Race is an independent predictor of survival in patients with soft tissue sarcoma of the extremities.

BACKGROUND: In the United States, race and socioeconomic status are well known predictors of adverse outcomes in several different cancers. Existing evidence suggests that race and socioeconomic status may impact survival in soft tissue sarcoma (STS). We investigated the National Cancer Database (NCDB), which contains several socioeconomic and medical variables and contains the largest sarcoma patient registry to date. Our goal was to determine the impact of race, ethnicity and socioeconomic status on patient survival in patients with soft tissue sarcoma of the extremities (STS-E). METHODS: W

Changes in cancer diagnoses and stage distribution during the first year of the COVID-19 pandemic in the USA: a cross-sectional nationwide assessment.

BACKGROUND: The emergence of COVID-19 disrupted health care, with consequences for cancer diagnoses and outcomes, especially for early stage diagnoses, which generally have favourable prognoses. We aimed to examine nationwide changes in adult cancer diagnoses and stage distribution during the first year of the COVID-19 pandemic by cancer type and key sociodemographic factors in the USA. METHODS: In this cross-sectional study, adults (aged ≥18 years) newly diagnosed with a first primary malignant cancer between Jan 1, 2018, and Dec 31, 2020, were identified from the US National Cancer Database

Medically underserved girls receive less evaluation for short stature.

OBJECTIVE: To determine if gender is associated with diagnostic evaluation by primary care pediatricians caring for children with growth-faltering. PATIENTS AND METHODS: This was a retrospective study of children who were attending 4 urban pediatric primary care practices affiliated with a tertiary pediatric hospital. Growth-faltering was defined as height at the <5th percentile or a z-score decrease of ≥ 1.5 SDs before 18 months of age or ≥ 1 SD thereafter. For each child, height z score, age, gender, race, insurance, diagnostic tests, and subspecialist appointments were examined. RESULTS:

Non-operative management of rectal adenocarcinoma in a safety-net system: a decade of real-world outcomes outside watch-and-wait eligibility.

Recent guidelines increasingly support watch-and-wait (W&W) strategies for locally advanced rectal cancer (LARC) following favorable response to neoadjuvant therapy. However, these strategies have largely been validated in highly selected trial populations and require intensive surveillance that may be difficult to implement in underserved settings. Limited data exist on outcomes among patients affected by adverse social determinants of health, particularly in safety-net hospital systems. We evaluated outcomes of patients with LARC in an underserved setting who were managed non-operatively out

Recognition of worsening heart failure symptoms, help-seeking attitudes, and time-to-presentation in chronic heart failure: a single-center KAP study in China.

BACKGROUND: Timely recognition and prompt help-seeking for worsening heart failure (HF) mitigate deterioration, yet patient-side delays remain common in China. This study evaluated how knowledge, attitudes, and practices (KAP) relate to time-to-presentation (TTP). METHODS: In a single-center cross-sectional study (March 2022-April 2025), adults with chronic HF completed a standardized KAP instrument; clinical data were abstracted. Among those with a worsening episode in the prior six months, TTP was categorized (≤6, 7-12, 13-24, 25-48, >48 h). Prespecified analyses comprised logistic regressi

Landscape of Emergency Medicaid and Health Care Coverage for Undocumented Immigrants in the US.

IMPORTANCE: Undocumented immigrants represent a disproportionate share of uninsured individuals in the US, owing to federal restrictions that limit access to affordable health care coverage. Emergency Medicaid (EM) and other state-sponsored programs offer mechanisms for receiving necessary medical care, but their scope and provisions are poorly understood by patients, clinicians, and policymakers alike. OBJECTIVE: To examine the national policy landscape of EM and other state programs designed to provide health care coverage for undocumented immigrants. EVIDENCE REVIEW: Between January 2024

Trimethoprim-sulfamethoxazole or clindamycin for community-associated MRSA (CA-MRSA) skin infections.

BACKGROUND: In the United States, community-associated methicillin-resistant Staphylococcus aureus (CA-MRSA) has emerged as the predominant cause of skin infections. Trimethoprim-sulfamethoxazole (TMP-SMX) and clindamycin are often used as first-line treatment options, but clinical data are lacking. METHODS: We conducted a retrospective cohort study of outpatients with skin and soft tissue infections managed from July 1 to December 31, 2006. Patients younger than 18 years of age were excluded, as were those who had no clinical admission or progress notes; were hospitalized within the 90 days

Major review: The underutilization of vision screening (for amblyopia, optical anomalies and strabismus) among preschool age children.

BACKGROUND: Nearly 80% of preschool age children never get an eye examination (1). Many "back to school" physical exams do not test for common vision disorders. Untreated eye and vision problems can interfere with most life experiences. The prevalence of undetected vision problems among preschool age children is estimated to be 5% to 10% (2). Failure to detect visual impairment early may have a permanent effect on long term vision outcomes, education achievement, and self esteem (3). The most common vision disorders among children are strabismus, amblyopia and optical problems impairing visual

Hepatitis C Care Cascade in Opioid-Dependent Emergency Department Patients.

