The Prevalence of Pisotriquetral Arthritis in the Setting of Scapholunate Advanced Collapse.

Background Previous authors have reported pisotriquetral pain and subsequent pisiform excision following partial or total wrist fusion in patients with scapholunate advanced collapse (SLAC). Prior studies have not considered the potential role of SLAC biomechanics on pisotriquetral osteoarthritis (PT OA) development preoperatively. Purpose To determine the prevalence and severity of PT OA in patients with SLAC as compared with a control population. Patients and Methods Magnetic resonance imaging (MRI) studies of 24 patients with SLAC wrist and 24 sex- and age-matched control patients were anal

[Arthroscopy of the distal radioulnar joint].

OBJECTIVE: Insertion of a small joint arthroscope into the proximal and distal parts of the distal radioulnar joint (DRUJ) allows visualization of the proximal pouch of the DRUJ, the joint surfaces of the sigmoid notch and the ulnar head, the convexity of the ulnar head and the proximal ulnar-sided surface of the triangular fibrocartilage complex (TFCC). INDICATIONS: Evaluation of joint pathologies in ulnar-sided wrist pain, especially in cases without diagnostic findings in standard X-rays and MRI, suspected cartilage lesions without osteochondral changes (signs of osteoarthritis), undefined

The Prevalence of Focal Carpal Chondromalacia Concurrent With Chronic Dynamic Scapholunate Instability.

PURPOSE: There is a paucity of information regarding the prevalence of focal chondromalacia with chronic dynamic scapholunate instability (CDSI), and current treatment algorithms do not account for its presence. We aimed to determine the prevalence and distribution of chondromalacia in CDSI. METHODS: We conducted a retrospective, longitudinal, cohort study of all wrist arthroscopies performed from January 2008 to December 2020. We included wrists with an arthroscopically confirmed primary diagnosis of CDSI. Patients were grouped based on the presence (CDSI+C) or absence (CDSI-C) of focal carp

Kienböck's disease in 2021.

The cause of Kienböck's disease is still unclear. It was initially considered as osteomalacia, before being recognized as avascular necrosis of the lunate. Its functional prognosis is doubtful, given that the progression often leads to wrist degeneration. Conservative treatment does not provide good results in adults; thus surgery is often needed. Certain anatomical factors such as the ulnar variance, configuration of the lunate or orientation of the radial glenoid have turned out not to contribute to necrosis but may contribute to lunate fracture. The lunate's vascularization can be precariou

A new definition of wrist sprain necessary after findings in a prospective MRI study.

INTRODUCTION: Wrist injuries with negative X-rays are diagnosed as acute wrist sprains. The prognosis is usually good, but some patients suffer from long-lasting pain and reduced wrist function, probably due to missed diagnosis followed by inappropriate treatment. The aim of this study was to investigate acute wrist sprains with MRI to detect the pathoanatomy of the injury. PATIENTS AND METHODS: This prospective magnetic resonance imaging (MRI) study included patients between 18 and 49 years, who attended the Accident and Emergency Department (A&E) Bergen, Norway, after sustaining an acute wr

Current innovations in wrist arthroscopy.

It has become clear that the stability of the scapholunate joint does not depend wholly on the scapholunate interosseous ligament, but rather on both primary and secondary stabilizers, which form a scapholunate ligament complex. Each case of scapholunate instability is unique and should be treated with tissue-specific repairs, which may partly explain why one procedure cannot successfully restore joint stability in every case. Wrist arthroscopy has a pivotal role in both the assessment and treatment of the scapholunate ligament complex derangements. Tears of the foveal attachment of the triang

Is Fixation of the Ulnar Styloid Necessary After Distal Radius Fracture Stabilization?

Introduction Fractures of the distal radius are frequently accompanied by fractures of the ulnar styloid process. Because the triangular fibrocartilage complex attaches to the base of the ulnar styloid and contributes to the stability of the distal radioulnar joint (DRUJ), the optimal management of these fractures remains controversial. While some surgeons advocate fixation of the ulnar styloid fragment to restore joint stability, others believe that fixation may not significantly influence clinical outcomes. Methods This retrospective comparative study included patients presenting with Fernan

Ulnar-Sided Wrist Pain: A Diagnostic Evaluation Guide From 30-Plus Years of Experience.

INTRODUCTION: While multiple ulnar-sided wrist pain (USWP) diagnostic evaluation guides have been presented, none have included original clinical data or statistical analysis. The purpose of this study is to provide a diagnostic evaluation guide derived from original clinical data and analysis to help clinicians arrive at a differential diagnosis for USWP. METHODS: Using a computer search of patients presenting with sprains, instability, and laxity of the wrist, 385 patient charts were identified. Patient demographics, mechanism of injury, subjective complaints, physical findings, and diagnos

Accuracy of Wrist MRI in Detecting Synovitis and Correlation with Arthroscopy.

