Narrative review of current context of malaria and management strategies in Uganda (Part I).

In accordance with international targets, the Uganda National Malaria Control Strategic Plan established specific targets to be achieved by 2010. For children under five, this included increasing the number of children sleeping under mosquito nets and those receiving a first-line antimalarial to 85%, and decreasing case fatality to 2%. This narrative review offers contextual information relevant to malaria management in Uganda since the advent of artemisinin combination therapy (ACT) as first-line antimalarial treatment in 2004. A comprehensive search using key words and phrases was conducted

Global report on preterm birth and stillbirth (3 of 7): evidence for effectiveness of interventions.

INTRODUCTION: Interventions directed toward mothers before and during pregnancy and childbirth may help reduce preterm births and stillbirths. Survival of preterm newborns may also be improved with interventions given during these times or soon after birth. This comprehensive review assesses existing interventions for low- and middle-income countries (LMICs). METHODS: Approximately 2,000 intervention studies were systematically evaluated through December 31, 2008. They addressed preterm birth or low birth weight; stillbirth or perinatal mortality; and management of preterm newborns. Out of 82

Safety and efficacy of repeat ivermectin mass drug administrations for malaria control (RIMDAMAL II): a phase 3, double-blind, placebo-controlled, cluster-randomised, parallel-group trial.

BACKGROUND: The success of crucial vector control efforts in Africa (eg, long-lasting insecticide-treated nets [ITNs] and indoor residual spraying) are threatened by widespread insecticide resistance and insufficient effect on outdoor mosquito biting. Studies have shown that ivermectin, used for the treatment of parasitic diseases, can kill malaria vectors that feed on the blood of treated people and thus might be an effective complementary vector control tool if administered widely to communities in malaria endemic regions. We aimed to test the safety of repeated, high-dose ivermectin mass dr

Oxygen blenders for prevention of retinopathy of prematurity are highly cost-effective in Uganda.

Blended oxygen has been shown to reduce severe retinopathy of prematurity (ROP) incidence worldwide, and more recently in Uganda. Although it has been established that ROP-driven visual impairment has a substantial societal burden and that screening and treatment programs are cost-effective in low- and middle-income countries, there is limited evidence for the economic impact of prevention with appropriate oxygen management equipment (OME) in these regions. We performed a cost-utility analysis of OME implementation comparing variations ranging between most and least favorable values. Disabilit

Prevalence of West Nile Virus in Children Being Evaluated for Lyme Disease.

BACKGROUND: West Nile virus (WNV) is a flavivirus typically transmitted between mosquitoes and birds with occasional spill-over into other animals, including humans. Children evaluated for suspected Lyme disease may share environmental risk factors for WNV and present with overlapping symptoms. Our goal was to determine the prevalence of WNV infection in children with suspected Lyme disease. METHODS: We performed a nested case-control study within a prospective cohort study of children with suspected Lyme disease aged 1-21 years enrolled at 1 of 8 Pedi Lyme Net emergency departments. Lyme dis

A survey of indoor and outdoor Anopheles density, species composition and circumsporozoite rate of malaria vectors on the Bijagós Archipelago, Guinea-Bissau.

BACKGROUND: The malaria-endemic Bijagós Archipelago is situated 50 km off the coast of mainland Guinea-Bissau. It is a seasonal malaria transmission setting, with insecticide-treated bed nets as its primary control strategy. Little is known about the vector diversity and behaviour across the Archipelago. METHODS: In 2019, a survey took place on 16 of the inhabited islands across the Archipelago. Adult mosquitoes were collected using odour-baited outdoor light traps and indoor light traps at houses selected at random. Larval surveys were conducted for each village sampled. Anopheles adults cau

A single mutation G454A in the P450 CYP9K1 drives pyrethroid resistance in the major malaria vector Anopheles funestus reducing bed net efficacy.

