Removal of Long-Term Broken Roots Displaced Into the Maxillary Sinus by Endoscopic Assistant.

PURPOSE: This report presented a patient with 2 long-term broken roots displaced in left maxillary sinus. The residual root fragments made the patient uncomfortable in both mind and body and interfered with prosthodontics work. The application of endoscope combined with piezoelectric device both helps in removing the broken roots successfully with minimally surgical injury and preserves the residual alveolar bone. METHODS: Computed tomography scans and 3-dimensional reconstructions located the broken roots. A 1.0 cm × 1.5  cm rectangle bone window on anterolateral sinus wall was opened by a p

Comparison of 2 clinical techniques for treatment of gingival recession.

BACKGROUND: In early case studies, use of a collagen barrier as a guided tissue regeneration (GTR) material has shown particular promise in procedures aimed at root coverage. The similarities between collagen membrane and subepithelial connective tissue graft (SCTG) have made collagen membrane an attractive and a possible alternative material for root coverage. The purpose of this randomized clinical trial was to compare these 2 techniques, SCTG versus a GTR-based procedure (GTRC), for root coverage/recession treatment. METHODS: Sixteen patients with bilateral Miller's Class I or II (gingival

Connective tissue repair in circumferential periodontal defects in dogs following use of a biodegradable membrane.

A biodegradable polylactic acid membrane was evaluated for its ability to enhance periodontal regeneration. In 8 Labrador dogs, circumferential defects were created around the maxillary 1st premolars. The defects were combination "1-wall vertical and horizontal". A membrane was adjusted to cover the defect on one side of the jaw, while the contralateral tooth served as control without membrane. After 3 months, healing was evaluated histologically. The results demonstrated that the amount of connective tissue repair with newly formed cementum approximated 50% of the defect height for both test

Topographic Evaluation of the Remineralizing Potential of Biomimetic Scaffolds on Enamel White Spot Lesions: An In Vitro Study.

OBJECTIVES: The modern approach to managing noncavitated white spot lesions (WSLs) emphasizes noninvasive strategies and biomimetic remineralization. Biomimetic scaffolds are designed to regenerate dental tissues rather than simply repair them. This study aimed to assess lesion depth, enamel structure, and the elemental composition of artificially induced WSLs after treatment with biomimetic remineralization techniques. MATERIALS AND METHODS: Ninety-six freshly extracted anterior teeth, free from caries or enamel defects and extracted due to periodontal disease, were used. White spot lesions

A critical analysis of research methods and experimental models to study pulpitis.

Pulpitis is the inflammatory response of the dental pulp to a tooth insult, whether it is microbial, chemical, or physical in origin. It is traditionally referred to as reversible or irreversible, a classification for therapeutic purposes that determines the capability of the pulp to heal. Recently, new knowledge about dental pulp physiopathology led to orientate therapeutics towards more frequent preservation of pulp vitality. However, full adoption of these vital pulp therapies by dental practitioners will be achieved only following better understanding of cell and tissue mechanisms involved

Favorable healing following space creation in rhBMP-2-induced periodontal regeneration of horizontal circumferential defects in dogs with experimental periodontitis.

BACKGROUND: Recombinant human bone morphogenetic protein-2 (rhBMP-2) is believed to be capable of inducing periodontal regeneration. However, the risk of aberrant healing events, such as root resorption and ankylosis, has been reported. We hypothesized that implantation of BMP-containing carriers directly on the root planed surface may be the cause of unfavorable healing. The purpose of this study was to evaluate the influence of a 1 mm spacer membrane, which separated the rhBMP-2 in polymer-coated gelatin sponge (PGS) and the root surface, on periodontal regeneration of experimentally induced

Localized bone reconstruction as an adjunct to dental implant placement.

Advances in bone reconstructive techniques, including the potential of barrier membrane usage for osteogenesis, have increased the indications for implant placement. This review discusses the critical biologic and clinical criteria involved in achieving a predictable success with barrier membranes. Experimental and clinical findings have shown that the type of adjunctive grafting material used, healing time of the membrane site, type and size of bone defect, and membrane exposure all influence the end result. Small bony defects like implant fenestrations and intact extraction sites seem not to

Treatment of gingival recession using guided periodontal tissue regeneration.

Gingival recession is an apical displacement of the gingival margin and involves loss of connective tissue attachment and the supporting alveolar bone. Various treatment modalities have been utilized in the treatment of gingival recession in order to eliminate etiologic factors, achieve root coverage, regenerate the lost periodontal attachment, and prevent perpetuation of the disease. During the past two decades, the broad concept of guided tissue regeneration has become specific, identifying the specific treatment involved as guided bone regeneration and guided periodontal tissue regeneration

Treatment of class II furcation defects using porous hydroxylapatite in conjunction with a polytetrafluoroethylene membrane.

The purpose of the present study was to determine whether the use of a porous hydroxylapatite in conjunction with guided tissue regeneration technique influenced the clinical results in the treatment of Class II furcation defects in humans. Fifteen subjects with a mean age of 39.4 +/- 9.3 years were used in this study. All subjects had Class II furcation defects on the buccal surfaces of 2 lower molars. In each subject one furcation defect was filled with granular porous hydroxylapatite and then a barrier of polytetrafluoroethylene periodontal material was positioned over the furcation. The ot

Longitudinal implant stability measurements based on resonance frequency analysis after placement in healed or regenerated bone.

Primary stability is an indicator of subsequent osseointegration of dental implants. However, few studies have compared the implant stability among anatomical regions and bone types; thus, not enough data exist regarding the stability of implants placed in regenerated bone (RB). The present study evaluated primary and long-term stability of implants placed in RB and non-regenerated healed bone (HB). A total of 216 screw cylinder implants were placed in 216 patients (98 in HB and 118 in RB, 6 [RB6, N = 68] or 12 [RB12, N = 50] months after tooth extraction). Implant stability was evaluated usin

From childhood to adulthood: oral rehabilitation of a patient with ectodermal dysplasia.

Ectodermal dysplasia, in all its varieties, occurs in approximately 1 in every 100,000 live births. Depending on the variety, hypohidrosis, hypotrichosis, hypodontia, oligodontia, and/or anodontia may be exhibited. Few long-term clinical reports exist. This report follows the development, growth, and initial treatment of a 10-year-old patient with ectodermal dysplasia and his subsequent oral rehabilitation 20 years later.
