Endobronchial Schwannoma Presenting as Persistent Cough: Diagnosis and Successful Bronchoscopic Resection.

Schwannomas are benign, slow-growing tumors that arise from Schwann cells of the peripheral nerve sheath. Pulmonary schwannomas are extremely uncommon, accounting for approximately 0.2% of all lung tumors and representing a small fraction of tracheobronchial neoplasms. We present a case involving a 49-year-old non-smoking male with a history of Crohn's disease who reported a persistent productive cough, dyspnea, and a subjective sensation of airway obstruction that did not improve with nebulized therapy. Chest CT revealed an endobronchial lesion obstructing the right mainstem bronchus. Flexibl

Sleep Disorders in the Allergy-Immunology Clinic.

The immunologic consequences of disordered sleep are evident in many disease states, including multiple allergic diseases and disorders cared for in the allergy-immunology clinic. Allergic rhinitis is significantly associated with disordered sleep, most commonly obstructive sleep apnea, which worsens the allergic process itself through chronic intermittent hypoxia and increased inflammatory activation. Patients with asthma also commonly suffer from obstructive sleep apnea, with both conditions worsening nighttime airway resistance. Disrupted sleep decreases the reaction threshold for patients

Treatment-emergent central sleep apnea after initiation of hypoglossal nerve stimulator.

UNLABELLED: Treatment-emergent central sleep apnea occurs when primary obstructive sleep apnea improves with positive airway pressure treatment, but central events emerge or persist. Although documented with positive airway pressure and other treatment modalities, emergence of central sleep apnea has rarely been reported with hypoglossal nerve stimulation. In this case report, we present 2 patients who developed central apnea during hypoglossal nerve stimulator titration with emergence of central events at higher voltages. Central apneas resolved in both patients at follow-up when maintained a

Airway emergency from megaoesophagus: A rare complication of an adjustable gastric band.

A 77-year-old lady with a laparoscopic adjustable gastric band (LAGB), implanted 12 years earlier for obesity, developed an unusual but almost fatal complication, characterised by dysphonia and stridor within minutes and a tensely swollen anterior neck. The condition mimicked haemorrhage into the subcutaneous tissues of the neck, and the airway was secured with an awake fibreoptic intubation. Subsequent computed tomography imaging of the abdomen and chest revealed megaoesophagus with dilatation up to 7 cm, proximal to the gastric band. The band was then deflated percutaneously with immediate r

Mucins: the frontline defence of the lung.

Mucus plays a vital role in protecting the lungs from environmental factors, but conversely, in muco-obstructive airway disease, mucus becomes pathologic. In its protective role, mucus entraps microbes and particles removing them from the lungs via the co-ordinated beating of motile cilia. This mechanism of lung defence is reliant upon a flowing mucus gel, and the major macromolecular components that determine the rheological properties of mucus are the polymeric mucins, MUC5AC and MUC5B. These large O-linked glycoproteins have direct roles in maintaining lung homeostasis. MUC5B is essential f

Therapeutic Approaches to Acquired Cystic Fibrosis Transmembrane Conductance Regulator Dysfunction in Chronic Bronchitis.

Chronic obstructive pulmonary disease is a common cause of morbidity and a rising cause of mortality worldwide. Its rising impact indicates the ongoing unmet need for novel and effective therapies. Previous work has established a pathophysiological link between the chronic bronchitis phenotype of chronic obstructive pulmonary disease and cystic fibrosis as well as phenotypic similarities between these two airways diseases. An extensive body of evidence has established that cigarette smoke and its constituents contribute to acquired dysfunction of the cystic fibrosis transmembrane conductance r

Perianesthesia implications of obstructive sleep apnea.

Obstructive sleep apnea (OSA) is a complex medical condition that affects not only the airway but also the cardiopulmonary, endocrine, and central nervous systems. Obstructive sleep apnea can usually be identified with a focused history and physical examination and is commonly associated with obese, middle-aged men with hypertension and glucose intolerance. A high index of suspicion for OSA should arise when reports of loud snoring, nighttime arousal, and acid reflux accompanied by a history of stroke, atrial fibrillation, or congestive heart failure are elicited during a perianesthesia evalua

[Treatment of acute asthma].

Asthma is a chronic inflammatory disorder of the airways with an increasing prevalence over the last 20 years worldwide. Exacerbation of asthma is defined as rapidly progressive increase in shortness of breath, cough, wheezing, or chest tightness, or some combination of these symptoms and a decrease in expiratory airflow that can be qualified by measurement of pulmonary function such as the peak expiratory flow rate (PEFR) and FEV1. All patients with asthma are at risk of having exacerbations and it has been described previously as acute asthma, asthma or status asthmaticus. A number of medica

Respiratory distress in neonates.

Respiratory distress due to either medical or surgical causes occurs commonly in neonates. It is the most common cause of admission to a neonatal surgical intensive care facility in a tertiary care hospital. The distress can be caused by a variety of clinical conditions; common conditions treated in medical intensive care units are transient tachypnea of the new born, respiratory distress syndrome, pulmonary air leak and pneumothorax. In surgical causes of respiratory distress in neonates the underlying mechanisms include airway obstruction, pulmonary collapse or displacement and parenchymal d

Outpatient management of chronic obstructive pulmonary disease.

The outpatient management of chronic obstructive pulmonary disease (COPD) is designed to limit the decline in respiratory function over time, to relieve the symptoms and improve the patient's functional status, and to manage complications when they arise. Factors that predispose to airway inflammation, including cigarette smoking and respiratory infections, are prevented by behavioral modification programs, measures such as exercise and nutrition to improve general health, and regular vaccination. Symptoms are relieved by bronchodilator and anti-inflammatory therapy, based upon the specific ne

Effects of the combination of skin cooling and hyperpnea of frigid air in asthmatic and normal subjects.

