The impact of size on middle-ear sound transmission in elephants, the largest terrestrial mammal.

Elephants have a unique auditory system that is larger than any other terrestrial mammal. To quantify the impact of larger middle ear (ME) structures, we measured 3D ossicular motion and ME sound transmission in cadaveric temporal bones from both African and Asian elephants in response to air-conducted (AC) tonal pressure stimuli presented in the ear canal (PEC). Results were compared to similar measurements in humans. Velocities of the umbo (VU) and stapes (VST) were measured using a 3D laser Doppler vibrometer in the 7-13,000 Hz frequency range, stapes velocity serving as a measure of energy

Assessment of middle ear structure and function with optical coherence tomography.

BACKGROUND: Current clinical tests for middle ear (ME) injuries and related conductive hearing loss (CHL) are lengthy and costly, lacking the ability to noninvasively evaluate both structure and function in real time. Optical coherence tomography (OCT) provides both, but its application to the audiological clinic is currently limited. OBJECTIVE: Adapt and use a commercial Spectral-Domain OCT (SD-OCT) to evaluate anatomy and sound-evoked vibrations of the tympanic membrane (TM) and ossicles in the human ME. MATERIALS AND METHODS: SD-OCT was used to capture high-resolution three-dimensional (3

Removal and Repositioning of a Piston Wire Prosthesis That Entered the Vestibule Secondary to Trauma in a Patient Who Underwent Stapedotomy.

Otosclerosis is a common cause of adult-onset hearing impairment, and stapedotomy is often performed as surgical treatment. Several studies have reported the complications of stapedotomy surgery; piston wire prosthesis (PWP) disruption or dislocation secondary to indirect force attributable to head trauma is described in many patients. Most PWPs that get displaced are slanted or are completely dislodged from the stapedotomy site and lodged within the middle ear. PWP dislocation into the vestibule is extremely rare. A 65-year-old woman who was involved in a traffic accident underwent computed t

Central versus marginal tympanic membrane perforations: does it matter? An analysis of 792 patients.

BACKGROUND: There is a lack of studies concerning chronic otitis media without cholesteatoma. OBJECTIVES: To perform an analysis of tympanic membrane perforations (TMP), compare the parameters of central and marginal TMP, combining both the traditional and more recent technologies available. MATERIAL AND METHODS: 792 consecutive patients. The TMP subgroups were divided by central and marginal locations and compared based on signs suggestive of previous tympanic retraction, namely, medialized malleus, tympanic remnants over the promontory, tympanic remnants over the ossicular chain, and incus

Ethanol infiltration into the stapedio-vestibular joint reduces low-frequency vibration of the ossicular chain and round window membrane in the guinea pig.

BACKGROUND: The synovial stapedio-vestibular joint (SVJ), which serves as a bridge between the stape and oval window, can be found in guinea pigs and most human adults. Unlike the fibrous SVJs in other animals, the contribution of the synovial SVJ to middle ear sound transmission remains unknown. AIMS/OBJECTIVES: In this study, we investigate whether sclerosis of the synovial SVJ contributes to frequency-dependent vibration of the ossicular chain and round window membrane (RWM). MATERIALS AND METHODS: A model of SVJ sclerosis model was established in the guinea pig using 75% ethanol. A laser

Numerical evaluation of implantable hearing devices using a finite element model of human ear considering viscoelastic properties.

Finite element method was employed in this study to analyze the change in performance of implantable hearing devices due to the consideration of soft tissues' viscoelasticity. An integrated finite element model of human ear including the external ear, middle ear and inner ear was first developed via reverse engineering and analyzed by acoustic-structure-fluid coupling. Viscoelastic properties of soft tissues in the middle ear were taken into consideration in this model. The model-derived dynamic responses including middle ear and cochlea functions showed a better agreement with experimental da

Measles virus prevalence in otosclerotic foci.

The etiology of otosclerosis is still unknown. Persistent measles virus infection of the otic capsule is supposed to be one of the etiologic factors in otosclerosis. The presence of measles virus was shown in otosclerotic patients by RT-PCR amplification of the viral RNA, detecting the viral proteins by immunohistochemistry and antimeasles immunoglobulin G in the perilymph samples. Nucleic acid (mRNA, vRNA, DNA) was extracted from pulverized, frozen stapes footplate samples of otosclerotic patients. Measles virus RNA was amplified by RT-PCR: reverse transcription and the first-round PCR amplif

Neuroma of the facial nerve masked by chronic otitis media.

The absence of facial twitching, weakness, or palsy makes the diagnosis of facial neuroma difficult. The authors report a case of neuroma of the horizontal portion of the facial nerve masked by the presence of a chronic ear. A woman with a long history of discharge and hypoacousia in her left ear presented with acute dizziness. Examination revealed grade 3 horizontal right nystagmus, left anacousia, and the appearance of an epitympanic cholesteatoma. Computed tomography (CT) was performed after the vestibular condition improved. The clinical diagnosis of chronic otitis media with cholesteatoma