INTRODUCTION: Emergency departments (ED) have become important venues for hepatitis C (HCV) screening. Similarly, EDs have had increasing engagement in the management and treatment of opioid use disorder (OUD). Persons with OUD are at disproportionate risk for acquiring HCV. We sought to assess the intersection between OUD and HCV care through observing the uptake of HCV screening and subsequent steps in the HCV care cascade in an ED-OUD cohort. METHODS: This retrospective cohort study analyzed patients enrolled in an ED-OUD treatment program as a convenience sample subgroup of a larger insti

Trends in Spending on Corrective Lenses.

IMPORTANCE: About 50% of the population has a refractive error, but corrective lenses can impose cost barriers on patients given the lack of widespread vision insurance. OBJECTIVE: To describe temporal trends and sociodemographic variation in corrective lens spending and payment source (third party vs out-of-pocket). DESIGN, SETTING, AND PARTICIPANTS: This survey-based study used data from respondents to the 2010 through 2023 Medical Expenditure Panel Survey (N = 155 928). Annual response rates varied between 26.1% to 53.5%. These data were analyzed from March 2025 to July 2026. EXPOSURES:

Social Determinants of Health and ESKD among Survivors of Critical Illness and Acute Kidney Injury.

KEY POINTS: During follow-up, 326 (1%) critical illness survivors with AKI developed ESKD; 2723 (9%) died. Black race and lack of health insurance were independently associated with higher risk of developing ESKD. Residence in a socioeconomically deprived neighborhood was independently associated with higher risk of ESKD. BACKGROUND: Adverse social determinants of health (SDOHs) are associated with higher risk of AKI. This study evaluated the association of individual- and neighborhood-level SDOHs with incident ESKD among intensive care unit (ICU) survivors of AKI. METHODS: Thirty thousand f

Policy and Payment Factors Influencing Adoption of Low-Barrier Buprenorphine Treatment.

IMPORTANCE: Treatment of opioid use disorder (OUD) with buprenorphine reduces overdose and all-cause mortality, yet access and retention remain limited. Much of the literature describing barriers to buprenorphine access and retention has focused on practitioner-level or patient-level barriers, but less is known about how potential policy and payment levers may influence prescribing behaviors and treatment practices from the practitioner perspective. OBJECTIVE: To explore perspectives among OUD treatment program clinicians and staff about how broader policy and payment structures influence ado

Gender-Affirming Hormone Therapy and Depressive Symptoms Among Transgender Adults.

IMPORTANCE: In the US, transgender, nonbinary, and gender diverse (TGD) adults have high rates of depression. Gender-affirming hormone therapy (GAHT) is associated with improved mental health outcomes, yet existing US studies have short follow ups and lack sample diversity. OBJECTIVE: To evaluate the use of GAHT delivered in primary care as an intervention for moderate-to-severe depressive symptoms in diverse TGD adult patients. DESIGN, SETTING, AND PARTICIPANTS: LEGACY was an observational cohort study conducted in federally qualified community health centers in Boston and New York that fol

Optimizing hypertension management in China: care cascade insights and individual determinants from a national survey.

OBJECTIVES: Hypertension is common in China, and its prevalence has been increasing. However, it remains inadequately controlled. This study examined patterns of patient attrition across the stages of hypertension management in the general population and their variations across subpopulations, and assessed the role of individual-level determinants of hypertension management. The findings would help inform clinical practice and policy decisions to enhance hypertension management in China. METHODS: Using data from the 2015 China Health and Nutrition Survey among adults aged ≥18 years, we applie

Association of federal poverty level with healthcare expenditures among opioids users in the United States (2008-2019): a serial cross-sectional study.

BACKGROUND: Opioid users across federal poverty levels have varying healthcare consumption, which could influence public health policies to address the opioid crisis. To better understand this relationship, we evaluated the associations between federal poverty level (FPL) with healthcare costs and utilizations among adult opioid users in the United States (US). METHODS: A serial cross-sectional study using pooled data (2008-2019) from the Medical Expenditure Panel Survey (MEPS) was used to evaluate the association between FPL with healthcare expenditures among a representative sample of the U

Impact of Primary Care Physician Interaction on Longitudinal Adherence to Screening Mammography Across Different Racial/Ethnic Groups.

PURPOSE: Regular contact with a primary care physician (PCP) is associated with increased participation in screening mammography. Older studies suggested that PCP interaction may have a smaller effect on screening mammography uptake among racial and ethnic minorities compared with whites, but there is limited contemporary evidence about the effect of PCP interaction on screening mammography uptake across different racial and ethnic groups. The purpose of this study was to evaluate the association between PCP contact and longitudinal adherence with screening mammography guidelines over a 10-yea