To investigate whether reducing integumental temperature influences pulmonary mechanics and interacts with inhaling cold air, 10 normal and 10 asthmatic subjects participated in a three-part trial in which cooling the skin of the head and thorax and isocapnic hyperventilation of frigid air were undertaken as isolated challenges and then administered in combination. Integumental cooling for 30 min caused airway obstruction to develop in both populations [change in 1-s forced expiratory volume (delta FEV1) asthmatic subjects = 10% ; normal subjects = 6%)]. Hyperventilation, however, only affecte

Cervical exenteration.

Cervical exenteration is a radical operation to remove the larynx, portion of the trachea, and the esophagus, and frequently requires a mediastinal tracheostomy. Highly selected patients with obstructing neoplasms of the esophagus and airway can be palliated and sometimes cured by this aggressive surgical approach. Fatal hemorrhage from pressure or exposure of the innominate artery is avoided by elective division of the artery (preoperative angiograms and intraoperative electroencephalographic control are essential), using the omentum to separate the trachea and great vessels, and removal of a

Fluid and electrolyte considerations in diuretic therapy for hypertensive patients with chronic obstructive pulmonary disease.

When a patient with chronic obstructive pulmonary disease (COPD) requires medical therapy for systemic hypertension, a number of special considerations may affect the choice of antihypertensive drug and subsequent management. Thiazide diuretics have no adverse effect on airway function and are the agents of choice for initial therapy. beta-Antagonists are usually considered first-line agents in antihypertensive therapy, but even relatively cardioselective ones may increase airway resistance in patients with obstructive lung diseases, and they should be used with caution, if at all, in such pat

Jinshui Liujun Decoction attenuates cigarette smoke-induced small airway remodeling via the TGF-β/Smad2/CFTR Axis.

Small airway remodeling is a key pathological feature of chronic obstructive pulmonary disease (COPD), and epithelialmesenchymal transition (EMT) is among the central mechanisms driving this remodeling process. Jinshui Liujun Decoction (JSLJD), a traditional Chinese herbal formula, has shown therapeutic potential in respiratory diseases, but its effects on COPD-associated small airway remodeling remain unclear. In this study, JSLJD significantly reduced collagen accumulation, airway wall thickening, and α-SMA expression in the lungs of cigarette smoke (CS)-exposed mice. In vivo and in vitro, J

Eosinophilic Airway Diseases: From Pathophysiological Mechanisms to Clinical Practice.

Eosinophils play a key role in airway inflammation in many diseases, such as allergic and non-allergic asthma, chronic rhinosinusitis with nasal polyps, and chronic obstructive pulmonary disease. In these chronic disabling conditions, eosinophils contribute to tissue damage, repair, remodeling, and disease persistence through the production a variety of mediators. With the introduction of biological drugs for the treatment of these respiratory diseases, the classification of patients based on clinical characteristics (phenotype) and pathobiological mechanisms (endotype) has become mandatory. Effect of continuous positive airway pressure treatment on permeability, inflammation and mucus production of human epithelial cells.

Continuous positive airway pressure (CPAP) therapy is the gold standard treatment for obstructive sleep apnoea, which affects millions of people worldwide. However, this therapy normally results in symptoms such as dryness, sneezing, rhinorrhoea, post-nasal drip, nasal congestion and epistaxis in the upper airways. Using bronchial epithelial (Calu-3) and nasal epithelial (RPMI 2650) cells in an in vitro respiratory model, this study, for the first time, investigates the effect of CPAP positive pressure on the human respiratory epithelial mechanisms that regulate upper airways lubrication chara

Herbst appliance effects on pharyngeal airway ventilation evaluated using computational fluid dynamics.

OBJECTIVE: To evaluate the effect of a Herbst appliance on ventilation of the pharyngeal airway (PA) using computational fluid dynamics (CFD). MATERIALS AND METHODS: Twenty-one Class II patients (10 boys; mean age, 11.7 years) who required Herbst therapy with edgewise treatment underwent cone-beam computed tomography (CBCT) before and after treatment. Nineteen Class I control patients (8 boys; mean age, 11.9 years) received edgewise treatment alone. The pressure and velocity of the PA were compared between the groups using CFD based on three-dimensional CBCT images of the PA. RESULTS: The ch

Vegetable dust and airway disease: inflammatory mechanisms.

Exposure to cotton or grain dust causes an obstructive bronchitis in certain subjects, mechanisms of which are poorly understood. A difficulty encountered in discerning mechanisms of this airway disease is the lack of knowledge of the active components of these dusts. Clinical features suggest common but not exact mechanisms of the airway disease associated with these vegetable dusts. Human and animal studies show evidence of acellular and cellular inflammatory mechanisms of the bronchoconstriction and inflammation associated with these disorders. Potential cellular sources include alveolar ma

Solitary fibrous tumour of the parapharyngeal space - a case report.

We present a case of a female patient with solitary fibrous tumour (SFT) originating in the parapharyngeal space which was successfully resected using an open transoral approach. The patient presented with a worsening airway obstruction, obstructive sleep apnoea, and dysphagia for three years. Rhinolaryngoscopy revealed a right nasopharynx and oropharynx lateral pharyngeal wall submucosal mass. Histopathological analysis was consistent with an SFT. This uncommon site of a rare tumour often presents diagnostic challenges or leads to overt misdiagnosis. We interrogated the literature in respect
