Long-term effects of nasal continuous positive airway pressure on vasodilatory endothelial function in obstructive sleep apnea syndrome.

Obstructive sleep apnea syndrome (OSAS) is associated with a dysfunction of vascular endothelial cells. The aim of this study was to investigate long-term improvement of endothelial dysfunction in OSAS with nasal continuous positive airway pressure (nCPAP) treatment. We investigated endothelium-dependent and endothelium-independent vasodilatory function in patients with OSAS using the hand vein compliance technique. Dose-response curves to endothelium-dependent vasodilator bradykinin were obtained in 16 subjects with OSAS before and after 6 months of nCPAP therapy and in 12 control subjects wi

Exercise tolerance, exercise hyperpnea and central chemosensitivity to carbon dioxide in sleep apnea syndrome in heart failure patients.

BACKGROUND: Sleep apnea syndrome (SAS) and exercise hyperpnea are common in patients with chronic heart failure (CHF), and although it is not known whether they are both regulated by the same mechanisms, the hypothesis of the present study was that they are related to augmented central chemosensitivity. METHODS AND RESULTS: The oxygen desaturation index (ODI) was evaluated in 29 patients and those with ODI > 5 times/h underwent polysomnography. Patients with an apnea-hypopnea index (AHI) > 15 /h without evidence of obstructive apnea were defined as central SAS (CSAS). Cardiopulmonary exercise

Percutaneous computed tomography-guided transdiscal low cervical cordotomy for cancer pain as a method to avoid sleep apnea.

Rheumatoid arthritis (RA) is a systemic inflammatory form of arthritis characterized by joint inflammation, pain, swelling, and stiffness. While contemporary treatment strategies based on early diagnosis, aggressive treatment, and regular monitoring have helped a significant number of patients achieve evidence-based treatment goals, RA still presents substantial management challenges to both clinicians and patients, and has the potential to lead to severe disability over time. In addition to its significant clinical consequences, RA has important economic implications. Both direct and indirect

Increased incidence of cardiovascular disease in middle-aged men with obstructive sleep apnea: a 7-year follow-up.

The incidence of a cardiovascular disease (CVD) was explored in a consecutive sleep clinic cohort of 182 middle-aged men (mean age, 46.8 +/- 9.3; range, 30-69 years in 1991) with or without obstructive sleep apnea (OSA). All subjects were free of hypertension or other CVD, pulmonary disease, diabetes mellitus, psychiatric disorder, alcohol dependency, as well as malignancy at baseline. Data were collected via the Swedish Hospital Discharge Register covering a 7-year period before December 31, 1998, as well as questionnaires. Effectiveness of OSA treatment initiated during the period as well as

[Preoperative nasal continuous positive airway pressure treatment as substitute for protective tracheostomy in severe obstructive sleep apnea syndrome].

OBJECTIVE: Preoperative nasal continuous positive airway pressure(NCPAP) treatment was used as a substitute for protective tracheostomy before UPPP surgery in 42 patients with severe obstructive sleep apnea syndrome(OSAS) from 1994 to 1996. METHODS: Fifty-two patients operated on three years before this period served as control group. All cases were diagnosed as severe OSAS by polysomnography (PSC). RESULTS: There were no statistical difference in main parameters between two groups. Seven cases in the control group underwent protective tracheostomy whose apnea and hypopnea index(AHI) was 28.

[Examination of low palatal arch with long low hanging soft palate in obstructive sleep apnea syndrome and cephalometry].

In order to clarify morphological abnormalities of the upper airway in patients with obstructive sleep apnea syndrome (OSAS), and to predict the severity as well as the effect of surgery, we statistically examined the relationship between the degree of low palatal arch and the result of cephalometry in 45 adult male patients with OSAS who underwent uvulopalatopharyngoplasty. The patients were divided into 3 types, A, B and C, according to the degree of low palatal arch on inspection, and type C was regarded as having a low palatal arch with a long low-hanging soft palate. It was found that (1)

[Paradoxical respiratory response, changes in heart rate and skin circulation in progressive isocapnic hypoxia in sleep apnea patients].

In 73 patients with sleep apnea syndrome, lung function tests, night-time polysomnography for determining the apnea index (AI), hyperoxic progressive hypercapnia tests under rebreathing conditions, and, after a short pause, isocapnic progressive hypoxia tests were carried out. The objective of these examinations was to study peripheral and central chemosensitivity, changes in heart rate and capillary cutaneous circulation under isocapnic hypoxia in patients with sleep apnea syndrome (SAS). The following linear relationships were presupposed for the evaluation: for peripheral chemosensitivity b

Outpatient treatment of sleep apnea syndrome with CO 2 laser, LAUP: laser-assisted UPPP results on 46 patients.