Metabolic mechanisms conferring pyrethroid resistance in malaria vectors are jeopardizing the effectiveness of insecticide-based interventions, and identification of their markers is a key requirement for robust resistance management. Here, using a field-lab-field approach, we demonstrated that a single mutation G454A in the P450 CYP9K1 is driving pyrethroid resistance in the major malaria vector Anopheles funestus in East and Central Africa. Drastic reduction in CYP9K1 diversity was observed in Ugandan samples collected in 2014, with the selection of a predominant haplotype (G454A mutation at

Evaluating the predictive performance of malaria antibodies and FCGR3B gene polymorphisms on Plasmodium falciparum infection outcome: a prospective cohort study.

BACKGROUND: Malaria antigen-specific antibodies and polymorphisms in host receptors involved in antibody functionality have been associated with different outcomes of Plasmodium falciparum infections. Thus, to identify key prospective malaria antigens for vaccine development, there is the need to evaluate the associations between malaria antibodies and antibody dependent host factors with more rigorous statistical methods. In this study, different statistical models were used to evaluate the predictive performance of malaria-specific antibodies and host gene polymorphisms on P. falciparum infe

Manipulating Solid Forms of Contact Insecticides for Infectious Disease Prevention.

Malaria control is under threat by the development of vector resistance to pyrethroids in long-lasting insecticidal nets, which has prompted calls for a return to the notorious crystalline contact insecticide DDT. A faster acting difluoro congener, DFDT, was developed in Germany during World War II, but in 1945 Allied inspectors dismissed its superior performance and reduced toxicity to mammals. It vanished from public health considerations. Herein, we report the discovery of amorphous and crystalline forms of DFDT and a mono-fluorinated chiral congener, MFDT. These solid forms were evaluated

Entomological surveillance of malaria vector: species, behaviors, and insecticide resistance in southern and central Benin.

BACKGROUND: Malaria transmission in Benin is shaped by ecological heterogeneity, vector biting behavior, and increasing insecticide resistance. However, integrated multi-season data combining vector behavior, infection, and resistance remain limited in southern and central Benin. METHODS: From December 2023 to October 2024, an entomological and molecular survey was conducted in six districts across southern and central Benin over dry and rainy season collection rounds. Adult mosquitoes were collected using human landing catches (HLC) and pyrethrum spray catches (PSC), identified morphological

Assessment of mesh shrinkage using fibroblast-populated collagen matrices: a proof of concept for in vitro hernia mesh testing.

PURPOSE: This study uses free-floating contractile fibroblast-populated collagen matrices (FPCMs) to test the shrinkage of different hernia mesh products. We hope to present this model as a proof of concept for the development of in vitro hernia mesh testing-a novel technology with interesting potential. METHODS: FPCMs were formed by seeding Human Dermal Fibroblasts into collagen gels. FPCMs were seeded with three different cell densities and cast at a volume of 500 μl into 24-well plates. Five different mesh products were embedded within the collagen constructs. Gels were left to float freel

Impact of field evolved resistance on biological parameters of non-targeted Aedes aegypti populations.

The yellow fever mosquito, Aedes aegypti L., known for transmitting viruses causing yellow fever, dengue, chikungunya, and Zika fever, presents a substantial risk to global human health. The development of insecticide resistance in disease vectors has become a significant problem in Ae. aegypti. Monitoring insecticide resistance is essential for resistance management in Ae. aegypti. This study involved the collection of Ae. aegypti populations from four important cotton-growing regions in southern Punjab, Pakistan, for resistance monitoring over a two-year period (2021-2022). This study also a

Malaria from hyperendemicity to elimination along international borders in Yunnan, China during 2003‒2020: a case study.

BACKGROUND: Border malaria is one of the most intractable problems hindering malaria elimination worldwide. Movement of both the human population and anopheline mosquitoes infected with Plasmodium spp. can cause cross-border malaria transmission. The Yunnan border area was still hyperendemic for malaria in the early part of this century. The objective of this case study was to analyze the strategies, interventions and impacts of malaria control and elimination in the Yunnan border area. MAIN TEXT: A total of 10,349 malaria cases and 17.1 per 10,000 person-years of annual parasite incidence (A

Dengue in parts of the Guinea Savannah region of Nigeria and the risk of increased transmission.

