Torsion of the Vermiform Appendix in an 18-Day-Old Neonate: A Case Report from Romania and Review of the Literature.

Background and Clinical Significance: Torsion of the vermiform appendix is a rare condition with a clinical presentation closely resembling acute appendicitis, while preoperative investigations are of limited value in distinguishing between the two entities. In most cases, the definitive diagnosis is made incidentally during surgery. Case Presentation: The authors present the case of an 18-day-old female neonate who presented with marked abdominal distension, diffuse spontaneous and palpation-induced abdominal pain, guarding, and signs of peritoneal irritation. The clinical manifestations and

Acute Abdomen due to Small Bowel Obstruction by Ascaris Lumbricoides in an 18-Year-Old Male: A Case Report and Review of the Literature.

Intestinal obstruction caused by Ascaris lumbricoides (AL) is rare but clinically significant, particularly in endemic regions. We report an 18-year-old Afghan male who presented with a two-month history of intermittent right lower quadrant pain, worsening over the past week with anorexia, constipation, nausea, and fatigue. Examination revealed generalized abdominal tenderness, involuntary guarding, and silent bowel sounds, suggesting an acute abdomen. Initial diagnosis was complicated appendicitis. After stabilization, laparotomy revealed a small bowel obstruction caused by a mass of AL, whic

A case report of an ileo-appendicular knotting causing strangulated small bowel obstruction.

INTRODUCTION: Ileal strangulation is a rare condition that leads to upper intestinal obstruction. It is difficult to diagnose preoperatively and requires urgent medical and surgical management. We report the management of a case of appendicular knot on the ileum. CASE PRESENTATION: A 52-year-old patient was seen for an occlusive syndrome evolving in a febrile context, with a biological inflammatory syndrome. The diagnosis was mesocoeliac appendicitis. Imaging was not performed due to lack of access. Laparotomy revealed the necrotic ileal loop, strangulated by an inflammatory appendix. An ileo

Ileal Diverticulitis Leading to Small Bowel Obstruction: A Rare Case of Ileal diverticular Disease.

While small bowel diverticulosis, particularly, ileal diverticulitis is rare and often asymptomatic, it can lead to chronic vague symptoms or acute complications. Acute presentations may mimic appendicitis, small bowel obstruction, or colonic diverticulitis but can also appear with atypical symptoms, making diagnosis challenging. In many cases, definitive diagnosis relies on imaging such as abdominal CT, which can identify features like extraluminal air or mass lesions, though surgical exploration may be required in complex cases. Small bowel diverticula are structural outpouchings of the inte

Ultrapotent influenza hemagglutinin fusion inhibitors developed through SuFEx-enabled high-throughput medicinal chemistry.

Seasonal and pandemic-associated influenza strains cause highly contagious viral respiratory infections that can lead to severe illness and excess mortality. Here, we report on the optimization of our small-molecule inhibitor F0045(S) targeting the influenza hemagglutinin (HA) stem with our Sulfur-Fluoride Exchange (SuFEx) click chemistry-based high-throughput medicinal chemistry (HTMC) strategy. A combination of SuFEx- and amide-based lead molecule diversification and structure-guided design led to identification and validation of ultrapotent influenza fusion inhibitors with subnanomolar EC50

Pediatric Ovarian Torsion in an 8-year Old Girl; A Case Report Study.

BACKGROUND: Ovarian torsion in children causes abdominal pain. The clinical symptoms resemble other abdominal diseases, such as appendicitis. It happens when the ovary twists on its ligamentous attachment, which stops blood from getting to the ovary. A fast diagnosis accompanied by a high level of clinical suspicion and immediate surgical intervention is obligatory to save the ovaries and avoid complications. CASE PRESENTATION: In this case study, we describe a case of ovarian torsion in an 8-year-old Iranian girl who came with persistent (non-colicky) right-side abdominal pain. After ultraso

Successful Use of Ultrasound Guided Quadratus Lumborum Block Without General Anesthesia for Open Appendectomy in a Patient with Heart Failure with Reduced Ejection Fraction- A Case Report and Literature Review.

