Accuracy of Weightbearing CT Scans for Patient-Specific Instrumentation in Total Ankle Arthroplasty.

BACKGROUND: Total ankle arthroplasty (TAA) is a popular and viable option for end-stage ankle arthritis. Posttraumatic arthritis is the most common etiology of ankle arthritis, which creates the additional challenge of osseus deformity. Accuracy and reproducibility in placing the implant on the mechanical axis has been shown to be paramount in all joint arthroplasty including total ankle replacement. Patient-specific preoperative navigation is a relatively new technology for TAA, and up until this past year has been based off of nonweightbearing (NWBCT) or simulated weightbearing computed tomo

Range of Normal and Abnormal Syndesmotic Measurements Using Weightbearing CT.

BACKGROUND: Early recognition of syndesmotic instability is critical for optimizing clinical outcome. Injuries causing a more subtle instability, however, can be difficult to diagnose. The purpose of this study was to evaluate both distal tibiofibular articulations using weightbearing computed tomography (CT) in patients with known syndesmotic instability, thereafter comparing findings between the injured and uninjured sides. We also aimed to define the range of normal measurement variation among patients without syndesmotic injury. METHODS: Patients with unilateral syndesmotic instability re

Outcome of Percutaneous Fixation of Calcaneal Fractures: A Prospective Analysis in an Indian Population.

Displaced intra-articular calcaneal fractures can be difficult to treat. Open surgical techniques are associated with wound complications, whereas nonoperative management leads to arthrosis. In the present study, 23 displaced intra-articular calcaneal fractures in 19 patients were treated with closed reduction and percutaneous Kirschner wire fixation. Sanders and Essex-Lopresti classification systems were used. We studied anatomical (Gissane and Bohler angles and width of calcaneus) and functional (Maryland Foot Score and American Orthopaedic Foot and Ankle Society Ankle-Hindfoot Score) outcom

Endoscopic Resection of Sinus Tarsi Ganglion Extended to the Anterolateral Ankle.

Sinus tarsi ganglia are believed to develop from the degeneration of surrounding ligaments or subtalar joint and may associate with sinus tarsi syndrome. Surgical excision of the ganglion and debridement of the sinus tarsi is indicated for symptomatic ones recalcitrant to conservative treatment. The purpose of this technical note is to describe the technical details of endoscopic resection of sinus tarsi ganglion extended to the anterolateral ankle, which is a combination of endoscopic ganglionectomy and debridement of the sinus tarsi via the same set of portals. This minimally invasive approa

Arthroscopic Management of Anterior Ankle Synovial Osteochondromatosis.

Surgical treatment has been recommended as the first choice of therapy in second and third stages of the disease. Complete synovectomy and removal of all loose bodies is advisable for prevention of recurrence of the disease. Recently, arthroscopic treatment of synovial chondromatosis is gaining popularity because of its favorable outcomes and very low complication and recurrence rates. Arthroscopic approach also allows management of concomitant pathology. The purpose of this Technical Note is to describe the technique of arthroscopic synovectomy and removal of loose bodies as treatment of syno

Aneurysmal Bone Cyst of the Lateral Malleolus Treated with Intralesional Curettage and Masquelet Technique: A Case Report.

An aneurysmal bone cyst is a locally destructive lesion considered to be a pseudotumor arising from the bone. Although this benign-like lesion is generally considered rare, several approaches for treatment have been presented. We report a case involving a 15-year-old female diagnosed with aneurysmal bone cyst of the lateral malleolus. Applying the Masquelet technique enabled the treatment of the lesion without causing instability to the ankle joint and the prevention of recurrence through the application of polymethylmethacrylate. To our knowledge this is the first documented case in our regio

Uncontacted tire explosion causing trauma to bilateral lower extremities: A case report.

It is uncommon for tire explosion related injuries on the lower extremity. The bilateral lower extremities were injured by tire explosion when the patient was seated in a bus. She sustained an open fracture with partial bone loss in the right calcaneus (a comminuted fracture in the right ankle joint) and a closed comminuted fracture in the left tibia and fibula. This damage was caused by uncontacted tire explosion, thanks to a thick floor between the exploded tire and the patient's feet. This type of injury on lower extremity caused by uncontacted tire explosion was uncommon.

Remitting Seronegative Symmetrical Synovitis With Pitting Edema: Appearance on FDG PET/CT.

Remitting seronegative symmetrical synovitis with pitting edema (RS3PE) is a rare condition in the elderly and can appear as a first presentation of various types of rheumatic and malignant diseases. We presented a 62-year-old man with the diagnosis of RS3PE based on the clinical sign and laboratory data. Because of the possibility of associated malignancies in RS3PE, FDG PET/CT was performed to exclude occult tumors. The images showed multiple, symmetrically, diffusely increased F-FDG uptake in the soft tissue around joints and bones in the shoulders, hips, knees, and ankles.

Understanding Frontal Plane Correction in Hallux Valgus Repair.

Hallux-abducto-valgus or "bunion" surgery is one of the most common surgical procedures for the foot and ankle specialist. As our understanding of the hallux-abducto-valgus deformity has grown, it is becoming clear that the anatomic CORA of the deformity may lie at the tarsometatarsal joint. There is also the component of the 3-dimensional nature of the deformity that may be best addressed at this CORA. With these issues in mind, it was necessary to address the shortcomings of the traditional Lapidus procedure and progress toward more consistent, instrumented steps that could address the 3-dim

Dysplasia epiphysealis hemimelica of the ankle: a fracture-like rare developmental disorder.

