close to half of them (45.3%) had success with repeat endoscopic intervention. Composite

Zenker's diverticulum, a rare cause of upper gastrointestinal bleeding.

UNLABELLED: The most common complication of Zenker's diverticulum is aspiration pneumonia, compression of the trachea and esophageal obstruction with large diverticulum, and increased risk of development of carcinoma. Thus bleeding occurs rarely, can be massive and life threatening, with ulceration being the most common cause. MATERIAL AND METHODS: We describe a patient with sever upper gastrointestinal bleeding as a result of a Zenker's diverticulum. A 75 year-old woman was referred to the emergency room and hospitalized for hematemesis, melena, asthenia and total dysphagia. In this particul

Zenker's diverticulum complicating achalasia: a 'cup-and-spill' oesophagus.

A 72-year-old woman presented with long-standing gastro-oesophageal reflux, regurgitation of swallowed food and worsening cervical dysphagia. Fluoroscopic barium oesophagography revealed a posterolateral pharyngeal pouch (Zenker's diverticulum (ZD)) complicating a 'cup and spill' oesophageal deformity with a smoothly tapered segment at the gastro-oesophageal junction. CT and high-resolution manometry confirmed that the underlying abnormality was a massively dilated oesophagus with aperistalsis and pan-oesophageal pressurisation, consistent with a diagnosis of oesophageal achalasia (type II). Endoscopic treatment of Zenker's diverticulum by carbon dioxide laser.

The management of Zenker's diverticulum remains controversial, as both external and endoscopic procedures are potentially associated with post-operative complications and risks. The endoscopic approach is based on cricopharyngeal myotomy or myectomy by laser, cautery or stapling. A retrospective chart review has been made from December 1994 to April 2009 of all patients with Zenker's diverticulum treated by endoscopic cricopharyngeal myectomy using CO(2) laser at the Department of Otorhinolaryngology - Head and Neck Surgery of the University of Brescia, Italy. Of the 28 patients (19 males, 9 f

[Extraesophageal reflux in patients suffering from Zenker's diverticulum].

INTRODUCTION: Patients with Zenker's diverticulum often present with a hiatal hernia. Theoretically, the gastric acid fluid could rise up to the hypopharynx producing an injury to the mucosa of Killian's triangle and hypertrophy of the cricopharyngeal muscle. We performed dual-channel pH monitoring in healthy people as a control group and in patients with a Zenker's diverticulum to elucidate the relationship between Zenker's diverticulum and gastroesophageal reflux disease (GERD) as well as extraesophageal reflux (EER). METHODS: Patients with Zenker's diverticulum underwent dual-channel 24-h

[Zenker's diverticulum. The endoscopic-microscopic, staple-assisted esophagodiverticulostomy].

BACKGROUND: The endoscopic-microscopic staple-assisted esophagodivertikulostomy of Zenker's diverticulum is a new modification of endoscopic operation techniques which minimizes complications and is able to speed up the patient's convalescence. PATIENTS AND METHODS: A total of 22 out of 32 patients with Zenker's diverticulum underwent surgery. The operation involves cutting the wall between the diverticular sac and the oesophagus and, at the same time, closing the mucosal wound edges using the "closed" surgery method. RESULTS: All operations were effective and uncomplicated. A total of 28 op

Botulinum toxin type A (Dysport) for the treatment of Zenker's diverticulum.

The standard treatment for Zenker's diverticulum (ZD) is surgical, but transendoscopic section of the diverticular spur has also been performed with good results. We treated two patients with the injection of 300-350 IU of botulinum toxin type A (Dysport) into the diverticular spur. Both patients were dysphagic because of a large ZD. A few hours after the treatment, the patients were able to return to a normal, nonrestricted diet. They remain symptom-free 12 and 3 months later, respectively. No complications have been observed. As a result of its ease of application and efficacy, this treatmen

[Dysphagia caused by Zenker's diverticulum after total laryngectomy].

Zenker's diverticulum is a mucosal lined outpouching of pharynx through Lainert's space that causes dysphagia of the upper digestive tract. Multiples theories try to explain the acquired etiology of this entity, attributing its origin to a disfunction of pharynx-esophageal sphincter. A case of total larynguectomy with hypopharyngeal diverticulum and progressive dysphagia to solid food is presented. We analyze the etiopathogenic mechanisms and the definitive characteristics of this entity. We review mundial literature, being exceptional the fact of finding clinical manifestations in diverticulu

[Microsurgical laser treatment of Zenker's diverticulum. Economic aspects].