BACKGROUND: Patients diagnosed with Heart Failure with Reduced Ejection Fraction (HFrEF) are at high risk of perioperative cardiovascular complications. While it is important to focus on optimizing their cardiac function, it is also crucial to address and optimize any other modifiable risk factors that could potentially impact postoperative outcome. This also includes careful consideration of anesthetic techniques to suit the patient and facilitate the surgery. However, there is a scarcity of evidence regarding the safety of specific anesthetic approaches for heart failure patients. CASE PRES

Correlation between the presence of a cecal appendix and reduced diarrhea severity in primates: new insights into the presumed function of the appendix.

Increased severity or recurrence risk of some specific infectious diarrhea, such a salmonellosis or Clostridium difficile colitis, have been reported after an appendectomy in human patients. While several other mammals also possess an appendix, the suspected protective function against diarrhea conferred by this structure is known only in humans. From a retrospective collection of veterinary records of 1251 primates attributed to 45 species, including 13 species with an appendix and 32 without, we identified 2855 episodes of diarrhea, 13% of which were classified as severe diarrhea requiring a

Primary Epiploic Appendagitis: A Mimicker of Abdominal Pain.

Epiploic appendagitis is a rare cause of acute lower abdominal pain. Epiploic appendices are fat-filled serosal outpouchings of the cecum and sigmoid colon. Primary epiploic appendagitis (PEA) is characterized by epiploic inflammation caused by torsion of the appendage leading to ischemia or thrombosis of the appendage draining vein. Secondary epiploic appendagitis occurs in association with other inflammatory conditions of the abdomen or pelvis, most commonly diverticulitis. PEA is an important clinical mimicker of more severe causes of acute abdominal pain, such as diverticulitis, appendicit

Benzene Sulfonyl Linked Tetrasubstituted Thiacalix[4]arene for Selective and Sensitive Fluorometric Sensing of Sulfosulfuron along with Theoretical Studies.

Herein, we designed two fluorescent tetrasubstituted benzene sulfonyl appended Thiacalix[4]arene receptors named L1 and L2, which sensitively and selectively detect Sulfosulfuron among other herbicides and pesticides. The detection limit (LOD) was found to be 0.21 ppm and 0.35 ppm, and the enhancement constant (Ks) was determined to be 7.07 X 104 M-1 and 5.55 X 104 M-1 for L1 and L2, respectively. Using the non-linear regression method, the association constant was obtained as 2.1 X 104 M-1 and 2.23 X 104 M-1 whereas, the binding ratio was found to be 1:1 for both L1 and L2, respectively. A Case of a Rare, Incidental Discovery of Fibrous Obliteration of an Appendiceal Diverticulum.

Diverticulosis of the appendix (DA) is rare and frequently found incidentally. Some cases are discovered after presenting with similar symptomatology to acute appendicitis, whereas other cases may be completely silent. Fibrous obliteration (FO) is a histologic finding indicative of cellular proliferation secondary to relapses of subclinical inflammatory processes. We report a case of a 75-year-old female with a history of chronic, intermittent abdominal pains who presented to the general surgery clinic after an abnormal thickening of the appendix was discovered on abdominal and pelvic computed

Improving the solubility of anti-proliferative thieno[2,3-b]quinoline-2-carboxamides.

Thieno[2,3-b]pyridines are a class of compounds known for their potent anti-proliferative activities against a range of human cancer cell lines. In this research, a number of strategies to generate analogues that have improved aqueous solubility whilst retaining the potent anti-proliferative actions, compared to previously-explored compounds in this class, were made. Herein we report the synthesis of 80 novel compounds, comprising two series, all based on the thieno[2,3-b]pyridine core structure. Overall, it was found that introducing alternative heterocycles did not notably improve the solubi

An Enlarged and Infected Prostatic Utricle as a Rare Cause of Lower Urinary Tract Symptoms in Adolescent Males.

