Clinical findings, therapeutic approach, and outcome of brucellar vertebral osteomyelitis.

BACKGROUND: Osteoarticular complications are the most common focal complications of brucellosis. Although vertebral osteomyelitis is the most frequent location in adults >30 years of age, little information is available about this serious complication of brucellosis, and great confusion surrounds its prognosis and the most appropriate treatment. METHODS: We undertook a descriptive, retrospective, observational study of 96 patients who received a diagnosis of brucella vertebral osteomyelitis from September 1982 through December 2005 at a tertiary care hospital. All of the patients were treated

Abdominal tuberculosis in children.

UNLABELLED: Abdominal tuberculosis (AT) is a rare cause of intraabdominal infection. Surgical intervention is rarely indicated, other than obtaining a specimen for histopathological diagnosis or for the treatment of complications. METHODS: The medical records of 14 patients who were operated on after the diagnosis of AT between 1983 and 2000 were reviewed retrospectively. RESULTS: Median age was 7 years (6 months to 10 years). The presenting clinical signs and symptoms were as follows: colicky abdominal pain (9), weight loss (8), abdominal mass (6), vomiting (5), and night fever (5). Two pat

Infectious sacroiliitis: a retrospective, multicentre study of 39 adults.

BACKGROUND: Non-brucellar and non-tuberculous infectious sacroiliitis (ISI) is a rare disease, with misleading clinical signs that delay diagnosis. Most observations are based on isolated case reports or small case series. Our aim was to describe the clinical, bacteriological, and radiological characteristics of ISI, as well as the evolution of these arthritis cases under treatment. METHODS: This retrospective study included all ISI cases diagnosed between 1995 and 2011 in eight French rheumatology departments. ISI was diagnosed if sacroiliitis was confirmed bacteriologically or, in the absen

Challenges and Strategies in Managing Recurrent Clostridioides difficile Infection in Older Adults.

BACKGROUND: Clostridioides difficile infections (CDIs) are caused by a Gram-positive, spore-forming bacillus and are defined by more than three episodes of watery diarrhoea per day. CDI is a major cause of morbidity and mortality in older adults, particularly over 65 years. Recurrent CDI leads to higher mortality and prolonged, debilitating illness. CASE PRESENTATIONS: This article presents two patients, aged over 80 years old, who developed recurrent CDI causing complicated and prolonged treatment courses. Patient 1 required an extended course of antibiotics for treatment of discitis and a c

Clinical profile and diagnosis of extrapulmonary tb in HIV infected patients: routine abdominal ultrasonography increases detection of abdominal tuberculosis.

OBJECTIVE: To study the clinical profile and assess the utility of the procedures performed for the diagnosis of extrapulmonary TB (EPTB) in HIV patients. DESIGN: Prospective observational study of HIV patients suspected to have EPTB. RESULTS: Two hundred and thirty HIV-infected patients were enrolled over 18 months. Of them, 87 cases had active TB, 60 (69%) of whom were of EPTB. Major presenting symptoms were fever (93.3%), weight loss (80%) and cough (61.6%). The most common site of active EPTB was the abdomen (70%), which could be detected due to routine use of abdominal ultrasonography,

Bacteremia with an iliopsoas abscess and osteomyelitis of the femoral head caused by Enterococcus avium in a patient with end-stage kidney disease.

A 70-year-old man on hemodialysis for end-stage kidney disease due to polycystic kidney disease presented with hip pain on extension and a high C-reactive protein level. Further examinations revealed an iliopsoas abscess and femoral head osteomyelitis caused by Enterococcus avium (E. avium) detected in blood and pus cultures. Complete resolution of the infection with ampicillin-resistant E. avium required six months of vancomycin therapy and two surgical drainage procedures. There have been no previous case reports in which both blood and abscess cultures confirmed E. avium infection. Careful

Pneumococcal native aortic valve endocarditis with mycotic abdominal aortic aneurysm, paraspinal and iliopsoas abscesses and pneumonia revealing a multiple myeloma.

