Magnetic resonance imaging of the upper airway in obstructive sleep apnea before and after oral appliance therapy.

OBJECTIVE: To determine the mechanism by which an oral appliance may be used to treat obstructive sleep apnea syndrome (OSAS). METHODS: Eleven OSAS patients (8 males and 3 females) were involved in the study. Mean age was 52.2 +/- 10.6 years; height was 166.6 +/- 7.2 cm, and weight was 75.6 +/- 9.3 kg; body mass index (BMI) was 27.2 +/- 2.9 kg/m2. Each patient underwent magnetic resonance imaging (MRI) and polysomnography before and after oral appliance therapy. Pharyngeal changes were measured and compared with the variation in sleep parameters. Also, Pearson's correlation and multiple linea

Congenital trismus secondary to masseteric fibrous bands: endoscopically assisted exploration.

The authors present an 18-month-old female infant with congenital trismus. Computed tomography and magnetic resonance imaging were not helpful in determining the cause. A surgical endoscope was used to explore her temporomandibular joints and temporal fossae, thus avoiding the morbidity of a bicoronal incision. The cause was bilateral fibrous bands on the anterior border of the masseter muscles. Incision of these fibrous bands led to relief of the trismus. This finding is consistent with a previously described variant of the Hect-Beals-Wilson trismus-pseudocamptodactyly syndrome. This patient,

Obstructive sleep apnoea. Part II: Treatment with a customised dental appliance.

The use of dental appliances in the treatment of patients with snoring and obstructive sleep apnoea is an important treatment modality for those patients not severe enough for continuous positive airway pressure (CPAP) or who cannot tolerate this form of treatment. A mandibular advancement splint has been specifically designed to eliminate snoring and obstructive sleep apnoea. The appliance's design parameters included ease of insertion, comfort, and maximum effectiveness. Customised appliances have been designed for dentate, semi-dentate, and edentulous patients. To date, over 100 appliances

Sagittal and Vertical Changes After Aligner Versus Acrylic Splint Expansion in Mixed Dentition: A Prospective Trial.

INTRODUCTION AND AIMS: The efficacy of acrylic splint expander (Splint) and Invisalign First aligners (First) in increasing transverse width in mixed dentition have been well documented, but little is known about their sagittal and vertical effects. Since sagittal and vertical changes are particularly critical in growing patients, the objective of this study was to assess and compare the sagittal and vertical treatment effects of First and Splint in mixed dentition with transverse maxillary deficiency. METHODS: Sixty subjects were included after propensity score matching (PSM) from initial 16

Obstructive sleep apnea syndrome indexes in patients treated with oral appliances with different vertical dimension: a preliminary study.

BACKGROUND: Mandibular reposition devices (MRDs) are a viable option for addressing snoring and mild to moderate obstructive sleep apnea syndrome (OSAS) as another possibility to continuous positive airway pressure (CPAP) ventilation during sleep. The aim of the study was to evaluate the efficacy of OSAS MRDs with variation in vertical dimension, considering the OSAS indexes: Apnea Hypopnea Index (AHI), Oxygen Desaturation Index (ODI) and Snoring Index. METHODS: Eleven patients with different level of OSAS were enrolled in this study. The patients in the study were evaluated at three differen

Mandibular advancement device versus CPAP in lowering 24-hour blood pressure in patients with obstructive sleep apnoea and hypertension: the CRESCENT trial protocol.

INTRODUCTION: Although treatment of obstructive sleep apnoea (OSA) using continuous positive airway pressure (CPAP) reduces blood pressure (BP), adherence to CPAP is often suboptimal. A mandibular advancement device (MAD) is a guideline-endorsed alternative therapy for OSA. Still, there is limited evidence on the relative efficacy between MAD and CPAP on BP reduction. We evaluate whether treatment of moderate-to-severe OSA using MAD can improve BP and other health-related outcomes compared with CPAP. METHODS AND ANALYSIS: This is a randomised, controlled, non-inferiority trial conducted. We w

MRI study of a physiotherapeutic protocol in anterior disk displacement without reduction.

Disk displacement of the temporomandibular joint (TMJ) without reduction has been considered a fundamental pathophysiologic abnormality of the joint. In this paper there are supportive clinical and MRI evidence of a successful repetitive manipulative technique for the treatment of acute and chronic locking. In 1500 treated cases, documented on videotape, only five cases required surgical intervention. The success of the therapy was confirmed by 13 MRI images of the TMJ of patients with acute and chronic locking before and after therapy. In all cases the anteriorly dislocated disk in closing an

Investigations on recurrent condyle dislocation in patients with temporomandibular joint dysfunction: a therapeutical concept.

In the last 2 years, 30 patients with recurrent condyle dislocation were examined and treated with two different two-phase procedures according to their main clinical symptoms. The success of both procedures, firstly, pericapsular injection of homogenous blood and, if necessary, elimination of occlusal disharmonies, and secondly, functional treatment with occlusal adjustment, in some cases followed by injection therapy, is outlined. Some suspected etiological factors are investigated. In 14 out of 19 patients in Group I injections were successful, three cases improved, and therapy failed in tw

TMJ fractures in children and adolescents: treatment guidelines.

TMJ fractures must be focused not only as a cause of direct damage to osseous structures, but also of future disturbances of dentofacial development. Treatment is aimed at restoring normal joint function, occlusion and symmetry. Any disturbance of condylar cartilage will result in alteration of mandibular development. The subsequent deformity of jaw and face will depend not only on the type, intensity extent and chronology of the noxious agent, but also on the particular time of occurrence and growth activity. Thus the effect will be more decided if the disturbance occurs early in life, during
