Occurrence of intracerebral aneurysm in an HIV-infected child: a case report.

A 12-year-old female diagnosed with human immunodeficiency virus infection at age 1 was admitted to our hospital with complaints of headache, left hemiparesis, and blurry vision. On admission, her CD4+ count was 97 cells/mm(3) (13%) and her viral load 44,783 ribonucleic acid copies/mL. Treatment had been initiated 3 months before admission with lopinavir/ritonavir, lamivudine, and stavudine. A computed tomographic scan and magnetic resonance imaging of the head revealed an aneurysm of the right middle cerebral artery. A head angiographic scan was consistent with intracranial arteritis. Six mon

Fenestration of the lamina terminalis as a valuable adjunct in aneurysm surgery.

OBJECTIVE: Hydrocephalus, vasospasm, and frontobasal injury are common complications after aneurysmal subarachnoid hemorrhage (SAH) from anterior communicating artery aneurysms. Previous studies have suggested that fenestration of the lamina terminalis (FLT) during surgery may be associated with reduced rates of shunt-dependent hydrocephalus and vasospasm. We report 106 patients affected by anterior communicating artery aneurysms and Fisher Grade 3 aneurysmal SAH and the affect of FLT on shunt-dependent hydrocephalus, vasospasm, and frontobasal injury. METHODS: During a 3-year period, 53 pati

[Effects of modified anaesthesia for patients undergoing brain artery aneurysm repair operation].

55 patients (age 45-63 years old) scheduled for elective brain artery anaeurysm repair operation were randomly divided into two groups: thiopental, atracurium besilate and fentanyl group (group I; n= 27) and propofol, vecuronium bromide and fentanyl in a continuous intravenous administration group (group II; n=28). In group II little changes of arterial blood pressure during the operation were observed. Also the mean time for patient's awaiking was significantly shorter which allowed effective postoperative evaluation of neurological status. In group II only 1 patient required administration o

[Ruptured thoracic aortic aneurysm presenting as massive epistaxis; report of a case].

An 80-year-old woman was transported to the emergency room at our hospital by the ambulance, suffering from massive epistaxis. As the patient was hemodynamically stable, otolaryngologist attempted to perform tamponade using nasal packing. However, the patient's condition deteriorated, with the development of dyspnea and a reduction in oxygen saturation to 90%. A chest X-ray performed at this point revealed left lung consolidation and pleural effusion while chest computed tomography (CT) demonstrated that rupture of a descending aortic aneurysm with periaortic hematoma and extension of hemorrha

[Long-term results after elective and emergency surgery of abdominal aortic aneurysm].

INTRODUCTION: Abdominal aortic aneurysm can be repaired by elective procedure while asymptomatic, or immediately when it is complicated--mostly due to rupture. Treating abdominal aneurysm electively, before it becomes urgent, has medical and economical reason. Today, the first month mortality after elective operations of the abdominal aorta aneurysm is less than 3%; on the other hand, significant mortality (25%-70%) has been recorded in patients operated immediately because of rupture of the abdominal aneurysm. In addition, the costs of elective surgical treatment are significantly lower. OBJ

[False aneurysm and bleeding caused by a secondary dislocated lesser trochanter fragment].

A 92-year-old woman incurred an unstable pertrochanteric hip fracture with avulsion of the lesser trochanter (type 31-A2 according to the AO classification). The fracture was treated by gliding nail osteosynthesis, without fixing the minimally displaced lesser trochanter. No intra- or postoperative complications were detected. Suddenly, after 30 days, a swelling of the proximal femoral region, accompanied by signs of haemorrhage, occurred.CT-scans showed a false aneurysm of the deep femoral artery and a dorsal laceration of the artery proximal to the aneurysm. X-rays showed a further dislocati

Idiopathic aneurysm of inferior vena cava associated with retroperitoneal ganglioneuroma: case report.

Idiopathic aneurysm of the inferior vena cava is rare. Many aneurysms are asymptomatic; others are diagnosed after complications develop. Ganglioneuroma is an infrequently occurring benign tumor preferentially located in the mediastinum and retroperitoneum. Generally the clinical manifestations are symptoms of compression related to growth of the mass. We present a case report of idiopathic aneurysm of the inferior vena cava associated with retroperitoneal ganglioneuroma. The tumor was resected in combination with a procedure to correct the aneurysm that has not previously been described, ie,

Ruptured renal artery stump aneurysm in a renal autotransplanted Behçet's disease patient.

A recurrent aneurysm at the anastomosis site or the remaining artery frequently occurs after the operative treatment of an aneurysm in Behçet's disease despite anti-inflammatory medication. Herein, a ruptured left renal artery stump aneurysm in a patient with Behçet's disease, who received a left nephrectomy, aorto-biiliac bypass and heterotopic autotransplantation of the right kidney for the treatment of an abdominal aortic aneurysm and renal hypertension one year prior to this admission, is reported. An aneurysm and rupture occurred despite the administration of anti-inflammatory medications

Image-based computational simulation of flow dynamics in a giant intracranial aneurysm.