Hypothesis  Wrist magnetic resonance imaging (MRI) has shown excellent diagnostic accuracy in evaluating soft-tissue pathology of the wrist including ganglion cysts, ligament tears, and triangular fibrocartilage complex (TFCC) pathology. However, it is unclear how often MRI detects synovitis that is subsequently encountered during wrist arthroscopy and may be a source of pain for patients with symptoms unresponsive to conservative treatment. The aim of this study is to assess the diagnostic accuracy of conventional MRI in the detection of wrist synovitis observed intraoperatively with wrist ar

Radiologic Evolution after Scapholunate Dorsal Capsulodesis for Chronic Tears.

Background  Many debates are still ongoing for the management of chronic scapholunate (SL) injuries. We have proposed an arthroscopic technique of dorsal capsulodesis with good clinical results. We now propose a radiological follow-up. Purpose  To determine if arthroscopic dorsal capsulodesis can improve the radiographic SL angle and maintain this correction over time. Methods  From January 2020 to January 2021, we included every patient with an SL instability and sorted them according to the European Wrist Arthroscopy Society (EWAS) classification. All patients had bilateral X-rays with a mea

Radially Based Extensor Retinacular Sling Reconstruction for Extensor Carpi Ulnaris Subsheath Injuries.

PURPOSE: Extensor carpi ulnaris (ECU) subsheath injuries result in ulnar-sided wrist pain and often present concurrently with intrinsic ECU pathology and ulnocarpal compartment injuries. There is a lack of surgical outcome data despite the variety of described ECU subsheath pathologies and reconstructive strategies. METHODS: We retrospectively reviewed our hand-center experience of 33 patients who prospectively underwent radially based extensor retinacular sling ECU subsheath reconstruction by 4 hand surgery-fellowship-trained surgeons between April 2010 and April 2021. Constrained Implant Arthroplasty for Distal Radioulnar Joint Arthrosis: Evaluation and Management of Soft Tissue Complications.

PURPOSE: Distal radioulnar joint (DRUJ) prostheses designed as semiconstrained devices aiming to replace the function of the ulnar head, sigmoid notch of the radius, and triangular fibrocartilage complex have demonstrated the capacity to restore the functional status of the DRUJ. However, soft tissue complications including tendons, nerves, and wounds, although documented, have not been the primary focus of prior reports. This study investigated short- to medium-term soft tissue complications after DRUJ semiconstrained implant arthroplasty. METHODS: We performed a retrospective review of pati

The ECU synergy test: an aid to diagnose ECU tendonitis.

PURPOSE: Chronic dorsal ulnar-sided wrist pain challenges even the most experienced physicians. The close anatomic proximity and the possibility of coexistent injuries can render physical examination maneuvers ambiguous, making it difficult to discern between intra-articular and extra-articular pathology. Using the principle of synergism, we describe the extensor carpi ulnaris (ECU) synergy test as a novel means for diagnosing ECU tendonitis and document its benefit in the clinical setting. METHODS: A retrospective chart review was performed, identifying adult patients experiencing greater th

Hangboard Training as a Preventive Strategy for Upper Extremity Pathologies in Rock Climbers.

CONTEXT: Rock climbing places significant mechanical demands on the fingers, wrists, elbows, and shoulders. The repetitive use of isometric gripping, extreme joint positions, and dynamic movements contributes to increasing upper extremity overuse injuries as the sport rises in popularity. Many of these injuries stem from inadequate finger and forearm strength, leading to compensation with proximal musculature and secondary stabilizers. Hangboarding has become a well-established regimen for improving finger flexor strength, but its role in injury prevention remains poorly defined. EVIDENCE ACQ

Variation in cartilage T2 and T2* mapping of the wrist: a comparison between 3- and 7-T MRI.

BACKGROUND: To analyze regional variations in T2 and T2* relaxation times in wrist joint cartilage and the triangular fibrocartilage complex (TFCC) at 3 and 7 T and to compare values between field strengths. METHODS: Twenty-five healthy controls and 25 patients with chronic wrist pain were examined at 3 and 7 T on the same day using T2- and T2*-weighted sequences. Six different regions of interest (ROIs) were evaluated for cartilage and 3 ROIs were evaluated at the TFCC based on manual segmentation. Paired t-tests were used to compare T2 and T2* values between field strengths and between diff

Suture-button construct for interosseous ligament reconstruction in longitudinal radioulnar dissociations: a biomechanical study.

PURPOSE: Longitudinal radioulnar dissociation is a triad of injuries consisting of distal radioulnar joint disruption, interosseous ligament complex (IOLC) tear, and radial head fracture. This renders the forearm longitudinally unstable, resulting in proximal migration of the radius and ulnar-sided wrist degeneration. We hypothesized that reconstruction of the central band of the IOLC in cadaver forearms using a Mini-TightRope suture-button construct would restore native forearm stability. METHODS: We implanted 8 fresh-frozen cadaver arms with steel beads into the distal radius and ulna, moun
