Approach to urinary diversion in the surgical patient.

Major surgical procedures may remove part or all of the bladder and make an incontinent or continent urinary diversion appropriate. Preoperative consideration must be given to 1) the stoma and its position, 2) the catheterizable channel, 3) the urinary continence mechanism, and 4) the substitute bladder reservoir. Complete bowel preparation and broad-spectrum antibiotics are desirable. The patient's motivation for taking care of a continent urinary diversion is important, since lifelong catheterization and mucous irrigation may be necessary. The status of the native bladder outlet and urinary

Stent versus no stent in ureteroneocystostomy in live-related renal transplant: A randomized controlled trial.

BACKGROUND: Ureteric stent decreases urological complications postrenal transplantation, but increases the risk of urinary tract infection (UTI). There is no consensus for the placement of a routine ureteric stent following a living donor kidney transplant in adults. METHODS: Between January 2018 and October 2019, 100 living donor renal transplant recipients were randomized to a stented or nonstented group. Incidence of UTIs, hematuria, pyuria or asymptomatic bacteriuria and urological complications were compared over a follow-up period of 3 months. RESULTS: A total of 93 patients were consi

The utilization of a spare ureter to create a continent catheterizable channel to the bladder in pediatric patients.

INTRODUCTION: Clean intermittent catheterization (CIC) has significant advantages over indwelling catheters. To facilitate CIC, a continent catheterizable channel (CCC) to the bladder is required in some cases. The Mitrofanoff appendicovesicostomy (APV) is considered the gold standard for pediatric CCC creation. However, when the appendix is unavailable or unsuitable for the creation of a CCC alternatives are required. OBJECTIVE: This study aims to share our single-center experience with using a spare ureter as a CCC in pediatric patients and compare its advantages and complications to those

Utilizing an Umbilical Ligament for Complex Ureteroneocystostomy.

INTRODUCTION: To overcome the tension of anastomosis between ureter and bladder for complex ureteroneocystostomy in children, we describe a novel technique utilizing an ipsilateral umbilical ligament (occluded umbilical artery) to fix the bladder. TECHNICAL CONSIDERATIONS: From July 1991 to December 2013, 18 patients (13 girls, 5 boys) underwent our technique for complex ureteroneocystostomy. Median age at surgery was 61 months. The main indications for surgery were ectopic ureter in 9 patients, primary obstructive megaureter in 3, and persistent vesicoureteral reflux after surgery in 2. The

Updated experience with the Monti catheterizable channel.

OBJECTIVES: The Monti catheterizable channel is used as an integral part of continent bladder reconstruction in children. We have updated our ongoing experience at Riley Children's Hospital with 199 patients. METHODS: We identified 199 patients for retrospective review, including all patients for whom a Monti ileovesicostomy was created from January 1997 to August 2004. We assessed the complications, surgical procedures, and stomal continence. RESULTS: At mean follow-up of 28 months, we found that 194 of 199 patients (97.5%) continued to use their Monti catheterizable channel for bladder dra

Insertion, care and management of suprapubic catheters.

A suprapubic catheter may be used to drain the bladder of urine as an alternative to an indwelling urethral catheter. It can be inserted as a temporary or permanent measure in patients with urinary dysfunction or if initial urethral catheter insertion or recatheterisation is problematic. This article discusses insertion of suprapubic catheters, management and care, including changing suprapubic catheters, drainage systems and the importance of patient education.

Rectus sheath tunnels for continent stomas.

Continent stoma rectus sheath tunnel (CSRST) has been used in antegrade colonic enema (ACE) and urinary continent cutaneous diversion (UCCD) stomas to reduce leakage and to support a straight track for the continent conduit. All patients that underwent CSRST between 1995 and 2005 were identified and their case notes retrospectively reviewed. Patients were divided into two groups: the ACE group and the UCCD group. Demographic data, age at surgery and complications including stenosis and leakage were recorded. Forty patients underwent CSRST between 1995 and 2005. The mean age and standard deviat

Laparoscopic Mitrofanoff appendicovesicostomy.

A novel technique of performing fully laparoscopic Mitrofanoff appendicovesicostomy in the management of neurogenic bladder is described. All steps, including appendix harvesting and extravesical appendiceal-vesical anastomosis with flap-valve mechanism creation, were completed laparoscopically and provided satisfactory preliminary clinical results. Additional studies are necessary for the evaluation of this novel technique.

Long-term followup and outcome of continent catheterizable vesicocstomy using the Rink modification.

PURPOSE: Multiple techniques have been described to create a Mitrofanoff channel in the pediatric population. A small subset of patients only requires creation of a catheterizable channel without bladder augmentation. These patients are ideal candidates for a procedure that avoids the use of intestine, especially in the absence of a suitable appendix. We used a modification of the Casale vesicostomy, as described by Rink, to create a continent vesicostomy in these children. We report our long-term experience with this technique. MATERIALS AND METHODS: We retrospectively reviewed the medical r

Primary vesicoureteric reflux treated by antireflux ureterocystostomy at the vertex of the bladder. A 12-year follow-up and analysis of operative failure.

A total of 618 children with primary vesicoureteric reflux (VUR) were examined preoperatively and 6, 9 and 24 months after an antireflux ureterocystostomy of the vertex of the bladder (AUVB); 206 children were operated on bilaterally, so that 824 AUVBs were performed. Most patients had grade 3 or 4 reflux and only 5% had grade 2 reflux. Successful results were noted in 792 ureteric units; in 10 cases of post-operative stenosis and 17 cases of recurrent VUR a second AUVB was performed. The following causes of operative failure were found: obstruction of the submucosal ureter after tailoring (5

[Surgical reconstruction of the ureter].

Defects in ureteral continuity and function can originate from various etiologies such as stricture, radiotherapy, tuberculosis, tumor, trauma or perforation due to iatrogenic injury. The surgical options for the management of ureteral defects are complex and depend on the location of the defect. The aim of the surgical management of ureteral stricture is the reconstruction of an anti-refluxive and nonobstructive flow of urine to preserve kidney function. There are numerous possibilities for the reconstruction of ureteral defects ranging from ureteroneocystostomy with or without psoas-hitch- o