Dysplasia epiphysealis hemimelica is a rare developmental disorder which affects the epiphyses. We report a case of the disease located in the ankle joint, referred to our clinic with the initial misdiagnosis of a Salter-Harris 3 type fracture of the distal epiphysis of the tibia. After correct diagnosis, the patient was treated surgically with the excision of the cartilaginous masses. Taking an accurate medical history and performing adequate imaging studies is essential in diagnosing and treating this disease. Fracture-like epiphyseal configurations in patients with no history of injury shou

Plantar plate tears: a review of the modified flexor tendon transfer repair for stabilization.

Forefoot pain is one of the most common presenting problems in a foot and ankle practice. One of the most common presenting problems, yet most commonly missed problems, is a plantar plate tear. Often the problem is considered to be potential neuroma, fat pad atrophy, or a generalized diagnosis of metatarsalgia or metatarsal head overload. Unfortunately, not enough attention is placed on the plantar and medial/lateral ligamentous structures of the metatarsal-phalangeal joints. This lack of attention results in poor diagnosis, lack of care, treatment for the wrong condition, and ultimate frustra

Avascular necrosis of the talus.

Avascular necrosis (AVN) of the talus has always been a surgical challenge because the talus is hidden by its anatomic location and has a precarious blood supply. Most cases (75%) of talar AVN are traumatically induced in association with talar body and talar neck fractures.AVN of the talus can be a significant problem because collapse of the talar dome leads to degenerative changes and pain and disability of the ankle and subtalar joints. Although there are many published treatments for posttraumatic AVN of the talus, critical outcome studies are still lacking.

Cystic fibrosis and hypertrophic osteoarthropathy in children. Report of three cases.

Three patients with cystic fibrosis were noted to have swelling of knee and ankle joints during exacerbation of their lung disease. Synovial fluid was analyzed in one patient and the synovium underwent biopsy in another. These studies excluded other causes of arthritis but did not contribute any new information on the nature of secondary hypertrophic osteoarthropathy. Radiological examination of long bones confirmed the diagnosis of hypertrophic osteoarthropathy in all three patients. Since many patients with cystic fibrosis survive longer, more instances of hypertrophic osteoarthropathy are e

Radiosynovectomy in the therapeutic management of arthritis.

Radiosynovectomy is a well-established therapy in arthritis and involves an intra-articular injection of small radioactive particles to treat a synovitis. In Europe, frequent indications are rheumatoid and poly-arthritis. Especially in Germany radiosynovectomy is the second common therapy in Nuclear Medicine with about 40,000-60,000 treated joints per year. In Spain, USA, Turkey, Argentines and Philippines the therapy is more use in hemophilic arthritis with excellent results. Especially in developing countries with low availability of clotting factors, the radiosynovectomy represent a cost ef

Radiologic analysis of the effect of tocilizumab on hands and large joints in children with systemic juvenile idiopathic arthritis.

OBJECTIVES: To assess the efficacy of tocilizumab for preventing damage to the joints of systemic juvenile idiopathic arthritis (sJIA) patients, we examined serial radiographs of the hands and large weight-bearing joints of these patients before and after treatment with this agent. METHODS: Nine patients with sJIA receiving 8 mg/kg of tocilizumab intravenously every 2 weeks were studied. The mean follow-up period was 82 months. The number of active joints and laboratory markers of inflammation were assessed before and after tocilizumab treatment, together with radiologic evaluation of the hip

Multiple joint and skeletal patterning defects caused by single and double mutations in the mouse Gdf6 and Gdf5 genes.

Growth/differentiation factors 5, 6, and 7 (GDF5/6/7) represent a distinct subgroup within the bone morphogenetic protein (BMP) family of secreted signaling molecules. Previous studies have shown that the Gdf5 gene is expressed in transverse stripes across developing skeletal elements and is one of the earliest known markers of joint formation during embryonic development. Although null mutations in this gene disrupt formation of some bones and joints in the skeleton, many sites are unaffected. Here, we show that the closely related family members Gdf6 and Gdf7 are expressed in different subse

Conservative treatment measures in hemophilic arthropathy.

Intraarticular bleeding and muscle bleeding occur spontaneously or as a result of trauma in hemophilia A or B. The most common sites for hemarthrosis are the knees joints, elbows and ankles joints. Resorption of intraarticular blood induces reactive hemophilic synovitis. Hyperplasia of the synovium can be followed by recurrent bleedings. These early reactions can change to a chronic synovitis and cause cartilage damage, finally resulting in a complete destruction of the joint. Since 1981 158 adults and 61 children with a hemophilia A or B were treated at the Center for hemophilic disorders Fra

Recovery from perturbation is highly individual and similar between different perturbation types and metrics: A dynamical systems perspective of flexibility in walking gait.

BACKGROUND: Flexibility refers to how individuals can adopt alternative solutions to regain or maintain regular walking patterns when constraints are modified. Flexibility is an important yet still emerging characteristic of motor coordination. RESEARCH QUESTIONS: In walking gait, what are the characteristics of flexibility? METHODS: Twenty male participants (age 21-30) walked on a split-belt treadmill. Sudden perturbations in the form of belt accelerations or decelerations (direction) during mid-stance phase were applied at pseudo-random intervals (20-30 stride cycles). Kinematic data of th
