Safe working zones using the minimally invasive lateral retroperitoneal transpsoas approach: a morphometric study.

PURPOSE: The minimally invasive lateral retroperitoneal transpsoas approach is a recent technique developed for lumbar interbody fusion and discectomy. The proximity of the retroperitoneal vessels and ventral nerve roots to the surgical pathway increases the risk of injury to these anatomical structures. A precise knowledge of the regional anatomy of the lumbar plexus is required for safe passage through the psoas muscle. Preoperative examination of the axial MRI images will allow the surgeon to observe the neural structures at the operative levels and confirm that abdominal vessels do not obs

Low donor-to-recipient weight ratio does not negatively impact survival of pediatric heart transplant patients.

A major limitation to success in pediatric heart transplantation is donor organ shortage. While the use of allografts from donors larger than the recipient is accepted, the use of undersized donor grafts is generally discouraged. Using the UNOS database, we wanted to evaluate whether using smaller donor hearts affects the short- and long-term survival of pediatric heart transplant patients. A retrospective analysis of data entered into the UNOS database from April 1994 to May 2008 was performed. Pediatric heart transplant recipients (ages 0-18 yr) with DRWR <2.0 were identified and divided int

Impairment of coronary flow reserve in orthotopic cardiac transplant recipients with minor coronary occlusive disease.

OBJECTIVE: Coronary occlusive disease is the major long-term complication after cardiac transplantation. The relation between minor angiographic abnormalities and myocardial perfusion has not been previously assessed in a large number of cardiac transplant patients. DESIGN: Prospective study. Coronary flow reserve was measured with an intracoronary Doppler flow probe in the proximal left anterior descending coronary artery in each patient. A dose of intracoronary papaverine producing maximal vasodilation was then administered. SETTING: A regional cardiothoracic centre and a supraregional tra

Effects of unilateral castration and unilateral cryptorchidism of the Holstein bull on plasma gonadotropins, testosterone and testis anatomy.

The effects of unilateral castration (UC) and induced unilateral cryptorchidism (CR) on plasma hormones and testis anatomy were studied in 36 Holstein bulls altered at either 3, 6 or 9 mo of age (n = 12). Plasma hormone concentrations were determined in six samples collected at hourly intervals on d 0, 1, 3, 7, 14 and 30, and then at monthly intervals through 6 mo after gonadal manipulation. Although plasma testosterone (T) showed a transient decrease (P less than .05) immediately after treatment, mean plasma concentrations of luteinizing hormone (LH) and T were unaffected by UC or CR over the

Subtrochanteric Shortening Osteotomy Provides Superior Function to Trochanter Slide Osteotomy in THA for Patients With Unilateral Crowe Type IV Dysplasia at a Minimum of 3 Years.

BACKGROUND: Performing THA in patients with high-riding developmental dysplasia of the hip (DDH) is associated with serious complications and technical challenges. Various methods of shortening osteotomy are available to facilitate femoral head reduction during THA in patients with high-riding hips; subtrochanteric shortening osteotomy and trochanteric slide osteotomy plus proximal shortening are the most common techniques. However, which approach is superior remains a topic of controversy. QUESTIONS/PURPOSES: (1) Is there any difference in clinical outcomes (defined as the Harris Hip Score [

Midterm outcomes of revision anterior fusion versus cervical disc arthroplasty in patients with prior single-level anterior cervical fusion.

BACKGROUND CONTEXT: Revision cervical surgery presents unique challenges due to altered anatomy and biomechanics from the previous fusion. Revision anterior cervical discectomy and fusion (ACDF) offers the benefit of immediate stability but further reduces cervical mobility and may increase stress on remaining segments, potentially leading to additional degeneration. By performing a cervical disc arthroplasty (CDA) in the presence of a prior ACDF, a hybrid surgery (HS) construct is created, potentially combining the benefits of fusion stability with CDA motion preservation. PURPOSE: To compar

Preliminary experience with a novel facet-based lateral mass drill guide for the placement of lateral mass screws compared to freehand technique: a cadaveric study.

BACKGROUND CONTEXT: Lateral mass screw fixation is the standard for posterior subaxial cervical fixation. Several freehand surgical techniques for placing lateral mass screws have been described which rely on anatomical landmarks and surgeon mastery of the technique to safely place screws. The accuracy of these freehand techniques is inherently variable and can be influenced by a surgeon's level of clinical experience. A novel technique was developed that utilizes the plane of the facet joint to create lateral mass screw pilot holes parallel with the joint line to improve the safety and accura

Development and validation of a pregnant phantom with adjustable fetal presentation for region-specific fetal radiation dose assessment during pelvic radiography.

BACKGROUND: Accurate assessment of fetal radiation dose during diagnostic X-ray examinations remains challenging because commercially available pregnancy phantoms do not adequately reproduce fetal anatomy or permit organ-specific dose measurements. PURPOSE: The purpose of this study was to develop a pregnant phantom with adjustable fetal presentation and to evaluate its anatomical realism and applicability for fetal dose assessment during pelvic radiography. METHODS: The developed phantom comprised an anthropomorphic phantom, a whole-body fetal phantom, acrylic components, and water-filled p

Automated Segmentation of Stellate Ganglion Block Region in Ultrasound Images Using Deep Learning Model.

BACKGROUND: The stellate ganglion region is densely vascularized and innervated, making the stellate ganglion block (SGB) technically challenging under ultrasound, particularly for beginners. Deep learning can segment complex ultrasound anatomy, but its application to SGB has not been systematically assessed. We developed and validated a multilevel feature fusion UNet (MLF-UNet) to automatically delineate the SGB region on ultrasound, aiming to support accurate needle placement and improve procedural safety. METHODS: In this retrospective study, 370 patients who underwent ultrasound-guided SG

Comparative Evaluation of Depth of Irrigant Penetration Using Different Irrigation Systems: An In Vivo Study.

INTRODUCTION: Effective irrigation is essential for successful root canal treatment because mechanical instrumentation alone cannot adequately clean the complex anatomy of the root canal system. Various irrigant activation techniques have been developed to enhance irrigant penetration into the apical third and anatomically inaccessible regions; however, clinical evidence comparing their penetration efficiency remains limited. The aim of our study was to compare the depth of irrigant penetration achieved using conventional manual irrigation, passive ultrasonic irrigation, sonic irrigation, air-

The Role of 3D Endoanal Ultrasonography (3D-EAUS) in the Evaluation of Perianal Fistulae-A Retrospective Observational Study at a Tertiary Care Centre.

PURPOSE: Perianal fistulae are sequelae of suppurative anal disease. Preoperative imaging provides a detailed evaluation of fistula anatomy and anal sphincter involvement. This helps in determining surgical approach and prevents complications such as recurrence and incontinence. Three-dimensional endoanal ultrasonography (3D-EAUS) has emerged as a novel technique in fistula imaging in recent times. The aim of this study was to evaluate the diagnostic utility of 3D-EAUS in perianal fistulae by identifying the preoperative imaging characteristics and their comparison with intraoperative findings

Multi-modal direct LINAC parameter prediction for pancreatic VMAT: An optimization-free approach.

BACKGROUND: Pancreatic cancer Volumetric Modulated Arc Therapy (VMAT) planning presents a significant dosimetric challenge due to the high-dose gradients required to spare adjacent, radiosensitive organs at risk (OARs) like the stomach and duodenum. This anatomical complexity has limited the scope of automated planning for this site. Broadly, while deep learning (DL) has been introduced to streamline treatment planning, most existing models only predict intermediate outputs, such as dose distributions or fluence maps, which still necessitate a subsequent, computationally expensive inverse opti