We report a rare case of multiple myeloma presenting with native aortic valve endocarditis with secondary embolic mycotic abdominal aortic aneurysm, contiguous paraspinal and iliopsoas abscesses, and pneumonia due to Streptococcus pneumoniae in a Chinese man. He was treated with aortic valve replacement, endovascular stenting of aneurysm, image-guided drainage of abscesses, and a 6-week course of endocarditic antibiotic therapy followed by chronic suppressive antibiotic therapy. Cases of multiple myeloma presenting with invasive pneumococcal infection were reviewed.

Large B-cell lymphoma mimicking iliopsoas abscess following open revision of proximal femur infected non-union: a case report.

BACKGROUND: Extranodal presentation of lymphoma is a rare occurrence. It has been postulated that chronic antigen stimulation may predispose a patient to the development of lymphoma. CASE PRESENTATION: We present a case report of a large extranodal B-cell lymphoma mimicking a postoperative abscess following surgery for an infected proximal femur nonunion in an 80-year-old Caucasian male of Italian descent. CONCLUSIONS: This case highlights the need to consider malignancy in revision surgery, careful examination of operative specimens and the need for further understanding of the role of meta

Assessing Demographic Characteristics, Microbiology, and Mortality Outcomes Associated With Vertebral Osteomyelitis: A National Single-Center Cohort Study in Malta.

Background Vertebral osteomyelitis (VO) is an uncommon but potentially debilitating spinal infection associated with delayed diagnosis, prolonged hospitalization, and significant morbidity. General international consensus indicates that the incidence of VO is increasing, which likely reflects an aging population and an increase in use of advanced imaging. This paper aims to describe the demographic characteristics, risk factors, microbiology, and short-term outcomes associated with VO in Malta. Methods We performed a retrospective single-centre cohort study of adult patients (>18 years old) di

Development and validation of a nomogram for predicting postoperative bone nonunion in spinal tuberculosis patients.

BACKGROUND: Postoperative bone nonunion is a critical complication following instrumented fusion for spinal tuberculosis. Preoperative prediction is essential for prevention. While clinical risk factors exist, current predictive tools lack validation in infected cohorts. PURPOSE: This study developed and validated a multivariate nomogram, provided an individualized preoperative estimate of nonunion risk in spinal tuberculosis patients, incorporating key clinical and radiological predictors to guide preventative strategies. METHOD: A retrospective cohort of 178 patients undergoing debridement

Haematogenous osteoarticular infections in paediatric sickle cell trait patients: A reality in a tertiary centre in West Africa.

BACKGROUND: Sickle cell trait (SCT) affects at least 5.2% of the world population, and it is considered asymptomatic by medical practitioners. There is a paucity of data regarding SCT paediatric patients and haematogenous osteoarticular infections (HOAIs). In our practice, some children with SCT presented HOAIs. This study aims to describe the pattern of HOAIs in children with SCT admitted in our unit. MATERIALS AND METHODS: A single-centre retrospective study of medical records of SCT paediatric patients treated for HOAIs between January 2012 and June 2019 was performed. The data extracted w

Whole-body PET/CT reveals systemic dissemination in spinal tuberculosis: findings from the Spinal TB X cohort.

INTRODUCTION: Spinal tuberculosis (STB) is a severe form of extrapulmonary tuberculosis associated with diagnostic delay and substantial morbidity. The extent of disease-isolated versus disseminated STB-remains poorly defined. We applied whole-body 18F-FDG-PET/CT to characterise the clinical and radiographic phenotype of STB. MATERIAL AND METHODS: The Spinal TB X cohort (NCT05610098) is a prospective study of HIV-infected and uninfected patients with suspected STB in Cape Town, South Africa. We analysed the first ten patients with microbiologically confirmed STB. Demographic, clinical, microb
