The superior orbital fissure syndrome (SOFS) has been known to be a condition caused by impairment of the nerves that cross the superior orbital fissure. Traumatic SOFS is an uncommon complication which occurs usually within 48 hours after a facial injury. A 25-year-old male sustained facial trauma following an altercation. Clinical findings on presentation included swelling, ecchymosis, hyphema, subretinal hemorrhage, and mild extraocular movement limitation upon lateral gaze on his right eyelids. Facial computed tomography scan confirmed fractures of the medial walls of the right orbit and h

BACKGROUND: Securing proximal control in complex paraclinoid aneurysm surgery through traditional techniques may be challenging and risky in certain situations. Advancements of anatomical knowledge of the cavernous sinus (CS) and hemostasis have made it more accessible as a surgical option.
OBJECTIVE: To describe the technique of temporary clipping of the horizontal segment of the intracavernous internal carotid artery (IC-ICA) in preparation for permanent clipping of complex paraclinoid aneurysms.
METHODS: Through an extradural pretemporal approach, the lateral wall of the CS is exposed. Th

PURPOSE: The purpose of this study was to localize the cell bodies of palisade endings that are associated with the myotendinous junctions of the extraocular muscles.
METHODS: Rhesus monkeys received tract-tracer injections (tetramethylrhodamine dextran [TMR-DA] or choleratoxin subunit B [CTB]) into the oculomotor and trochlear nuclei, which contain the motoneurons of extraocular muscles. All extraocular muscles were processed for the combined immunocytochemical detection of the tracer and SNAP-25 or synaptophysin for the visualization of the complete muscle innervation.
RESULTS: In all musc

Gaucher's disease is caused by a lack of glucocerebrosidase, and in type 2 various neurological symptoms can occur;however, the detailed pathogenesis of the condition remains undetermined. Here we performed neurophysiological examinations in an 18-month-old girl with Gaucher's disease type 2 to specify the brainstem lesions. Developmental delay occurred from the age of 8 months, and the patient subsequently showed strabismus, loss of head control, failure to thrive and irregular respiration. She was admitted to our hospital at the age of 18 months. A definite diagnosis of Gaucher's disease typ

The cholinergic structures in the cranial nerve motor nuclei and lamina IX of the cervical spinal cord of the rat were examined immunohistochemically with a monoclonal antibody to choline acetyltransferase (ChAT). The brainstem motor nuclei were classified into 3 groups according to the way of distribution of ChAT-positive structures in the neuropil of the nucleus. 1. The oculomotor, trochlear and abducent nuclei contained moderately ChAT-positive perikarya and dendrites. A small number of ChAT-positive bouton-like structures were found in the neuropil, but they were not in contact with ChAT-p

UNLABELLED: CASEs of primary brain stem lesion following closed head injury, verified on CT scan, have been increasingly reported recently. However, most of them have supratentorial lesions in addition to brain stem, resulting in a poor outcome. In this paper, a case of localized brain stem hematoma following closed head injury, is reported based on CT findings.
CASE: A 26-year-old man slipped down on his back and hit the right occipital area with following loss of consciousness for several minutes. Since then, he continued to have gait disturbance and visited our hospital on the next day. On

PURPOSE: To investigate the superior oblique (SO) extraocular muscle cross section in normal controls and in SO palsy using high-resolution magnetic resonance imaging (MRI).
DESIGN: Prospective observational study.
METHODS: At a single academic medical center, high-resolution MRI was obtained at 312 μm in-plane resolution using surface coils in multiple, contiguous, quasi-coronal planes perpendicular to the orbital axis in 12 controls and 62 subjects with SO palsy. Previous strabismus surgery was excluded. Imaging was repeated in central gaze and infraduction. In each image plane along the S

PURPOSE: To compare inferior oblique (IO) myectomy with recession for the treatment of superior oblique (SO) palsy.
METHODS: A retrospective review of medical records identified all patients with SO palsy who underwent IO weakening procedures. Patients were excluded if IO muscle surgery was bilateral, combined with other vertical muscle surgery and if follow up was less than 4 weeks. Good outcome parameters were: improvement of deviation in primary gaze, resolution of diplopia in primary and reading gazes and correction of head tilt to the contralateral side. We analysed subsets of subjects w

Anterior plagiocephaly due to unicoronal craniosynostosis is commonly associated with an ipsilateral hypertropia and a contralateral head tilt. The ipsilateral superior oblique weakness has been attributed to the shortening of the orbital roof with retroplacement of the trochlea, and to heterotopic positioning of the vertical rectus muscles within the orbit. These anatomical aberrations may alter ocular rotations to mimic a true congenital superior oblique palsy. This paper describes two siblings with synostotic plagiocephaly and simulated superior oblique palsy with ocular torticollis success

PURPOSE: To evaluate the adaptive vertical vergence aftereffect and determine whether it contributes to a difference of vertical deviation with respect to gaze distance in patients with vertical strabismus.
DESIGN: Prospective noncomparative studies.
METHODS: Eighty-four patients with unilateral superior oblique palsies were enrolled and classified into three types-A, B, and C-based on the difference in vertical deviation between distant and near viewing. The prism adaptation test was performed for 2 to 3 hours to correct vertical deviation and the response of vertical deviation to the prism

BACKGROUND AND OBJECTIVE: Treatment of lesions located within and surrounding the orbit pose considerable surgical challenges, due to the presence of critical neurovascular structures in such deep, confined spaces. Historically, transcranial and craniofacial approaches have been widely used to deal with orbital pathologies. However, in recent decades we have witnessed the emergence of minimally invasive techniques to reduce morbidity and hospitalization times. Among these techniques are the endoscopic endonasal approach (EEA) and the subsequently developed endoscopic transorbital approach (ETO

INTRODUCTION AND IMPORTANCE: Orbital Apex Syndrome (OAS) are characterized by multiple symptoms, such as impaired eye movement, periorbital pain, and visual disturbance. AS symptoms may be caused by inflammation, infection, neoplasm, or a vascular lesion, potentially involving a variety of nerves, such as the optic, oculomotor, trochlear or abducens, or the ophthalmic branch of the trigeminal nerve. However, OAS caused by invasive aspergillosis in post-COVID patient is a very rare phenomenon.
CASE PRESENTATION: A 43-year-old male with a history of diabetes mellitus and hypertension who had re

BACKGROUND: Middle cranial fossa syndrome refers to a group of neurological signs caused by damage to the oculomotor, trochlear, abducent, and trigeminal nerves and sympathetic innervation to the eye as they cross the middle cranial fossa due to different pathologies. Neurological deficits include external ophthalmoparesis or ophthalmoplegia, ptosis, reduced or absent direct and indirect pupillary light reflex, mydriasis, and reduced or absent corneal and nasal sensation. Neoplasia is the most common etiology in dogs and cats.Canine leishmaniosis is a vector-borne infectious disease caused by