Dysuria with lower abdominal pain is a common presentation for a urinary tract infection (UTI), and diagnosis is based on symptoms together with a urinalysis and urine culture suggestive of infection. UTI is uncommon in circumcised males who are not sexually active. When urine culture is negative, alternate diagnoses including, but not limited to, gastroenteritis, severe constipation, appendicitis, or epididymitis need to be considered. In patients with a known urologic history of proximal hypospadias and/or disorders of sexual development, rarer diagnoses also need to be considered. This pape

Half-Sandwich Cyclopentadienylruthenium(II) Complexes: A New Antimalarial Chemotype.

A small library of "half-sandwich" cyclopentadienylruthenium(II) compounds of the general formula [(η5-C5R5)Ru(PPh3)(N-N)][PF6], a scaffold hitherto absent from the toolbox of antiplasmodials, was screened for activity against the blood stage of CQ-sensitive 3D7-GFP, CQ-resistant Dd2, and artemisinin-resistant IPC5202 Plasmodium falciparum strains and the liver stage of Plasmodium berghei. The best-performing compounds displayed dual-stage activity, with single-digit nanomolar IC50 values against blood-stage malaria parasites, nanomolar activity against liver-stage parasites, and residual cyto

Point-of-care ultrasound for the acute abdomen in the primary health care.

Point-of-care ultrasound (POCUS) is a focused examination, which is performed and interpreted at the bedside by the treating physician answering a specific clinical question. It is currently utilized as an essential adjunct to physical examination in many medical specialties. Recent advances in technology have made POCUS machines portable, affordable, and could be used with minimal training even by nonradiologists. This review aims to cover the fundamental physics of POCUS and its applications for diagnosing the acute abdomen in the primary health care including the most common causes encounte

Second-Strand Synthesis-Based Massively Parallel scRNA-Seq Reveals Cellular States and Molecular Features of Human Inflammatory Skin Pathologies.

High-throughput single-cell RNA-sequencing (scRNA-seq) methodologies enable characterization of complex biological samples by increasing the number of cells that can be profiled contemporaneously. Nevertheless, these approaches recover less information per cell than low-throughput strategies. To accurately report the expression of key phenotypic features of cells, scRNA-seq platforms are needed that are both high fidelity and high throughput. To address this need, we created Seq-Well S3 ("Second-Strand Synthesis"), a massively parallel scRNA-seq protocol that uses a randomly primed second-stra

IgG4-Related Appendiceal Disease: A First Case Report Fulfilling All Pathological Diagnostic Criteria and With Concomitant S100-Positive Dendritic/Schwann Cell Hyperplasia.

IgG4-related disease is a recent entity that has been described in a wide variety of organ systems. A 46-year-old female presented with acute appendicitis accompanied by a mass-forming lesion, raising a concern for neoplasm, and therefore, hemicolectomy was performed. The lesion revealed a dense lymphoplasmacytic infiltrate accompanied by storiform fibrosis and obliterative phlebitis. The IgG4/IgG plasma cell ratio was >50%, and the number of IgG4-positive plasma cells was >100/high-power field. In order to assess the IgG4/IgG plasma cell ratio, MUM1 was employed instead of IgG to successfully

The role of appendectomy at the time of laparoscopic surgery for benign gynecologic conditions.

PURPOSE OF REVIEW: The risk-benefit ratio of concurrent appendectomy at the time of gynecologic surgery has long been debated and remains controversial. However, emerging data on the appendix's role in chronic pain syndromes point to a previously unrecognized link between gynecologic disorders and appendicular pathology. In this article, we review the indications for appendectomy at the time of laparoscopic gynecologic surgery for the treatment of endometriosis and chronic pelvic pain. RECENT FINDINGS: The incidence of appendiceal endometriosis is highly variable depending on the patient popu