BACKGROUND: Dengue virus (DENV) is spreading to parts of the world where it had not been previously reported. Increased international travel has led to intercontinental importation of dengue by visitors returning from countries in sub-Saharan Africa. Although dengue is well documented in the rain forest region of Nigeria, there is a dearth of information in the Guinea Savannah region, which is a major transit point for local and international visitors in the most populous nation in Africa. We provide preliminary evidence of dengue activity in the Guinea Savannah and highlight the risk factors.

Determinants of the current and future distribution of the West Nile virus mosquito vector Culex pipiens in Spain.

Changes in environmental conditions, whether related or not to human activities, are continuously modifying the geographic distribution of vectors, which in turn affects the dynamics and distribution of vector-borne infectious diseases. Determining the main ecological drivers of vector distribution and how predicted changes in these drivers may alter their future distributions is therefore of major importance. However, the drivers of vector populations are largely specific to each vector species and region. Here, we identify the most important human-activity-related and bioclimatic predictors

Vectorial transmission of Trypanosoma cruzi: an experimental field study with susceptible and immunized hosts.

The dynamics of vectorial transmission of Trypanosoma cruzi and the level of host (guinea pigs) protection after immunization with attenuated parasites (TCC strain) was studied under natural climatic conditions in an endemic region of northern Argentina. The experimental design included two guinea pig corrals isolated by mosquito netting. One (controls) had 17 healthy and susceptible adult guinea pigs. The other had 19 guinea pigs immunized with attenuated T. cruzi TCC strain. Each corral was colonized in April 1988 with equal-sized populations of Triatoma infestans naturally infected by T. cr

Antibodies to blood stage antigens of Plasmodium falciparum in rural Gambians and their relation to protection against infection.

Cross-sectional and longitudinal studies were performed in a rural population living in The Gambia to examine the relationship between several in vitro assays of the host immune response to asexual stages of Plasmodium falciparum and protection from malaria in vivo. Assays included an enzyme-linked immunosorbent assay for antibodies to schizont antigens; an indirect immunofluorescence assay for total antiblood-stage antibodies; an immunofluorescence assay on glutaraldehyde-fixed parasites to detect antibodies to antigen Pf 155; an assay for serum inhibition of red blood cell invasion; a micro-

Prevention against malaria before the first antenatal visit and absence of anaemia at the first visit were protective from low birth weight: results from a South Kivu cohort, Democratic Republic of the Congo.

BACKGROUND: There is little information on the causes of low birth weight (LBW, <2500 g) in South Kivu. The authors determined the prevalence of LBW among full-term newborns, and its relationship with malaria and anaemia at the first antenatal visit (ANV1) in the rural health zone of Miti-Murhesa, in the eastern Democratic Republic of the Congo. METHODS: Four-hundred-and-seventy-eight pregnant women in the second trimester attending their first antenatal clinic were recruited between November 2010 and July 2011, and followed-up until delivery. Besides information on use of preventive measures

Determinants of malaria vaccine willingness among caregivers of children under five years in Akoko, Southwestern Nigeria.

BACKGROUND: Malaria remains a significant public health challenge in Nigeria, especially among children under five years. The recent introduction of WHO-recommended malaria vaccines offers renewed hope for reducing malaria-related morbidity and mortality, but caregivers' willingness to vaccinate their children depends on their awareness, perceptions and practices. Guided by the Health Belief Model (HBM), this study investigated caregivers' willingness to vaccinate their children against malaria and the factors associated with this willingness in the Akoko area of Ondo State, Nigeria. Low ITN utilization among households in SWEPR Ethiopia: challenge for malaria elimination, a mixed design approach.

INTRODUCTION: To prevent malaria starting from 2005 more than 2 billion insecticide-treated nets (ITNs) have been distributed globally. Even though Ethiopia has been doing nationwide ITN distribution starting from 2016 with 64% ITN coverage, there is a significant gap in its utilization in different settings. Therefore, this study aimed to assess the utilization of ITN and associated factors among households in SWEPRS, 2023. METHODS: Community-based mixed study designs were applied in the present study. Firstly, the malaria area was selected purposively and the study participants were reached