Zenker's diverticulum (hypopharyngeal/proximal oesophageal diverticulum/pouch) is a relatively uncommon cause of dysphagia usually in elderly patients. We describe the results of the first 10 patients operated for ZD with micro-endoscopic laserdiverticulotomy (LD), where the "spur" between the diverticulum and oesophagus is coagulated by means of a CO2 laser in our department. The results are compared with the results of the last nine patients operated with conventional diverticulectomy (DE) via incision on the neck. Two patients in the DE group had complications (wound infection and pneumonia

Where's the ulcer? Spontaneous bleeding from Zenker's diverticulum.

An 89-year-old man with a history of coronary artery disease status post coronary artery bypass grafting and atrial fibrillation on aspirin and warfarin, presented to the emergency department with a symptom of haemoptysis. He reported a history of dysphagia for 3 months prior to presentation, but had yet to seek medical attention. On presentation, he was hypotensive and tachycardic with a haemoglobin of 6.6 g/dL and an International Normalised Ratio (INR) of 3.9. Esophagogastroduodenoscopy was performed and showed active bleeding from Zenker's diverticulum. The patient's INR was reversed with

Iatrogenic Zenker's diverticulum perforation: A conservatively treated case.

Zenker's diverticulum is the most common type of diverticulum in the esophagus. Most patients are elderly. Typical symptoms are regurgitation, dysphagia, halitosis and chronic cough. The most common treatments are open surgical diverticulectomy with cricopharyngeal myotomy or diverticulopexy. Most instances of esophageal perforation are iatrogenic. Rapid diagnosis and therapy provide the best chance for survival. If diagnosed early, cervical esophageal perforations can be treated conservatively if there are no signs of systemic sepsis. In this case report a conservatively treated cervical esop

[Zenker's diverticulum--surgical management in a series of six cases].

Zenker's diverticulum is a protrusion of the posterior mucosal wall of the hypopharynx through the weakened muscular layer, between the oblique fibers of the inferior constrictor of the pharynx and the transverse fibers of the cricopharyngeal muscle (Killian's dehiscence). Pharyngoesophageal pulsion diverticulum is the most common of all the oesophageal diverticuli and is characterised by dysphagia, regurgitation, gurling sounds in the neck and aspiration. The current principles of surgical management includes a pharyngoesophageal myotomy accompanied by a diverticulectomy or a diverticulopexy.

Endoscopic treatment of Zenker's diverticulum using CO2 laser.

OBJECTIVES: The objective of the present study was to review the technique of endoscopic laser-assisted esophagodiverticulostomy (ELAED) for the treatment of Zenker's diverticulum. METHODS: We reviewed 83 cases of ELAED performed for the treatment of Zenker's diverticuli during the past decade. RESULTS: ELAED-treated patients had a reduced hospital stay and were able to start an oral diet earlier than were patients who underwent other surgical procedures. Two (2.4%) of our patients developed small fistulas, which closed spontaneously. An additional 2 (2.4%) of our patients required conversio

[Zenker's diverticulum in the elderly. Description of a case and surgical treatment].

The authors report a case of Zenker's giant hypopharyngeal diverticulum in an elderly patient who underwent surgery due to the severity of symptoms. This diverticulum, which is both juxtasphincteric and epiphrenal, has a pulsion pathogenesis: the presence of a hernia on the esophageal side (jato?), with which Zenker's diverticulum is frequently associated and which is often followed by reflux esophagitis, is enough to cause motor asynchronism of the crico-pharyngeal muscle which, in the presence of hypertonic conditions during deglutition, leads to the formation of a high-pressure pouch which

Management of benign fistulae between Zenker's diverticulum and the trachea.

Cases of acquired benign fistula of the respiratory tree and esophagus have been reported in relation to Mycobacterium tuberculosis and trauma. Fistula formation between the trachea and a Zenker's diverticulum is a similar type of condition in which nutritional and immunological deficiencies can rapidly result in further complications which may mimic a malignancy. Spontaneous fistula formation and vocal cord paralysis may result. Surgical management of this condition includes interposing viable muscle between the esophagus and trachea at the repair site.