The pharyngeal airway obstruction during sleep in the obstructive sleep apnea syndrome (OSAS) can be improved after treatment by laser-assisted uvulopalatoplasty (LAUP), a new technique performed under local anesthesia. It permits surgery to be performed without hospitalization or general anesthetic. LAUP has many advantages over the traditional uvulopalatopharyngoplasty (UPPP). It is simple, reliable, hemostatic, sterilizing, and relatively painless. Our experience with LAUP in sleep apnea syndrome is described in 46 patients from December 1988 to May 1993. Among 40 patients classified as suc

[Effects of nasal continuous positive airway pressure (NCPAP) on nocturnal renal function in obstructive sleep apnea syndrome (OSAS)].

Nocturnal renal function was examined in 8 patients with obstructive sleep apnea syndrome (OSAS) and the effects of nasal CPAP (NCPAP) on renal function were also studied. Nocturia was observed more than twice in all cases when no treatment was performed, but it disappeared after initiation of NCPAP. Fractional nocturnal urine volume and creatinine clearance decreased significantly from 1.36 +/- 0.15 ml/min to 0.75 +/- 0.20 ml/min (p less than 0.01) and from 116.8 +/- 46.5 ml/min to 101.1 +/- 33.0 ml/min (p less than 0.05), respectively, after initiation of NCPAP. Expansion hyoidplasty: a potential surgical procedure designed for selected patients with obstructive sleep apnea syndrome. Experimental canine results.

Some patients with "idiopathic" obstructive sleep apnea syndrome have an inspiratory, circumferential, passive collapse of the hypopharyngeal lumen as the mechanism of airway obstruction. Permanent tracheotomy has offered documented dramatic resolution of the airway obstruction with reversal of the associated cardiopulmonary sequelae. Tracheotomy is not, however, without psychological and physical complications detracting from its use. In hope of developing a surgical alternative, the expansion hyoidplasty was conceived. The hyoid bone is transected just medial to each lesser cornu. The trisec

Respiratory pauses and apnea during daytime sleep in normal infants during the first year of life: longitudinal observations.

Respiratory pauses (3-10 sec in duration), apnea (less than 10 sex in duration), and periodic respiration observed in thoracic respirograms were measured in 226 polysomnograms obtained on 17 normal infants during the first year of life. All subjects had one or more respiratory pauses in a majority of recordings; 35% had respiratory pauses in all recordings; 75% of respiratory pauses were associated with body movement. There is marked intersubject and even intrasubject variability in respiratory pause rates. The range of mean respiratory pause rates among subjects was 2.0 - 14.4/h, and for sing

Association of obstructive sleep apnea with pancreatic cancer in high-risk patients under surveillance in CAPS and PCYST cohorts.

BACKGROUND: Obstructive sleep apnea (OSA) has been linked with increased cancer incidence and mortality. We hypothesized that OSA may be a risk factor for neoplastic progression among patients undergoing pancreatic cancer surveillance. METHODS: Retrospective medical record review of two Johns Hopkins Hospital cohorts undergoing pancreas surveillance; patients with familial/genetic risk enrolled in the Cancer of the Pancreas Screening (CAPS) study and patients with incidentally detected pancreatic cysts, enrolled in the PCYST study. Time-to-progression to pancreatic ductal adenocarcinoma (PDAC

Impact of obstructive sleep apnea on outcomes of acute respiratory distress syndrome: propensity matched analysis of a national cohort.

BACKGROUND: The effects of co-existing OSA on the outcomes of ARDS are unclear. OBJECTIVES: This retrospective study aimed to compare the outcomes and resource utilization rates of OSA patients who develop ARDS with a matched control group. MATERIALS AND METHODS: Adult patients hospitalized between 2016 and 2019 with a diagnostic code for ARDS were identified within the Nationwide Inpatient Sample. Patients with acute cardiogenic pulmonary edema were excluded. By utilizing propensity score calculations, patients with OSA were individually matched with and compared to patients without OSA. Pr

A Case of Late Onset Type1 Narcolepsy Complicated by Sleep Apnea Syndrome in which Cataplexy Improved with CPAP Treatment.

A 73-year-old woman experienced sudden drowsiness and sleep attacks for 10 years, as well as a loss of strength in her legs when laughing. Polysomnography (PSG) showed an apnea-hypopnea index (AHI) of 87.0, indicating severe obstructive sleep apnea (OSA). The Multiple Sleep Latency test showed three sleep-onset rapid eye movement periods, and symptoms of sleep attacks and cataplexy were observed, leading to a diagnosis of narcolepsy. After Continuous Positive Airway Pressure treatment, the AHI decreased to 21.0, and cataplexy occurred only once a month. Late-onset narcolepsy is a rare conditio

Utility of gait biomarkers in screening for obstructive sleep apnea among patients with Parkinson's disease: a cross-sectional study.

BACKGROUND: Obstructive sleep apnea (OSA) is a common but often overlooked comorbidity in patients with Parkinson’s disease (PD), impacting both motor and non-motor symptoms and potentially affecting gait control. Early identification of PD with comorbid OSA (PD-OSA) is crucial for improving patient outcomes. METHODS: This study employed a wearable inertial sensor system to systematically collect spatiotemporal gait parameters and dynamic stability indicators during a standardized Timed Up and Go (TUG) test in 61 PD patients. Patients were categorized into PD-OSA and PD-NOSA groups based on ap