BACKGROUND AND PURPOSE: Blood flow dynamics are thought to play an important role in the pathogenesis and treatment of intracranial aneurysms; however, hemodynamic quantities of interest are difficult to measure in vivo. This study shows that computational fluid dynamics (CFD) combined with computed rotational angiography can provide such hemodynamic information in a patient-specific and prospective manner. METHODS: A 58-year-old woman presented with partial right IIIrd cranial nerve palsy due to a giant carotid-posterior communicating artery aneurysm that was subsequently coiled. Computed ro

Familial association of basilar bifurcation aneurysm and moyamoya disease--four case reports.

Four patients presented with familial intracranial aneurysms and familial moyamoya disease, including one patient with both familial intracranial aneurysm and moyamoya disease. Basilar bifurcation aneurysms were present in two patients, moyamoya disease in one, and both basilar bifurcation aneurysm and moyamoya disease in one. These events are most likely to arise from different genetic abnormalities associated with basilar bifurcation aneurysm and moyamoya disease.

Stent-graft treatment of a large internal carotid artery vein graft aneurysm.

Aneurysm degeneration of internal carotid artery interposition vein grafts can occur, and their repair can be complicated by repeated neck dissection and difficulty in obtaining distal artery control. We used a covered stent graft to exclude a large internal carotid artery interposition vein graft aneurysm. Retrograde flow was induced in the internal carotid artery so that an embolization protection balloon could be placed through the aneurysm. The stent graft was then placed with embolization protection. The repair was performed without complication, and the aneurysm remained successfully exc

Ruptured middle cerebral artery aneurysm with intramural myxoid degeneration in a child.

A 9-year-old girl presented with a subarachnoid hemorrhage. Cerebral angiography showed a saccular aneurysm arising from the horizontal portion of the right middle cerebral artery. The aneurysm was successfully clipped, and the dome of the aneurysm was excised for pathological study. Histological examination of the aneurysm wall showed that the entire thickness of the wall showed an increased myxoid degeneration. No dissection was present. Most intracranial aneurysms in childhood are believed to be of the saccular type similar to that in adults, but the pathogenesis of the aneurysm formation r

Treatment of a spontaneous intracranial dissecting aneurysm with stent-assisted coil embolization.

Arterial dissection is now recognized as an important cause of stroke. Most reported dissections involve the extracranial portion of the internal carotid artery (ICA) and vertebral arteries. Spontaneous dissecting aneurysms of the intracranial ICA are uncommon. Endovascular treatment for cerebral aneurysms has become widespread; however, the dissecting aneurysm is still difficult to treat if the parent artery has to be preserved. More recently, stenting has been advocated for use with endosaccular coiling, with the coils held in place by the stent. We herein report a spontaneous intracranial I

Aneurysm of the right ventricular outflow following bovine valved venous conduit insertion.

A case of aneurysm of the right ventricular outflow tract is described after repair of tetralogy of Fallot using a Contegra supported conduit. Angiograms revealed that the aneurysm was located between the ventricular anastomosis and the proximal ring of the conduit confirming echocardiographic data. Because the conduit between the rings was not dilated, the valve was perfectly functioning. Pulmonary anastomosis was severely stenosed explaining the dilatation seen below. Conduit replacement with resection of the aneurysmal part of the failing conduit was performed. Supported conduits do not eli

[Left renal infarction due to dissecting aneurysm of the renal arterial branch].

Primary renal artery dissection is a rare condition that causes renal infarction and renovascular hypertension. We describe a case of a 48-year-old man who was admitted for left renal infarction due to a primary dissecting aneurysm of renal artery. He was diagnosed by a contrast enhanced abdominal CT scan and a renal angiography demonstrating a dissection at the branch of the left renal artery. He was treated with conservative medical management, including anticoagulation therapy. Forty-five primary renal artery dissections in 38 patients from previous reports were analyzed. Based on this anal

Rapidly enlarging iliac aneurysm secondary to listeria monocytogenes infection: a case report.

Infected aneurysms caused by Listeria monocytogenes are rare. Worldwide, 16 cases have been reported, none in the iliac system. The authors report the case of an 80-year-old man being followed for small aortic and right common iliac artery (RCIA) aneurysms who presented with progressive gastrointestinal symptoms. Serial computed tomography demonstrated a 200% increase in RCIA diameter with development of infection over 1 month. Right axillobifemoral bypass and aneurysm resection were performed. The authors believe this case represents the first description of bacteremic seeding of an iliac deg
