Incisional Infiltration of Liposomal Bupivacaine in Geriatric Hip Fracture Surgery: A Randomized Parallel-Group Trial.

BACKGROUND: Hip fracture volumes are rising with population aging, and inadequate analgesia following hip fracture surgery has been shown to impede early mobilization and recovery. Liposomal bupivacaine (LB) may extend analgesia, but previous orthopaedic trials have yielded mixed results. We therefore tested whether adding local incisional infiltration of LB to multimodal analgesia confers clinically relevant advantages in geriatric hip fracture surgery. METHODS: This study was a single-center, prospective, randomized, participant and assessor-blinded, parallel-group exploratory trial in olde

Effects of Topical Administration of Bupivacaine HCl to the Ovarian Pedicle on Postoperative Pain, Antioxidant Enzyme Activity and Inflammatory Cytokines in Dogs Undergoing Laparoscopic Ovariectomy.

BACKGROUND: Laparoscopic ovariectomy (LOE) is a common procedure for elective sterilization in dogs and is associated with less tissue trauma than open techniques. However, manipulation of the ovarian pedicle may still induce nociceptive stimulation and contribute to perioperative inflammatory responses and changes in antioxidant enzyme activity. Topical administration of local anaesthetic to the ovarian pedicle may improve perioperative analgesia in minimally invasive surgery. OBJECTIVES: To evaluate the effects of topical administration of bupivacaine HCl to the ovarian pedicle on postopera

The analgesic effects of quadratus lumborum block versus caudal block for pediatric patients undergoing abdominal surgery: a systematic review and meta-analysis.

BACKGROUND: Since children cannot express pain, postoperative pain treatment for them is relatively lacking. In this meta-analysis, we compared the postoperative analgesic effects of quadratus lumborum block (QLB) and caudal block (CB) in surgeries involving the lower abdomen, inguinal region, and urogenital system in children. OBJECTIVE: This review examined the postoperative analgesic effects of QLB and CB in pediatric patients (0-18 years of age) undergoing abdominal surgery. The primary endpoint was the rate of postoperative rescue analgesia, defined as the proportion of patients who retu

Use of Bilateral Rectus Sheath Blocks in Open Umbilical Hernia Repair.

BACKGROUND: In recent years, the rectus sheath block (RSB) has become increasingly prevalent in laparoscopic surgery. However, there is currently no definitive research on its use in the open repair of umbilical hernias with cirrhotic ascites. OBJECTIVE: In this study, we assessed the safety and clinical efficacy of ultrasound-guided (US-guided) bilateral RSBs in open umbilical hernia repair for patients diagnosed with cirrhotic ascites. STUDY DESIGN: Seventy-two patients diagnosed with umbilical hernias that presented with cirrhotic ascites and who were admitted to our hospital were randoml

Effect of Intravenous Dexamethasone on Postoperative Pain in Patients Undergoing Total Knee Arthroplasty: A Systematic Review and Meta-Analysis.

BACKGROUND: Postoperative pain after total knee arthroplasty (TKA) is intense and remains an unsolved problem. Some studies show that perioperative, multimodal analgesia, including intravenous dexamethasone, can provide a better analgesic effect; however, the validity of studies has raised concerns and questions remain around the efficacy, dosing, and safety of dexamethasone in patients undergoing total knee arthroplasty. OBJECTIVES: The purpose of this systematic review and meta-analysis was to evaluate the impact of intravenous dexamethasone on postoperative pain among patients undergoing T

Comparison of 0.25% Ropivacaine for Intraperitoneal Instillation v/s Rectus Sheath Block for Postoperative Pain Relief Following Laparoscopic Cholecystectomy: A Prospective Study.

INTRODUCTION: As Laparoscopic Cholecystectomy (LC) is not a totally pain free procedure, with the pain being most intense on the day of surgery and on the following day. Various techniques are available for postoperative pain relief like intraperitoneal instillation of local anaesthetics and rectus sheath block (RSB)which may provide effective pain relief. AIM: To compare the efficacy of preemptive administration (initiated before the surgical procedure) of intraperitoneal instillation and rectus sheath block using ropivacaine for postoperative analgesia after laparoscopic cholecystectomy. The identification of potent nonopioid analgesics and their potential for perioperative use.

Development of potent nonopioid analgesics (NOAs) has attracted great attentions from both academic and industrial worlds, aiming at replacing current opioid drugs in acute and chronic pain management. Among the diverse proposed pathways toward nonopioid analgesia, voltage-gated sodium channel stood out as one of the most promising targets in developing potent NOA. Diverging from the prevailing focus on highly subtype-selective blockers, we report here the access of potent analgesia via the simultaneous inhibition on multianalgesic-related sodium channel subtypes. Originated from AI-driven dru

An Individualized, Interdisciplinary Approach to the Perioperative Care of Patients on Buprenorphine.

PURPOSE OF THE REVIEW: This review provides an overview of the pharmacodynamics and pharmacokinetics of buprenorphine followed by a review of the published research on the perioperative use of buprenorphine for acute postoperative pain, and analgesia management in patients already on buprenorphine. RECENT FINDINGS: Current evidence comparing buprenorphine with full opioid agonists is heterogeneous, showing either modest superiority or comparable analgesic efficacy, with a reduced incidence of certain adverse effects such as respiratory depression. Despite concerns that its high receptor affin

Beyond the Operating Room: Ultrasound-Guided Tibial Nerve Block for Plantar Wound Care in a Desert Ultramarathon.

Severe plantar lesions are common during multistage desert ultramarathons and may require debridement and sequential dressing in austere field clinics. These procedures can be intensely painful because blistering, maceration, epidermal loss and sand contamination expose highly sensitive plantar tissue. We describe 3 runners treated during the Marathon des Sables who required advanced plantar wound care and received ultrasound-guided tibial nerve block for procedural analgesia. In each case, an isolated tibial nerve block was performed at the medial ankle using 8 mL of 1% lidocaine without epin

The effect of scheduled metamizole on opioid consumption after cardiac surgery.

This retrospective study evaluates the impact of implementing a standardized scheduled metamizole dosing protocol within a multimodal analgesia approach after cardiac surgery. The results showed that scheduled metamizole administration was associated with lower opioid consumption, while maintaining adequate pain control and safety. Pain scores measured by the Numeric Rating Scale improved from 1.12 pre-protocol to 0.89 post-protocol (p < 0.0001). Mean opioid consumption decreased from 119.51 mg morphine equivalents to 95.91 mg (p < 0.0001). No cases of clinically relevant agranulocytosis or pe

Bilateral erector spinae plane block on opioid-sparing effect in upper abdominal surgery: study protocol for a bi-center prospective randomized controlled trial.

BACKGROUND: Erector spinae plane block (ESPB) is a promising technique for effective analgesia. It is still uncertain if ESPB offers the same opioid-sparing effect as thoracic paravertebral block (PVB) in midline incision for upper abdominal surgery. METHODS: The study is a prospective, bi-center, randomized, controlled, non-inferior trial. One hundred fifty-eight patients scheduled for upper abdominal surgery will be randomly assigned to receive bilateral ESPB or PVB before surgery. The primary outcome will be the equivalent cumulative analgesia dosage of sufentanil during the surgery, which
